Although
gout
is known to cause destruction of bone by the deposition of tophaceous material, the documented cases of pathologic fractures due to
gout
are rare. We report the case of a
pathologic fracture
of the patella. Union of the fracture was seen at 3 months after surgery.

There are relatively few reports concerning management of the musculoskeletal problems of children with renal failure. From a population group of 124 children with renal failure treated at The Children's Hospital, Camperdown, 16 were referred for management of skeletal problems. These problems included genu valgum, slipped capital femoral epiphysis, ankle valgus, procurvatum of the tibia, osteonecrosis, osteochondritis dissecans, "brown tumors,"
gout
, and
pathological fracture
. The methods of management of these problems are discussed.

Introduction. Tophaceous
gout
of the patella is rare and may masquerade as a tumour or tumour-like condition. Cases. We report two patients with
gout
involving the patella, one complicated by a
pathological fracture
and the other occurring in a bipartite patella in a young adult. Discussion. Typical imaging appearances and measurement of serum urate will usually confirm the diagnosis but, occasionally, the serum urate level may be normal in active
gout
and in such cases, a biopsy will be required. Conclusion.
Gout
of the patella may masquerade as a tumour or tumour-like condition and it is important to consider
gout
in the differential diagnosis.

Intraosseous lipomas are one of the rarest bone tumors found in the body. The incidence has been reported to be <0.1% of all primary bone tumors. The differential diagnoses of an intraosseous lipoma in the calcaneus include plantar fasciitis, retrocalcaneal bursitis,
gout
, stress fracture, unicameral bone cyst, aneurysmal bone cyst, osteoblastoma, enchondroma, chondromyxoid fibroma, nonossifying fibroma, giant cell tumor, chondroblastoma, fibrous dysplasia, and chondrosarcoma. It has been reported that 60% to 70% of patients with an intraosseous lipoma present with symptoms. This article describes a case of a
pathologic fracture
secondary to a large intraosseous lipoma, the surgical treatments, and the subsequent resolution of symptoms. The purpose of our report was 3-fold: (1) to increase awareness of intraosseous lipomas and their potential to cause pathologic fractures in the calcaneus; (2) to suggest a possible treatment protocol for intraosseous lipomas in the calcaneus; and (3) to describe a rare case of an intraosseous lipoma of the calcaneus not located exclusively in the neutral triangle.

A 48-year-old man visited the emergency room with right hip pain that started abruptly while walking out of the bathroom. Computed tomography showed an intraosseous mass in the femoral neck. The patient had a 15-year history of
gout
and had numerous bilateral tophi in his hands, feet, knees, and elbows. After operation, we diagnosed a
pathological fracture
due to intraosseous tophi. Patients with hip pain who have many subcutaneous tophi and long-standing
gout
should be diagnosed carefully. Peri-hip joint pain caused by
gout
is uncommon, however, if a patient complains of pain, a simple X-ray may be required. If intraosseous tophi are present, appropriate treatment (e.g., strict hyperuricemia control with or without prophylactic internal fixation), may be required before fracture occurs.
