A total of 152 infants were followed from birth to 1 year of age in a rural community of Egypt to document
Giardia lamblia infection
and to determine the effect of breast-feeding on enteric infections by this protozoan. Asymptomatic Giardia infections persisted as long as 4 months, with a mean duration of excretion of 7.18 weeks. The incidence of asymptomatic infection was 4.5 episodes per child-year. Exclusively breast-fed infants had lower risk for asymptomatic (odds ratio [OR] = 0.66, 95% confidence interval [CI] = 0.45-0.96, P < 0.05) and symptomatic infections (relative risk [RR] = 0.50, 95% CI = 0.27-0.90, P < 0.05). Furthermore, breast-fed infants had fewer clinical manifestations, including mucus in stool (23.8% versus 76.2%, P = 0.08), loss of appetite (17.6% versus 82.3%, P < 0.05), and abdominal tenderness (17% versus 82.9%, P < 0.05) compared with infants who were not exclusively breast-fed. Breast-feeding should be considered as an effective means to prevent Giardia infections and should be encouraged in regions where G. lambia is highly endemic.

This case report details a child with coeliac disease and
giardiasis
. Treatment of the infection without recourse to a gluten-free diet cured the symptoms of diarrhoea and returned the small intestinal morphology to normal. Thus, the patient moved from active to latent coeliac disease. Potential and latent forms of coeliac disease are being increasingly recognized, since markers have become available to identify patients and investigations developed to test for gluten sensitivity in the small intestinal mucosa. This case provides an opportunity to consider potential and latent forms of coeliac disease and how these impact on clinical practice and the wider understanding of the disorder.

In patients with coeliac disease, a regression of intestinal damage without a gluten-free diet is a very rare event. We describe a young child with diarrhoea, intestinal mucosa atrophy and positive serum anti-endomysial and anti-tissue transglutaminase (anti-tTG) antibodies during intestinal
giardiasis
infection. He showed normal intestinal mucosa architecture and negative anti-endomysial and anti-tTG antibodies after his
giardiasis
was cured, although he continued to assume a normal diet. Re-evaluations on a 6-monthly basis showed that he was symptom free, and all haemato-chemical parameters were within normal limits. Three years after the initial diagnosis, a third intestinal biopsy showed: normal mucosa architecture; an increase in the intra-epithelial CD3+ and gamma/delta+ lymphocyte counts; and immunoglobulin-A anti-endomysial antibody detection in the supernatant of the intestinal mucosa culture incubated with gliadin. An active coeliac disease status, with intestinal mucosa atrophy, may regress to a latent coeliac disease status with normal intestinal mucosa histology after removal of the environmental factors that have presumably precipitated mucosa damage. Serum anti-endomysial and anti-tTG antibody behaviour is not a permanent, life-long feature and this must recommend the repetition of anti-endomysial or anti-tTG antibody assays in the same patient whenever coeliac disease diagnosis is again suspected, irrespective of previous negativity.

Giardiasis
was the commonest organic cause in 57 (67.0 percent), either alone or with other parasitic infestations. Other organic causes include gallstones (4.7 percent), urinary infections (4.7 percent), esophagitis/gastritis (3.5 percent) and abdominal tuberculosis (2.3 percent). Single parent, school phobia, sibling rivalry, RAP in other family members and nocturnal enuresis are significant factors associated with nonorganic causes

We tested 53 fixed duodenal biopsy samples from 25 patients with Whipple disease for the presence Giardia and 6 fresh duodenal biopsy samples for Giardia duodenalis DNA by polymerase chain reaction (PCR). We also used histological examination and PCR assay to determine the prevalence of G. duodenalis in duodenal biopsy samples from 150 control patients without Whipple disease. Three of 25 patients with Whipple disease had histological evidence of
giardiasis
, whereas only 1 of 150 control patients was affected (P<.001). By PCR, we found Giardia in 2 of 6 patients with Whipple disease whom we tested, but in only 2 of 150 control patients (P<.001). In a literature review, we identified 15 other cases of coinfection. The occurrence of these diseases may be promoted by a common immune defect or a common source of infection, or infection with one may predispose to infection with the other.

A total of 271 Bedouin, 140 of them younger than 16 years and 110 of them female, were examined as part of a health survey. The Bedouin of the southern Sinai showed evidence of stunted growth. Sixty-six subjects (24 female) were clinically anaemic. This might have been nutritional or secondary to
giardiasis
. Simple nutritional strategies to increase the protein and iron content of the diet might help to prevent these problems. Twenty Bedouin had sensorineural hearing loss that was thought to be autosomal recessive in one family grouping. In addition, ten adults had had an uvulectomy, a traditional means of thirst quenching.

Comparable figures on disease incidence between countries are difficult to attain. We therefore compared risk of infection for Swedes going to other European countries. We took as the numerator the number of imported cases from European countries of campylobacter and giardia infection in the national Swedish surveillance database, and as the denominator, the number of visitors to each country from a commercial database on foreign travel. Risk of infection in tourists was also compared to national incidence figures for a selection of countries. During the 7 year period 1997-2003, 14,829 campylobacter and 1112 giardia infections were diagnosed in Swedes returning from a European country. The travel database contained information on 14,519 overnight trips to such a country during the same period. Risk of campylobacter infection was over 100 per 100,000 travellers to Portugal and Turkey, but only 1/100,000 in Finland. Risk of
giardiasis
was highest in Russia (50/100,000). There appears to be substantial underreporting of campylobacter infection in many European countries. In conclusion, the risk of infection with campylobacter and giardia varies 100-fold across Europe. Returning tourists as a sentinel population are a useful tool to estimate these differences. There are large--and unexplained--differences between the risk for travellers and reported national incidence.

A cross-sectional study to examine the association of
giardiasis
with protein-energy malnutrition, vitamin A deficiency and iron deficiency anaemia was conducted among Orang Asli children in Selangor, Malaysia. A total of 281 children aged 2-15 years were studied. The data were collected using structured questionnaires, anthropometric measurements and laboratory analysis of blood and faecal samples. The results showed that 24.9% of the children were infected with Giardia duodenalis, while 56.5, 61.3 and 15.1% had significant underweight, stunting and wasting, respectively.
Giardiasis
was statistically identified as a strong predictor of significant wasting in this study population.
