Forty-five patients with parasitologically confirmed symtomatic giardiasis were treated with tinidazole. A course of 150 mg twice daily for 7 days cured 14 of 19 patients (74%), and a single dose of 2,000 mg cured 24 of 26 (92%). After the single dose, mild side effects were common including maltase, lassitude, and dizziness. Three probable cases of the so-called "postgiardiac syndrome" were seen. Either dosage of tinidazole initially cleared all stool samples of Giardia lamblia, and most clinical and parasitological failures were first detected a few weeks after the treatment. This emphazises the significance of long follow-up periods.

Go.10213, a new nitroimidazole, was studied in 12 male volunteers for tolerability and in 20 patients with intestinal amoebiasis for antiamoebic activity. Go.10213 was well-tolerated by volunteers up to a dose of 400 mg X 3. Patients also tolerated well the dose of 100-150 mg X 3 for 7 days. In two patients, there were mild and transient side-effects like headache, dizziness, fatigue, etc. No neurological side-effects were observed. There were no significant changes in blood pressure, pulse rate and ECG. The organ function tests did not show any adverse effects of Go.10213 on bone-marrow, kidney, liver, etc. Go.10213, at a dose of 150 mg X 3, showed potent antiamoebic activity in 10 patients with intestinal amoebiasis, as judged by the clinical relief, the eradication of the trophozoites and cysts of Entamoeba histolytica from stools and the healing of colonic ulcers. Go.10213, a novel nitroimidazole, may prove to be the most potent and safe agent against the protozoal infections, e.g. amoebiasis, giardiasis and trichomoniasis.

Ornidazole or tinidazole as a single dose of 1.5 g was given to 100 patients with symptomatic giardiasis. Parasites disappeared from the feces of all patients, usually within 2 or 3 days, and symptoms subsided in 98 cases, usually within 5 days. During an 8-wk follow-up, parasites reappeared in 10 patients, 5 after each drug, between 1 and 7 wk after treatment. The relapses were usually symptomatic, and the probability that they were reinfections was negligible. Ornidazole appeared in serum more rapidly than tinidazole, and the mean elimination half-lives were 10.9 and 11.1 h, respectively. Subjective side effects were equally common after both nitroimidazoles; dizziness was associated with rapid initial absorption. We found no correlation between drug absorption or elimination and therapeutic efficacy. Both drugs are recommendable as a single oral dose of 1.5 g in giardiasis.
