The parasitological, clinical efficacy and tolerability of albendazole in the treatment for both
giardiasis
and
hookworm
infection in a remote Aboriginal population was investigated. Albendazole at a dose rate of 400 mg daily for 5 days was highly effective in reducing
hookworm
egg numbers and both Giardia antigen and cysts. The 36.6% prevalence of Giardia prior to treatment fell to 12% between days 6 and 9, 15% for days 10-17 and rose to 28% between days 18 and 30. Tolerability and clinical efficacy were excellent. The effect of albendazole on
hookworm
was longer lasting than that on Giardia, reducing percent infection from over 76-2% on days 6-9 and zero by day 18-30 despite conditions highly conducive to rapid re-infection. We conclude that albendazole is highly efficacious against both parasites when used as described but that long term community benefit may require additional education programmes to avoid re-infection with Giardia although treatment strategies would seem appropriate for
hookworm

This study aims to determine the prevalence of protein-energy malnutrition and its association with soil-transmitted helminthiases in Orang Asli (Aborigine) children in Selangor, Malaysia. The results obtained from 368 children aged 2-15 years showed that the overall prevalence of mild and significant underweight was 32.1% and 56.5% respectively. The prevalence of mild stunting was 25.6% while another 61.3% had significant stunting. The overall prevalence of mild and significant wasting was 39.0% and 19.5% respectively. The overall prevalence of ascariasis, trichuriasis and
hookworm
infection were 61.9%, 98.2% and 37.0% respectively and of these 18.9%, 23.5% and 2.5% of the children had severe infection of the respective helminthes. The overall prevalence of
giardiasis
was 24.9%. The present study vividly shows that stunting and underweight are highly prevalent among Orang Asli children and therefore of concern in this community. In this population intestinal parasitic infections, especially severe trichuriasis and
giardiasis
, were identified as the main predictors of stunting and wasting respectively, in addition to age between 2 to 6 years.

Infection caused by intestinal parasites is still a common health problem in a poor-hygiene population especially for children in developing countries. A cross-sectional study was conducted among 781 Karen students (age: 3 to 19, males: 325, females: 456) to determine the current status of intestinal parasitic infections in a mountainous area in the North of Thailand. The study was drawn from three schools in the Doi Inthanon region, in Mae Chame district of Chiang Mai province, from December 2002 to June 2003. The techniques used for the diagnosis were: stool concentration by using the 'formalin-ether' technique and perianal region examination by using the 'Scotch-tape' technique. The average rate of intestinal parasites for the group tested using the stool concentration technique was 42.06% (male: 46.87%, females: 38.82%); and 22.66% (males: 28.92%, females: 18.20%) when using the Scotch-tape technique.Among helminth-infected individuals, enterobiasis was found at the highest prevalence (15.49%). Other common infections were ascariasis (9.78%), trichuriasis (5.90%) and
hookworm
infection (2.20%). Strongyloildiasis was found only in 0.92%. For protozoa infection, the major cause is the non-pathogenic species "Entamoeba coli" (27.68%). The other non-pathogenic protozoa (Endolimax nana, Chilomastix meslini and Iodamoeba butschlii) had a low prevalence from ranged 0.18%-4.79%. The prevalence of pathogenic
Giardia lamblia infection
was 2.21%. Entamoeba histolytica infection was found in only one case. Based on the two techniques used, the results from the Scotch-tape provided a higher sensitivity for the detection of Taenia spp. and Enterobius vermicularis eggs. Drug treatment was given to all the infected students. School-based health education should be implemented in order to prevent and control the infections.

One hundred and fifty-three British soldiers and 86 Royal Air Force (RAF) personnel were deployed on a hostage rescue operation in Sierra Leone. For 3 days they were exposed to various infection risks and 6 weeks later some of the soldiers presented with gastrointestinal complaints. Both groups were screened with structured questionnaires, blood investigations and (where indicated) faecal microscopy and charcoal culture for helminths. Definite and probable cases of helminth infection were treated with albendazole and all soldiers were screened again after 3 months. Among the soldiers investigated, 73/145 (50%) reported gastrointestinal symptoms and 70/139 (50%) had eosinophilia. Among these, 17/66 (26%) had
hookworm
infection, 6/66 (9%) had Strongyloides stercoralis infection and 1/66 (2%) had
Giardia lamblia infection
. Eosinophilia was most strongly associated with entering the enemy camp and being in the platoon that attacked the area around the camp latrines. Among RAF personnel, who were not involved in activities on the ground, 3/86 (3%) had borderline eosinophilia. Treatment of 105/153 (69%) soldiers with albendazole was well tolerated and, on follow-up screening 3 months later, 23/124 soldiers (19%) had gastrointestinal symptoms and 18/121 (15%) had eosinophilia. Faecal investigations and schistosomiasis serology tests were all negative at this stage.

The aim of this study was to survey for the current rate of
Giardia lamblia infection
in three different districts in two provinces of Thailand, Surin and Samut Sakhon, in March 2002, October 2003, and March 2004. Two districts are rural areas and another is sub-urban. Volunteers of any age from Surin Province and children aged between 5 to 7 years old from three secondary schools in Samut Sakhon were enrolled for the stool examination. The method used was stool examination by both simple smear and normal saline concentration technique on fresh collected feces. All samples were tested in duplicate. Out of 3,358 healthy individuals from rural Surin Province, 75 cases (2.2%) were found positive for G. lamblia, 30 of which were below 10 years of age. By comparison, 656 individuals from sub-urban Samut Sakhon Province volunteered and 43 (6.5%) were positive for G. lamblia. Other intestinal parasites, both helminth and protozoa, were also identified from these two groups:

In April 2004, an outbreak of acute diarrheal illness occurred among the Orang Asli (aborigine) in the Cameron Highlands, Pahang State, Peninsular Malaysia, where rotavirus was later implicated as the cause. In the course of the epidemic investigation, stool samples were collected and examined for infectious agents including parasites. Soil transmitted helminthes (STH), namely Ascaris lumbricoides (25.7%), Trichuris trichiura (31.1%) and
hookworm
(8.1%), and intestinal protozoa, which included Giardia lamblia (17.6%), Entamoeba histolytica/E. dispar (9.4%), Blastocystis hominis (8.1%) and Cryptosporidium parvum (2.7%), were detected. Forty-four (59.5%) were infected with at least one parasite, 24 (32.4%), 12 (16.2%) and 8 (10.8%) had single, double and triple parasitic infections, respectively. STH were prevalent with infections occurring as early as in infancy. Giardia lamblia, though the most commonly found parasite in samples from symptomatic subjects, was within the normally reported rate of
giardiasis
among the various communities in Malaysia, and was an unlikely cause of the outbreak. However, heavy pre-existing parasitic infections could have contributed to the severity of the rotavirus diarrheal outbreak.

We conducted a cross sectional study to examine the association of intestinal parasitic infections and protein energy malnutrition (PEM) with iron-status indicators and anaemia among Orang Asli children in Selangor, Malaysia. A total of 281 children aged 2 - 15 years were studied. The data were collected using structured questionnaires, anthropometric measurements and laboratory analysis for blood and faecal samples. All children were infected either by A. lumbricoides, T. trichiura or
hookworm
and almost 19%, 26% and 3% of the children had severe infection of ascariasis, trichuriasis and
hookworm
infection respectively. The prevalence of
giardiasis
among them was 24.9%. Overall, 41.5% of the children were anaemic (haemoglobin < 11.0 g/dL). Of these 61.0% of the children had iron deficiency and 36.5% had iron deficiency anaemia (IDA), which accounted for 88.0% of anaemia in this population. Severe trichuriasis had the most significant correlation with anaemia and iron deficiency in this population. It contributed to low concentrations of haemoglobin, serum iron and serum ferritin and high total iron binding capacity (TIBC). Significant underweight and stunting were associated with low concentrations of haemoglobin and serum iron while significant wasting was significantly associated with low concentration of serum ferritin. Logistic regression analysis confirmed that severe trichuriasis was a strong predictor of IDA. It also confirmed that children who were significantly underweight and whose mother was working were independent predictors of IDA in this population.
