In two cases there was simultaneous or sequential occurrence of amebiasis, shigellosis, and giardiasis in male homosexuals. Enteric pathogens may, under the proper conditions, be venereally transmitted. In particular, the sexual practices of male homosexuals, most significantly, oral-anal contact, appear to provide the necesseary link for transmission. Evidence suggests that this is a growing problem.

Stool specimens from 89 high-risk, sexually active homosexual men were screened for enteric protozoal cysts. Patients whose specimens contained nonpathogenic cysts were investigated further by examination of fresh purged specimens. A total of 27 of the initial specimens (30%) contained protozoal cysts. Twenty-three patients (26%) harbored pathogens. There were 18 cases (20%) of amebiasis and 11 cases (12%) of giardiasis. Six men (7%) had concurrent amebiasis and giardiasis. The presence of infections correlated with a history of anilingus but not with place of birth, travel, or history of symptoms. The exceptionally high rate of prevalence of intestinal protozoal infection in this population suggests that enteric protozoal infections are important among sexually transmitted diseases.

Among 10,418 patients of a Prague hospital, a plain infection with intestinal parasitic protozoans was identified in 1,319 persons (12.7%). Of these, 3.5% were infested with Giardia intestinalis, 0.3% with Entamoeba histolytica forma minuta, 5.7% with Endolimax nana. We evaluated the frequency of findings of protozoans in various clinical diagnoses. A statistically significant increase in frequency was recorded for E. nana in diagnoses of eosinophilia, giardiasis, amoebiasis and helminthiasis. A slight increase above the average was found for Entamoeba coli in diagnoses of giardiasis and helminthiasis. Most cases of infection with Entamoeba histolytica were associated with a stay abroad. No increase in the frequency of these protozoans was recorded for patients with diarrhea. An analysis of the results indicated that a nonpathogenic amoeba might participate in the origin of intestinal disorders in man.

Tinidazole has been reported to be highly effective against trichomoniasis, giardiasis and amoebiasis. In vitro, tinidazole is more active against trichomonads than metronidazole and possesses antiprotozoal activity at least comparable to metronidazole against Entamoeba histolytica, Tinidazole gives higher serum levels in animals following oral administration than metronidazole and is well distributed in organs and tissues. When tinidazole or metronidazole is given to healthy volunteers at a dose of 2g orally, the serum level of tinidazole at 48h is considerably higher than that of metronidazole. At 72h tinidazole is still present but metronidazole is not. These pharmacokinetic differences result from the longer half-life of tinidazole. These findings suggest that tinidazole might prove to be more useful than metronidazole in the treatment of protozoal infections when given in once daily oral doses of 2g.

To prevent diarrhea, efforts must be directed toward decreasing environmental contamination of enteropathogens, altering exposure of persons to naturally occurring agents, or to increasing host tolerance to prevalent enteropathogens. A vaccine is feasible in the control of certain enteric infections among high risk groups: typhoid fever and cholera, in areas of hyperendemicity or during an epidemic, shigellosis in institutionalized populations that are at risk to develop illness due to known prevalent serotypes, enterotoxigenic Escherichia coli for US travelers to Latin America, and perhaps rotavirus for all children under 3 years of age. In diarrhea, the most useful form of treatment is an oral glucose/electrolyte solution, which in most cases will prevent dehydration. Lactose containing foods should be removed from the diet early in the course of diarrhea. Antimicrobial agents are necessary in the treatment of typhoid fever, are useful in the treatment of shigellosis, giardiasis, and amebiasis, and are contraindicated in intestinal salmonellosis.

Metronidazole which has been widely used for many years in the treatment of trichomoniasis, amoebiasis and giardiasis, has recently been shown to be active against anaerobic bacteria. Serum, cerebrospinal fluid and tissue concentrations bactericidal for Bacteroides species are attained after usual dosages given orally or intravenously or higher dosages given rectally (suppository). Prospective studies have demonstrated that the addition of metronidazole to regimens for pre-operative bowel preparation, decreases the frequency of postoperative infection and eliminates anaerobic infection. Similarly, anaerobic infection after acute appendicectomy or hysterectomy has been virtually eliminated by metronidazole given before and up to 1 week after surgery. Metronidazole has been successfully used in the treatment of anaerobic infections of the chest, head, gastrointestinal and female genitourinary tract, and of anaerobic septicaemia and bacteraemia. Metronidazole is the most active agent available against obligate anaerobes and is likely to be of major value in the treatment of serious infections due to these organisms. Although the absence of formal comparative trials in many areas of use makes it difficult to clearly state the relative therapeutic efficacy of metronidazole, compared with other drugs such as clindamycin, chloramphenicol or penicillin, it is nevertheless a very effective agent in the treatment and prevention of anaerobic infections.

Because of the form of sexual contact and the number of such contacts among homosexual men, physicians should be watchful for sexually transmitted infections in the homosexual community. Hepatitis B, anorectal gonorrhea, shigellosis, amebiasis, and giardiasis are now known to be among these infections. For treatment to be effective and epidemics avoided, physicians should question affected male patients about their sexual habits and advise abstinence from sexual contact until treatment is finished and cultures or smears are negative.

An open multicentre trial was undertaken in 8 countries to assess the efficacy and tolerance of short course treatment regimens of tinidazole Trichomoniasis: of 859 patients with trichomonal vaginitis given a 2g single dose of tinidazole 717 (95.2%) were cured. Side-effects occurred in 82 patients (9.5%) and in 12 were regarded as severe. Giardiasis: of 74 children with symptomatic giardiasis given tinidazole in a single dose of approximately 50 mg/kg body weight, 65 (88%) were parasitologically and symptomatically cured. 2 (2.7%) complained of side-effects, none of which was severe. Intestinal amoebiasis: of 502 patients, comprising 458 adults and 44 children, with symptomatic intestinal amoebiasis who received tinidazole as a single daily dose on 2 to 3 consecutive days, 477 (95%) were parasitologically cured with complete or marked improvement in symptoms. Side-effects occurred in 50 patients (10%) and in 11 were regarded as severe. Amoebic liver abscess: 82 patients were given tinidazole in single doses of 2g on 3 consecutive days. The response was rated as excellent in 60 and good in 17 (overall cure 93.9%). Side-effects occurred in 9 patients (10.9) and in 2 were regarded as severe. These findings confirm the efficacy and tolerance of short regimens of tinidazole given in single daily doses.

Elevated levels of serum immunoglobulin E (IgE) are found in many parasitic diseases. In order to determine the influence of Entamoeba histolytica on IgE levels of clinically symptomatic patients with intestinal amoebiasis, a controlled study was performed. There was no difference between the serum IgE values from Brazilian patients with amoebiasis and those from age-, race- and sex-matched normal Brazilian controls. Based also on our previous similar studies on giardiasis and Chagas' disease, we conclude that protozoan infections do not elevate serum IgE levels.

Although improved sanitation in the United States has resulted in a general diminution in the prevalence of intestinal parasitic infections, some, such as enterobiasis, are not reduced by improved environmental or water sanitation and others, such as amebiasis and hookworm infection, persist in foci. Other infections, notably giardiasis, have increased due to the travels of American tourists in other countries where environmental sanitation is poor. North American physicians, therefore, may expect at some time to have to treat parasitic infections. Favored chemotherapeutic agents currently are: for amebiasis--metronidazole and diiodohydroxyquin; for giardiasis--metronidazole; for tapeworm infections--niclosamide; for enterobiasis--pyrantel pamoate, piperazine citrate, or pyrvinium pamoate; for ascariasis--piperazine citrate; for stronglyoidiasis--thiabendazole; for trichuriasis--mebendazole; for hookworm infection--pyrantel pamoate; for schistosomiasis japonica--tartar emetic; and for schistosomiasis mansoni--niridazole.
