Advances in ventilation physiology have brought solutions to fundamental problems of thoracic surgery and suggested ways of coping with problems of surgical technique and cilincal pathology. The domain of thoracic surgery should cover all those areas requiring specific surgical approach to endothoracic organs, with particular emphasis on special preoperative and postoperative care of such patients. This applies to thoracic trauma as well. A specialist in general surgery the thoracic surgeon needs additional training in this specific field. Departments of thoracic surgery must be organized as special units of lagre medical centres, each serving an area of no less than one million inhabitants. We are moreover convinced that pediatric cases ought to be dealt with bu a skillful thoracic surgeon regularly working with children. In the field of surgery of

the effects of radicality and the influence of positive mediastinal lymph nodes on surgical outcome, as well as the selection of patients for postoperative radiotherapy are currently under discussion. Operative management of pulmonary tuberculosis is becoming simpler, the disease occurs in atypical forms and secondary complications, above all aspergillosis, are on the increase. The choice of thoracotomy for various intrathoracic interventions calls for greater selectivity. Tracheal resection with primary anastomosis has been fully accepted, whereas prosthetic reconstruction is still subject to major dilemmas. Surgical stabilization of the chest following trauma, as an alternative to internal pneumatic fixation, ought to be considered more often in the future. In the management of

surgery tends to prevail over conservative methods of treatment, wherebu the correct choice of tests for the evaluation of reflux is a matter of great significance. Numerous problems accompanying gastroesophageal resection for carcinoma still await solutions.

The etiology of idiopathic pulmonary fibrosis (IPF) remains poorly understood, but some studies have suggested that cigarette smoking or other occupational or environmental exposures, diabetes mellitus, or
gastroesophageal reflux
may play a role. In this study we evaluated the clinical records of a group of 97 consecutive patients with IPF, and 560 patients suffering 5 different respiratory disorders that were examined as controls: asthma (n=111), chronic obstructive pulmonary disease (n=132), squamous cell
lung carcinoma
(n=118), lung adenocarcinoma (n=101) and patients with otorhinolaryngology problems but without lung disease (n=98). In bivariate analyses male sex, diabetes mellitus and being former cigarette smoker were associated with IPF. After adjusting by these variables, multivariate analysis revealed that type 2 diabetes mellitus [11.3% in IPF patients vs 2.9% in controls, OR=4.3 (95% CI: 1.9-9.8), p<0.0001] was an independent risk factor associated to IPF. Our results provide additional evidence of a putative relationship between DM2 and idiopathic pulmonary fibrosis. Experimental research is necessary for thorough assessment of the pathogenic mechanisms involved in this association.

A 69-year-old woman was referred to a gastroenterology clinic with a 1-year history of protracted nausea and postprandial vomiting. She had a background of gastro-
oesophageal reflux
disease, irritable bowel syndrome and chronic obstructive pulmonary disease with a significant smoking history. Her laboratory work-up including autoimmune screen, coeliac serology and synacthen test were unremarkable. Upper gastrointestinalendoscopy and CT imaging ruled out mucosal and obstructive causes. Gastric emptying studies demonstrated a delayed gastric emptying consistent with diagnosis of gastroparesis. Concurrently, she underwent a CT of the thorax for unresolved consolidation on her chest X-ray. This revealed a locally advanced primary
lung carcinoma
. In this context, with all other causes excluded, her gastroparesis was deemed to represent a paraneoplastic phenomenon. Gastroparesis is a frequent, under-recognised and important complication of cancer.
