Empyema complicating laparoscopic fundoplication is exceedingly rare, as is Capnocytophaga infection in the immunocompetent host, with the exception of gingivitis. We report a 29-year-old healthy man who presented with Capnocytophaga empyema 10 days after uneventful elective, laparoscopic Nissen fundoplication for gastroesophageal reflux disease. The exact mechanism of this complication is not known, but hypotheses, including a mini-Boerrhave's syndrome, can be drawn based on knowledge of the operation, the involvement of Capnocytophaga sp., and a patient history that included severe gingivitis. Because of prompt operative evacuation of the empyema and expedient identification of Capnocytophaga in the empyema fluid, appropriate antibiotic therapy was initiated. The infection was adequately treated, and the patient recovered fully. To the best of our knowledge, this is the first report of such a complication.

Was made a comparative evaluation of the influence of treatment of gastroesophageal reflux disease (GERD) on the course of inflammatory periodontal diseases in patients with combined pathology of periodontal and GERD. Was performed an assessment of GERD symptom severity, the establishment of periodontal status of patients and analysis of the effects of GERD therapy on the course parodontopaty inflammatory genesis (chronic catarrhal gingivitis and chronic periodontitis mild). In patients with inflammatory periodontal diseases, coupled with GERD, was marked by a more pronounced therapeutic effect of combined treatment in comparison with isolated local treatment CDW (HKG and HGPI), which was reflected in the improvement of the parameters of the microcirculation system of periodontal tissues. This suggests the presence of pathogenic link between chronic inflammatory periodontal diseases (CDW) and the presence of pathological gastroesophageal reflux.

Gastroesophageal reflux disease (GERD) is defined by the presence of symptoms induced by the reflux of the stomach contents into the esophagus. Although clinical manifestations of GERD typically involve the esophagus, extra-esophageal manifestations are widespread and less known. In this review, we discuss extra-esophageal manifestations of GERD, focusing on clinical presentations, diagnosis, and treatment. Common extra-esophageal manifestations of GERD include chronic cough, asthma, laryngitis, dental erosions, and gingivitis. Extra-esophageal involvement can be present also when classic GERD symptoms are absent, making the diagnosis more challenging. Although available clinical studies are heterogeneous and frequently of low quality, a trial with proton pump inhibitors can be suggested as a first-line diagnostic strategy in case of suspected extra-esophageal manifestations of GERD.
