In the last 10 years Campylobacter jejuni has emerged as the most frequent cause of bacterial
gastroenteritis
in man. Acute enterocolitis, the most common presentation of C. jejuni infection, can affect persons of all ages. C. jejuni has been found in virtually every country where investigations have been carried out. The frequent finding of dysenteric stools suggests that mucosal damage due to an invasive process analogous to that seen in shigellosis is important in the pathogenesis. Campylobacteriosis in man is mainly a foodborne infection in which foods of animal origin, particularly poultry, play an important role. Epidemiological investigations have demonstrated a significant correlation between the handling and consumption of poultry meat and the occurrence of Campylobacter enteritis. Barbecues appear to present special hazards for infection, because they permit easy transfer of bacteria from raw meats to hands and other foods and from these to the mouth. Milk is sometimes found to be contaminated and consumption of raw milk has caused several outbreaks of campylobacteriosis. Campylobacter can remain viable in fresh cheese for only a short period of time. The organism is also found in shellfish, such as clams. Campylobacter is probably very vulnerable to factors such as high temperatures and dry environments, and also to the presence of
oxygen
in atmospheric concentrations. Therefore, it is assumed that the organism does not persist in products like pelleted feed, meals, egg powder and spices, which are often contaminated by Salmonella. A number of preventive measures on different levels, taken simultaneously, are needed to reduce the incidence of campylobacteriosis in man.

Malignant external otitis (MEO) is still a potentially lethal disease. Early treatment based on a correct diagnosis is the most important single factor in achieving a cure for the disease. The preferred treatment is long-term systemic antibiotics followed by surgical intervention. Hyperbaric
oxygen
therapy may be supplemented in refractory cases. A new fluoroquinolone, Ciprofloxacin, has been successfully used in four cases of MEO which did not respond to the accepted treatment. Ciprofloxacin is active against a broad spectrum of bacteria, including Pseudomonas aeruginosa, and several clinical studies have demonstrated its efficacy in the treatment of urinary tract and soft tissue infections, osteomyelitis, pneumonia, and
gastroenteritis
. This report is the first of which we are aware to document the use of Ciprofloxacin in the treatment of MEO.

Campylobacter jejuni has recently been recognized as an important cause of human
gastroenteritis
in many countries. The clinical features of C. jejuni infections vary from those of a mild
gastroenteritis
to a severe enterocolitis. The most common symptoms of the disease are fever, abdominal pain and bloody diarrhoea. The small intestine is the main site of infection, but the colon may also be involved. The main pathogenesis of C. jejuni appears to be invasion of the wall of the gut as in salmonellosis. Isolation of the organism from faeces requires culture in a selective medium containing antibiotics and incubation under reduced
oxygen
tension at 42 degrees C. Most cases of campylobacter enteritis are sporadic and it is often difficult to confirm their source. Although cross infection between humans occurs rarely, the disease is mainly a zoonosis with many possible routes of infection. Human infections have been associated with the consumption of contaminated food, especially poultry, unpasteurized milk, and water, as well as contact with domestic animals such as dogs and cats. In most cases campylobacter enteritis is a selflimiting disease and therefore decision on treatment should be taken on clinical grounds. When considered necessary, erythromycin is the drug of choice. Information about C. jejuni infection has accumulated rapidly in recent years, but much remains to be learned, especially about its epidemiology.

The authors report a case of refractory non-infectious circulatory shock with catecholamine and massive fluid loading-resistant features related to hypopituitarism. A 76-year-old man was admitted for shock after suffering from
gastroenteritis
for 3 days. He was pale and had sparse axillary and pubic hair and small testes. Right catheterization showed shock with low preload pressure and a low
oxygen
extraction ratio relevant for septic shock. Ultrasound tomography revealed a distended gallbladder due to a stone without peritoneal effusion. A non-inflammatory hydrops of the gallbladder was removed surgically. No microorganism was isolated. Cerebral computed tomography (CT) scan showed a pituitary mass. In the post-surgical period the shock became uncontrollable. Cortisol replacement therapy was instituted and clinical and hemodynamic improvement occurred after 2 h. Hormonal screening on admission before catecholamine administration showed a major decrease in all the hypothalamic-pituitary hormone concentrations. The patient died on day 15 with multiple organ failure. Hypopituitarism, probably owing to pituitary adenoma, was the only disease identified in this case. Hormone replacement therapy dramatically improved the clinical and hemodynamic status, although the role of an abdominal sepsis could not be eliminated. Arguments that pituitary hormone deficiency might increase the hemodynamic consequences of adrenal deficiency are discussed.

therapy, and if there are clinical and radiologic signs of acute pulmonary edema, the diagnosis of ARDS must be considered and a complete intensive care therapy is compulsory in order to alleviate the severe prognosis of this syndrome.

Campylobacters are responsible for increasing numbers of
gastroenteritis
cases in humans as well as miscarriages and diarrhea in farm and domestic animals. Surface waters are potential reservoirs and transmitting vehicles for these bacteria. Subject of analysis were surface water samples collected from the River Odra, Szczecin Lagoon and the Pomeranian Bay in monthly or bimonthly intervals, starting from April 1998. Analysis directed on campylobacters included enrichment in Preston broth prior to plating on CCDA isolation medium and identification, to the species level, by the api CAMPY tests. The detection level of the method was 1 CFU/10 ml. Numbers of total and faecal coliforms were counted according to the national standards. In addition water temperature and water saturation with
oxygen
were measured. Presence of Campylobacter spp., at the level detectable by the method applied, was confirmed in 19.7% of the Odra River, 5.6% of the Szczecin Lagoon and 0% of the Pomeranian Bay surface water samples. The contamination level of the Campylobacter--positive surface water samples did not exceed 10 CFU/ml. Isolation frequency depended on water purity class, counts of total and faecal coliforms and sampling site (p < 0.01). Species dominating in the River Odra and Szczecin Lagoon surface waters were C. jejuni ssp. jejuni and C. coli, respectively.

Salmonella enterica serovar Typhimurium causes human
gastroenteritis
and a systemic typhoid-like infection in mice. Infection is initiated by entry of the bacteria into intestinal epithelial cells and is mediated by a type III secretion system that is encoded by genes in Salmonella pathogenicity island 1. The expression of invasion genes is tightly regulated by environmental conditions such as
oxygen
and osmolarity, as well as by many bacterial factors. The hilA gene encodes an OmpR/ToxR family transcriptional regulator that activates the expression of invasion genes in response to both environmental and genetic regulatory factors. HilD is an AraC/XylS regulator that has been postulated to act as a derepressor of hilA expression that promotes transcription by interfering with repressor binding at the hilA promoter. Our research group has identified four genes (hilE, hha, pag, and ams) that negatively affect hilA transcription. Since the postulated function of HilD at the hilA promoter is to counteract the effects of repressors, we examined this model by measuring hilA::Tn5lacZY expression in strains containing negative regulator mutations in the presence or absence of functional HilD. Single negative regulator mutations caused significant derepression of hilA expression, and two or more negative regulator mutations led to very high level expression of hilA. However, in all strains tested, the absence of hilD resulted in low-level expression of hilA, suggesting that HilD is required for activation of hilA expression, whether or not negative regulators are present. We also observed that deletion of the HilD binding sites in the chromosomal hilA promoter severely decreased hilA expression. In addition, we found that a single point mutation at leucine 289 in the C-terminal domain of the alpha subunit of RNA polymerase leads to very low levels of hilA::Tn5lacZY expression, suggesting that HilD activates transcription of hilA by contacting and recruiting RNA polymerase to the hilA promoter.

Salmonella enterica serovar Typhimurium causes self-limiting
gastroenteritis
in humans and a typhoid-like disease in mice that serves as a model for typhoid infections in humans. A critical step in Salmonella pathogenesis is the invasion of enterocytes and M cells of the small intestine via expression of a type III secretion system, encoded on Salmonella pathogenicity island 1 (SPI-1), that secretes effector proteins into host cells, leading to engulfment of the bacteria within large membrane ruffles. The in vitro regulation of invasion genes has been the subject of much scientific investigation. Transcription of the hilA gene, which encodes an OmpR/ToxR-type transcriptional activator of downstream invasion genes, is increased during growth under high-osmolarity and low-
oxygen
conditions, which presumably mimic the environment found within the small intestine. Several negative regulators of invasion gene expression have been identified, including HilE, Hha, and Lon protease. Mutations within the respective genes increase the expression of hilA when the bacteria are grown under environmental conditions that are not favorable for hilA expression and invasion. In this study, the intracellular expression of invasion genes was examined, after bacterial invasion of HEp-2 epithelial cells, using Salmonella strains containing plasmid-encoded short-half-life green fluorescent protein reporters of hilA, hilD, hilC, or sicA expression. Interestingly, the expression of SPI-1 genes was down-regulated after invasion, and this was important for the intracellular survival of the bacteria. In addition, the effects of mutations in genes encoding negative regulators of invasion on intracellular hilA expression were examined. Our results indicate that Lon protease is important for down-regulation of hilA expression and intracellular survival after the invasion of epithelial cells.

Campylobacter jejuni is a predominant cause of food-borne bacterial
gastroenteritis
in the developed world. We have investigated the importance of a homologue of the periplasmic HtrA protease in C. jejuni stress tolerance. A C. jejuni htrA mutant was constructed and compared to the parental strain, and we found that growth of the mutant was severely impaired both at 44 degrees C and in the presence of the tRNA analogue puromycin. Under both conditions, the level of misfolded protein is known to increase, and we propose that the heat-sensitive phenotype of the htrA mutant is caused by an accumulation of misfolded protein in the periplasm. Interestingly, we observed that the level of the molecular chaperones DnaK and ClpB was increased in the htrA mutant, suggesting that accumulation of non-native proteins in the periplasm induces the expression of cytoplasmic chaperones. While lack of HtrA reduces the
oxygen
tolerance of C. jejuni, the htrA mutant was not sensitive to compounds that increase the formation of
oxygen
radicals, such as paraquat, cumene hydroperoxide, and H2O2. Using tissue cultures of human epithelial cells (INT407), we found that the htrA mutant adhered to and invaded human epithelial cells with a decreased frequency compared to the wild-type strain. This defect may be a consequence of the observed altered morphology of the htrA mutant. Thus, our results suggest that in C. jejuni, HtrA is important for growth during stressful conditions and has an impact on virulence.

The pathophysiological mechanism of Salmonella enterica subsp. enterica serovar Typhimurium (Salmonella typhimurium) induced
gastroenteritis
is controlled by interplay of various cell signaling events. Adherence of this organism through type-1 fimbriae is known to be a vital prerequisite for the establishment of infection. In the present investigation male albino Wistar rats were immunized with purified type-1 fimbriae and challenged intragastrically with S. typhimurium. Electrolyte transport and level of different second messengers were studied in four different groups of animals. Transepithelial fluxes of Na+ and Cl- revealed absorption in immunized-challenged group as observed in case of control and immunized group while secretion was observed in infected group. Ca2+ and 3-0-methyl-D-glucose fluxes did not show any change. Significant increase in the level of intracellular Ca2+, cAMP, membrane form of protein kinase C, prostaglandins, NADPH oxidase, glucose-6-phosphate dehydrogenase, 6-phosphogluconate dehydrogenase, total

However, in the immunized-challenged group, the values of all the parameters were found to be same as that of control as well as immunized groups. Na+, K(+)-ATPase and calmodulin levels were found to be unaltered in all the groups of animals. Thus, the immunization with type-1 fimbriae has been found to be quite effective leading to the prevention of multiple physiologic derangements in isolated ileal cells suggesting the protective role of the fimbriae.
