Twenty patients with gastrointestinal mucormycosis are reviewed. This often fatal opportunistic fungal infection was diagnosed histologically, and was categorized as colonization (five patients), infiltration (seven patients), or vascular invasion (eight patients). There were no fatalities from colonization. In 10 patients, mucormycosis complicated peptic ulcer disease. Seven of these patients had infiltrative or invasive disease. The presentation and operative findings mimicked malignancy in five of these seven patients, and six had successful surgical intervention. The other patient was cured by medical therapy alone. Ten patients had infection associated with other gastrointestinal diseases: post-traumatic peritonitis (four patients), transmural
amoebiasis
(two patients), tuberculosis (one patient),
gastroenteritis
(one patient), gastric carcinoma (one patient) and diabetes (one patient). Eight patients had significant infection and only one survived. In this series, mucormycosis had a less aggressive course when complicating peptic ulcer than when it occurred in association with other gut diseases.

Diarrhoeal disease is a common problem in developing countries. As a result of recent advances in diagnostic methodology, the causative agents can now be identified in most cases of acute diarrhoeal diseases. Enteric bacterial pathogens are the common cause of
gastroenteritis
in developing countries. Appropriate uses of antibiotics in selected cases of diarrhoea will decrease symptoms or reduce faecal shedding of the organism and prevent spread of infection. Antimicrobial agents improve the diarrhoea associated with cholera, shigellosis, enteric fever, enterotoxigenic Escherichia coli, giardiasis,
amoebiasis
, and probably Vibrio parahaemolyticus, and enteropathogenic E. coli. Antibiotics have no role in the treatment of viral diarrhoea or uncomplicated salmonella

. Most of the diarrhoeal diseases are self-limited and the wrong choice of antimicrobial agents will worsen the symptoms. Treatment of gastrointestinal infections with antimicrobials will change intestinal microflora, promote the emergence of resistant strains and overgrowth of potential pathogenic bacteria and fungi. Risks and benefits should be considered before prescribing antimicrobial agents.

A rural development project carried out in Southern Zimbabwe for 5 years was aimed at improving nutrition, combatting diseases, educating villagers about proper hygiene, improving water quality, and assessing the development and nutritional status of children under 5. The community investigated consisted of 10,000 people or 1,439 families with an average of 7 persons per family. The main staple of their diet was maize, and malnutrition was prevalent. Water holes infested with bilharzia were the source of drinking water for both man and animal. The project succeeded in vaccinating 90% of preschool children against whooping cough, diphtheria, tetanus, polio, measles, and tuberculosis. A control district was chosen to compare the developmental data obtained by the Cole Slide Rule Calculator of 229 children under 5 with those of 242 children in the project. Malnutrition was studied in 200 children hospitalized in the children's ward of a district hospital, 1/3 of whom were less than 1 year old.

gastroenteritis
was found in 86% of children under 2. Most over 2 were severely undernourished. A nutritional rehabilitation village called Hutano Village was established in 1982 to function as a nutritional center, staffed by a full-time health worker and an assistant. In the 1st 9 months of its existence, 114 children were taken in, and the mothers received instruction in vegetable gardening, raising chickens and rabbits, hygiene, and family planning. The average attendance runs to 25 children and 15 to 17 mothers. In spite of successful medical intervention in malnutrition cases, the relapse of children into an undernourished state remains a difficult issue, whose cause lies in inadequate water supply, poor soil, lack of resources, and low family socio-economic status.

In acute diarrhea of infancy we distinguish between infectious and noninfectious causes. In the latter we know some autosomal recessive disorders, e.g. the glucose-galactose-malabsorption, the lactase deficiency as well as the sucrase-isomaltase deficiency. In addition the most frequent acquired disorders like the cow's milk protein intolerance and celiac disease contribute also to the group of noninfectious causes of diarrhea. Here the most effective therapy consists of the elimination of the toxic agent from the diet. In infectious diarrhea we find most frequently rotavirus as the agent but also yersinia, campylobacter fetus, salmonella, shigella, E. coli, lamblia giardia and entameba hystolytica. Generally a conservative treatment with a dietetic regimen is preferred. Only in severe cases with yersinia and campylobacter infection the addition of antibiotic drugs is necessary. however have to be treated with metronidazol. As the absorption of glucose is coupled with that of sodium within the small intestine in acute

we find a combined disturbance between salt and carbohydrate absorption. A solution containing glucose and salt is recommended therefore for oral rehydration. The amount administered within the first 24 hours should be between 150-250 ml/kg per day. So called "antidiarrhoic drugs" are questionably effective.

The population in sub-Saharan Africa is growing faster than increases in food production, resulting in a net decrease in food production per capita. The Food and Agriculture Organization has stated that there is a "risk of widespread hunger" which could be prevented by "effective planning of water resources". However, the potential effects of such schemes on the human population are often inadequately assessed and the effect of large dams on human health is not clear. The potential risk to human health of water resources was emphasized a few years ago but no effective preventive programs were implemented, probably because of inadequate availability of information and lack of awareness. The effects on health of "large" water resource projects are not uniform within a population. Decision-makers have tended to focus on the positive effects, to obtain support for their plans. These include: 1) improvement in the well-being of the population (safe water more readily available, new infrastructure, better access to health care) and 2) increases in the food supply (more vegetables and fish available due to irrigation). Thus, there has been a logical expectation that more, better quality food will become available as a result of these schemes, whereas in fact, health and nutrition has often worsened, particularly in young children. Most of the diseases associated with water resource management are communicable, including diseases directly related to the presence of large quantities of water, such as: malaria, which increases in incidence immediately after the building of the dam, after which a new balance develops between the human population and the parasites, schistosomiasis, the disease which increases most in response to the building of dams, particularly in its most severe gastrointestinal form, diarrhea, as water is a major means of dissemination for many organisms, including those causing digestive tract infections and

, salmonellosis, cholera), due to poor sanitation, other parasitic infections, such as onchocerciasis and trypanosomiasis, which should be monitored as they may also threaten the population. Other communicable diseases may appear or increase in incidence with the influx of migrants to the irrigated area. Sexually-transmitted diseases and HIV infection are a particular problem. The large numbers of insects (mosquitoes, blackflies) may also have harmful effects on populations adapting to the new environment. These effects are related to each other and to the environmental changes. New types of food affect people's feeding habits and generate new sources of income. However, they may also lead to new and higher expenditure. There are also likely to be major socio-demographic changes associated with changes in reproductive behavior and women's activities. The location and nature of new homes and infrastructure (e.g. schools, health centers, roads) also contribute to the success or failure of the dam project. There are many constraints to be considered and a more comprehensive approach to the problem is required. Health and nutritional status may be used as simple indicators of the ability of the population to adapt to a new environment. This makes it possible to construct a causal model to identify the most effective and relevant areas of intervention. Health and nutrition issues are of vital importance and scientific findings should be used in decision-making processes for planning future large dam schemes.

Evaluation of 82 cases of
amoebiasis
including 4 of liver abscess was done. Microscopy of 62 stool samples revealed trophozoites in 19 (30.6%) and cysts in 8 (12.9%). PVA preserved samples were stained by iron-hamatoxylin and trichrome. Twenty-nine samples (46.7%) were positive by culture on Endamoeba, NIH and Charcoal agar. Additional two culture positive cases were negative by direct microscopy. Three (75%) samples of liver abscess pus were culture positive. Of the sera screened for amoebic antibodies by ELISA, 13 (17.3%) were positive. Stool and blood of healthy controls and 25

In the developed countries diarrhea is the most common reason for missing work, while in the developing world, it is a leading cause of death. Internationally, the mortality rate is 5-10 million deaths each year. "Traveller's diarrhea" is a polyetiologic common health problem of international travellers which affects travellers generally for days, but it can result in chronic postinfectious irritable bowel syndrome as well. Infectious agents usually cause acute
gastroenteritis
either by adherence of the intestinal mucosa, or by mucosal invasion, enterotoxin production, and/or cytotoxin production. The incubation period can often suggest the cause of etiology. When symptoms occur within 6 hours of eating, ingestion of preformed toxin of S. aureus or Bacillus cereus should be suspected. The incidence of hypervirulent C. difficile associated colitis is an emerging problem as a healthcare system associated infection. While infectious agents do not commonly cause chronic diarrhea, those that do include C. difficile, Giardia lamblia, Entamoeba histolytica, Cryptosporidium, Aeromonas and Yersinia .

Amebiasis
is one of the most common parasitic infections worldwide. Entamoeba histolytica and Entamoeba dispar are two morphologically indistinguishable human protozoa parasites that are genetically distinct species. Differential diagnosis of E. histolytica (pathogenic) and E. dispar (non-pathogenic) is essential both for treatment decision and public health knowledge. Stool samples from 500 randomly selected children complaining of
gastroenteritis
were examined microscopically by direct wet smear and subjected to detection of E. histolytica antigen by ELISA using TechLab E. histolytica II test. E. histolytical E. dispar were also identified at molecular level by targeting the 166 bp, and 752 bp sequences of the 18Sr RNA gene of E. histolytica and E. dispar respectively using polymerase chain reaction technique (PCR). The overall prevalence of E. histolytica/dispar by microscopic examination was 30/500 (6%). E. histolytica was positive in 161500 (3.2%) by antigen detection ELISA technique, whereas 10 samples were not detected microscopically. PCR was able to confirm the presence of E. histolytica in 13/16 cases whereas the 3 samples recorded negative were positive by ELISA; even so there was a good agreement k = 0.86 between the two techniques. In conclusion, stool antigen detection test by ELISA is recommended over PCR in detecting and confirming E. histolytica amoebic enteritis.

The present investigation deals with the development of effective method to capture and eliminate microbial contamination of water and improve the quality of water and thus decreasing the contaminated waterborne infections. Over the last decade, numerous biomedical applications have emerged for magnetic nanoparticles (MNPs) specifically iron oxide nanoparticles. For the first time, we have explored functionalized cobalt ferrite nanoparticles (NPs) for capture and detection of pathogens. The captured bacterial were separated by using simple magnet. To begin with, the prepared NPs were confirmed for biocompatibility study and further used for their ability to detect the bacteria in solution. For this, standard bacterial concentrations were prepared and used to confirm the ability of these particles to capture and detect the bacteria. The effect of particle concentration, time, and pH has been studied, and the respective results have been discussed. It is observed that the presence of amine group on the surface of NPs shows nonspecific affinity and capability to capture Escherichia coli and Staphylococcus aureus. The possible underlying mechanism is discussed in the present manuscript. Based upon this, the present material can be considered for large-scale bacteria capture in water purification application.
