Three third trimester fetuses were exposed to a subgroup 2, type 7 adenovirus (adeno 7) by intraamnionic infection. The virus caused preterm delivery of two clinically ill calves and one stillbirth. The two premature calves died 12 and 72 hours after birth. An elevated serum neutralizing antibody titer (1:256) to adenovirus 7 was found in one principal calf at birth.
Adenovirus
7 was recovered from several tissues of the live calves and the spleen of the stillborn calf. Fetuses exposed by intraamniotnic injection with virus carrier only, were born healthy after normal gestational periods and no viruses were isolated from the tissues. Clinically ill calves were weak, severely depressed and unable to stand and nurse. Gross postmortem lesions were nonspecific and consisted of petechial and ecchymotic hemorrhages and edema of the gastrointestinal tract. Histopathological lesions included vasculitis, necrosis of the mucosa of the forestomach, mild
gastroenteritis
and acute, nonsuppurative focal necrosis of the liver, kidney and adrenal gland. Intranuclear inclusion bodies were seen in pericytes, macrophages, hepatocytes, epithelial cells of adrenal cortical sinusoids of the zona glomerulosa and zona fasciculata and renal tubular epithelium.

Adenoviruses are among the many pathogens and opportunistic agents that cause serious infection in the congenitally immunocompromised, in patients undergoing immunosuppressive treatment for organ and tissue transplants and for cancers, and in human immunodeficiency virus-infected patients.
Adenovirus
infections in these patients tend to become disseminated and severe, and the serotypes involved are clustered according to the age of the patient and the nature of the immunosuppression. Over 300 adenovirus infections in immunocompromised patients, with an overall case fatality rate of 48%, are reviewed in this paper. Children with severe combined immunodeficiency syndrome and other primary immunodeficiencies are exposed to the serotypes of subgroups B and C that commonly infect young children, and thus their infections are due to types 1 to 7 and 31 of subgenus A. Children with bone marrow and liver transplants often have lung and liver adenovirus infections that are due to an expanded set of subgenus A, B, C, and E serotypes. Adults with kidney transplants have viruses of subgenus B, mostly types 11, 34, and 35, which cause cystitis. This review indicates that 11% of transplant recipients become infected with adenoviruses, with case fatality rates from 60% for bone marrow transplant patients to 18% for renal transplant patients. Patients with AIDS become infected with a diversity of serotypes of all subgenera because their adult age and life-style expose them to many adenoviruses, possibly resulting in antigenically intermediate strains that are not found elsewhere. Interestingly, isolates from the urine of AIDS patients are generally of subgenus B and comprise types 11, 21, 34, 35, and intermediate strains of these types, whereas isolates from stool are of subgenus D and comprise many rare, new, and intermediate strains that are untypeable for practical purposes. It has been estimated that adenoviruses cause active infection in 12% of AIDS patients and that 45% of these infections terminate in death within 2 months. In all immunocompromised patients, generalized illness involving the central nervous system, respiratory system, hepatitis, and
gastroenteritis
usually have a fulminant course and result in death. Treatments for adenovirus infections are of little proven value, although certain purine and pyrimidine analogs have shown beneficial effects in vitro and may be promising drugs.

Adenovirus
(Ad) type 40 and 41 DNAs were directly extracted from stool specimens of children with
gastroenteritis
. Two new strains of Ad41, Sanekata and Ehime strain, were cloned and their restriction maps were constructed. The left terminal end of the cloned Ad41 genome, EcoRI-E fragment of the Sanekata strain and EcoRI-F fragment of the Ehime strain, had transforming ability in rat 3Y1 cells. Only one of the 35 isolates of Ad40 tested showed a different restriction profile, while three different restriction profiles were found in DNAs from Ad41 isolates.

From March 1987 to February 1988, fecal excretion of adenovirus types 40 and 41 and rotavirus serotypes in 194 children (age, 0 to 3 years) from a rural community of Guatemala was monitored. In total, 458 samples taken during 385 episodes of diarrhea and 191 specimens obtained during symptom-free periods were examined by enzyme-linked immunosorbent assay. Fifty-seven children hospitalized because of diarrhea were also studied. Among the rural children, 43 (22.2%) excreted adenovirus types 40 and 41 and 20 (10.3%) shed rotaviruses.
Adenovirus
types 40 and 41 were associated with 54 (14.0%) illnesses, and rotaviruses were associated with 18 (4.7%) illnesses. Asymptomatic infections with adenovirus types 40 and 41 were documented in nine children and with rotaviruses in two children. Fifteen typeable rotaviruses were identified as serotype 2. In the hospital population, 36 (63.2%) children had viral infections. Rotaviruses were identified in 29 (50.9%) and adenovirus types 40 and 41 were identified in 15 (31.2%) of 48 subjects tested. Dual infections by these viruses were found in eight children. Of 22 typeable strains of rotaviruses, 9 (34.6%) were serotype 1, 12 (46.1%) were serotype 2, and 1 (3.8%) was serotype 3. All the children infected with serotype 2 rotavirus were coinfected with other enteric pathogens, while only three (37.5%) of those infected with rotavirus serotype 1 excreted another pathogen.

A 2-year study from January 1981 to December 1982 was undertaken to determine the role of viruses in the causation of diarrhoea in hospitalized children in Pune, Maharashtra State, India. The stool samples of 426 children (213 diarrhoeal and 213 non-diarrhoeal controls) were investigated by electron microscopy and ELISA for the presence of viruses. Six morphologically distinct viruses were visualized: rotavirus, coronavirus-like particles (CVLP), adenovirus, astrovirus, calicivirus and small round virus-like particles (SRV). Rotavirus was detected in 28.6% of the diarrhoeal patients and in 1.4% of the controls. The frequency of infection with rotavirus was highest in the children aged less than 5 years. The mean age of rotavirus-positive patients was 11 months. Although rotavirus was detected in almost every month, there has a seasonal trend for colder months when CVLP cases were fewest. However, the prevalence of CVLP was greater in the control group (23.0%) rather than in those with diarrhoea (8.9%). In the control group, CVLP were detected more frequently during the summer months. An inverse relationship between CVLP and rotavirus was observed in children.

One-hundred and fifty-seven consecutive children below seven years of age (primary care n = 48, hospitalized patients n = 109) with acute
gastroenteritis
of assumed infectious origin were studied. Rotavirus was demonstrated by electron microscopy of faeces in 44% of all patients. The occurrence of rotavirus among patients in primary care, 15%, was significantly lower than among hospitalized patients, 57% (p less than 0.01).
Adenovirus
was isolated in six per cent and enterovirus in two per cent of the patients with no differences between the two groups. Yersinia enterocolitica and Campylobacter jejuni were demonstrated in each three per cent. Salmonella and Shigella spp. or Giardia lamblia were not found in any cases. Thus the cause of
gastroenteritis
was established in 58% of the patients. This figure was lower among patients in primary care (27%) than among hospitalized patients (72%), a difference mainly due to the high occurrence of rotavirus in the latter group. Clostridium difficile was recovered in 20 cases (12%), eight of which harboured one more enteropathogenic agent. Cultures from the nose and throat revealed Streptococcus pneumoniae, Haemophilus influenzae, Branhamella catarrhalis or group A, C and G streptococci in 58% of the patients with no differences regarding the occurrence of rotavirus in faeces. Neither Clostridium difficile nor respiratory tract pathogens were found to play a role in causation of
gastroenteritis
in the children investigated.

An epidemiological survey was undertaken to establish the importance of rotavirus as a cause of admission of children to hospital in Buenos Aires, Argentina. Between August 1978 and January 1980 rotavirus infection was detected in 62 (34%) of 180 infants and children admitted to hospital with acute gastro-enteritis. In identification of the virus, electron microscopy (EM) was found to be as sensitive as counterimmunoelectrophoresis (CIE). There was agreement between EM and CIE diagnosis in 90.5% cases.
Adenovirus
and coronavirus were detected by electron microscopy less often. A serological response to rotavirus was revealed by complement fixation test in 38 (72%) of 53 infected patients. Rotaviruses were detected throughout the year, but peak incidence occurred during the cooler months, May to August. Infection was commoner among children under six months of age. Diarrhoea was generally mild in these children and respiratory illness showed no significant association with rotavirus
gastroenteritis
. These results indicate that rotaviruses may be a major cause of infantile acute
gastroenteritis
in Buenos Aires, Argentina especially during the winter.

The aetiology of rotavirus and adenovirus in acute
gastroenteritis
was studied in a prospective series that comprised 283 children admitted consecutively with diarrhoea during a 1-year period. Rotavirus was associated in 49% of the cases by solid-phase radioimmunoassay and electron microscopical examination of stool specimens, or by serology.
Adenovirus
was detected by radioimmunoassay in the stool specimens of 29 (11%) patients, including 8 cases of possible dual infection with rotavirus. Rotavirus infections showed a typical age distribution and seasonal clustering between January and June, whereas the adenovirus-associated cases did not form a distinctive subgroup. Enteropathogenic bacteria were found in 10% of cases, and were nearly as common in association with rotavirus infection as not. Respiratory symptoms accompanied diarrhoea in 34% of the patients with rotavirus and in 25% of those with neither rotavirus nor adenovirus. Therefore we could not confirm the existence of a 'rotavirus syndrome', nor could we confirm an association of respiratory symptoms with rotavirus infection. Use of antibiotics before the onset of diarrhoea was more common among those with non-viral diarrhoea (23%) than in the rotavirus group (13%). Rotavirus infections appeared to be common among cases of 'antibiotic-induced' diarrhoea.

Fastidious enteric adenovirus have recently been recognized as an important cause of acute
gastroenteritis
in young children. Their inability to grow in vitro has hampered classification by conventional methods. With modern immunological and chemical techniques the enteric adenoviruses have been shown to be distinct from the 39 established human adenovirus serotypes. In a prospective study of the viral, bacterial and parasitic aetiology of acute
gastroenteritis
410 children and 205 age-matched controls were studied. An enteropathogenic agent was detected in 67% of the diarrhoeic patients and 57% were of viral origin. Rotavirus was the major agent found in 43% of the patients whereas adenovirus was found in 13%. Of the 50 adenovirus specimens, so far fully characterized by electron microscopy, ELISA-assays, DNA-restriction analysis and isolation studies 70% were identified as enteric adenoviruses. Two serotypes, adeno 40 and 41, were detected representing the new subgroups F and G. Twelve of 17 paired serum specimens, from children with enteric adenovirus showed a significant rise in hemagglutination inhibition titers. Infection with enteric adenoviruses showed 2 small seasonal peaks in summer and late winter. Infection occurred early in life, 85% of the children aged less than 3 years. Diarrhoea was the main symptom with an average duration of 9 days.
Adenovirus
type 41 seemed to cause diarrhoea of longer duration. Fever and vomiting was mild with a mean of 2 days. Respiratory symptoms occurred in 20% of the cases. The incubation period could be estimated as 7 days. Virus was excreted for 10-14 days.

Thirty-five stool specimens, collected over a 14-week period from pediatric
gastroenteritis
patients and shown to contain adenovirus by electron microscopy, were inoculated onto 293 and HeLa cells. Virus isolates were characterized by serum neutralization and restriction endonuclease cleavage analysis of viral DNA from infected cells.
Adenovirus
was isolated upon primary inoculation of 293 cells from all 35 specimens shown to contain adenovirus by electron microscopy. Fastidious adenoviruses 40 and 41 (Ad40 and Ad41) were found in 17 (49%) of the stool specimens, and 4 of these specimens contained a conventional species (Ad1, Ad1, Ad18, Ad31) as well as Ad40. This was first manifest by the observation that four of the isolates which initially grew only in 293 cells acquired the capacity to grow in HeLa cells upon subsequent passage. In each case, the conventional species was undetectable by DNA analysis in the original inoculum but was selected in 293 cells and became the only one detectable by the second passage. Four other specimens, containing Ad1 or Ad31 alone, failed to grow initially in HeLa cells but did grow in 293 cells. The results of this study demonstrate therefore that (i) 293 cells are more sensitive than HeLa cells for the isolation of conventional as well as fastidious enteric adenovirus species and (ii) identification of viruses from patient specimens should involve minimal passage of the virus in cell culture, as a single passage can result in misdiagnosis of the virus associated with the infection.
