Cervical secretions of clover-affected and control ewes in the luteal phase of the ovarian cycle were obtained by flushing the anterior vagina. The flushings were analysed for proteins, carbohydrates and enzyme activities, and were found to be similar to the secretions of the normal ovine uterus. There was significantly more protein, carbohydrate and acid-soluble glycoprotein but less alkaline phosphatase, N-acetylglycosidases (EC 3.2.1.30 and 3.2.1.53) and ribonuclease I in the vaginal flushings of clover-affected ewes. The observed changes were not due to more inflammation in the cervix of clover-affected ewes as there were fewer bacteria, leukocytes and epithelial cells and no elevation of lysozomal enzyme activities in their flushings. It is suggested that the cervix of the clover-affected ewe behaves as though under a stronger than normal oestrogenic stimulation during dioestrus.

Scombroid fish poisoning, one of the most common adverse reactions to fish, is also probably one of the most common causes of a flushing syndrome. The reaction usually involves fishes of the Scombridae family but, in Hawaii, the reaction is most often due to mahimahi (Coryphaena hippurus). Onset of the reaction is usually abrupt and commonly associated with a prominent flush resembling a sunburn. Headache, tachye to a toxin with histamine-like properties, which is formed because improper refrigeration enables endogenous bacteria to decarboxylate histidine normally present in dark-meat fishes. Symptoms are usually promptly relieved by parenteral antihistamine therapy.

This study was carried out to test the general belief that fimbriae should be considered almost indispensable to ovum capture. Surgical procedures, including resection of the mesotubarium superius and infundibulum tubae together with the creation of a fistula, are described. An attempt to enhance tubal patency by temporarily using estrogens is discussed. Ovum pickup was determined by flushing both tubes and the uterus 2 days following ovulation induction. The prior use of estrogens did not appear to have increased subsequent ovum pickup in control animals; on the contrary, it even seems possible that estrogens had negatively influenced pickup by the fistulas. With five fistulas, however, three recovered ova represented a pickup rate of 7% to 14% of all ova available following one human chorionic gonadotropin-induced ovulation. These figures suggest that ovum pickup by terminal ampullary fistulas is not negligible; they might indicate that absence of fimbriae offers neither good protection against pregnancy following sterilization nor a hopeless prognosis for fimbriectomy reversal when a distal patent tube is present.

The mode of operation of grinding faces, rod diamonds and diamond grinding heads which are expecially employed in oto-surgery was investigated in slow motion pictures with the aid of high frequency cinematography. The R.P.M. ranged between 5,000-80,000, the picture frequency was usually 6,000 pictures per second. Full efficiency of the diamond grinding heads (DS) is only guaranteed by adequate flushing. Water stream flushing and drop flushing (drop sequence depending on R.P.M.) must be centered on the grinding head, as increased soiling due to filling of the diamond surface occurs otherwise. Because of the fixed mounting of the spray tube in the direction of the shaft, spray flushing is most useful, although not unsable for diamond faces. Due to its very fine-rough surface, the DS produces grinding dust by abrasion from the bone or cartilage which is readily distributed in the airspace over the whole working area. The same does also apply to finely atomized fluid sprays. Cutting is performed tangentially. The percussing DS touches the bone only with a small part of its surface and easily gets soiled. Depending on the R.P.M., the cutting speed was 10,000-80,000 R.P.M. These values also apply for the fluid dropping rate. Contrary to the rose bit or the surgical fraises, the DS got soiled more easily and produced definitely finer cuts.

A new technique has been developed to make deep renal cortical structures in rats accessible for micropuncture: The left kidney is dissected free and immobilized in a lucite cup. A lense-shaped slice, 1--2 mm thick and about 5 x 5 mm wide, is cut off from the dorsal aspect of the kidney. Blood oozing from the cut surface is removed by flushing with saline and suction by microsponges. The bleeding stops in 1--3 min and causes none or only transient fall in arterial pressure (PA). Up to 40 glomeruli become visible and remain circulated for several hours, as shown by injection of dye or silicone rubber. Glomerular capillary pressures (PG), measured with servocontrolled counter pressure (Wiederhielm), showed no consistent change with time and no correlation to PA. Average PG +/- S.E. in mmHg (number of glomeruli in parentheses) were: Wistar rats (WR), Inactin anesthesia, 57.8 +/- 1.4 (41), Membumal anesthesia, 58.1 +/- 1.3 (13). Sprague Dawley rats, Inactin, 58.1 +/- 1.7 (14). In WR, PG was lower in deep than in midcortical glomeruli: less than or equal to 0.4 mm below kidney surface, 57.9 +/- 1.8 (20); 0.5--0.9 mm: 60.5 +/- 1.5 (20) and greater than or equal to 1.0 mm: 53.8 +/- 2.5 (13). Pressure in Bowman's capsule: 11.2 +/- 0.6 (30). The observed PG is higher than previously reported on the Munich mutant strain of WR, and suggests that glomerular filtration equilibrium is not reached.

The standard 12 sidehole pigtail catheter is compared with pigtail catheters with 6, 8, and 10 sideholes in 40 patient studies. The catheter with fewer sideholes performed as well as the standard catheter with respect to the quality of left ventricular opacification, lack of catheter recoil during angiography, and absence of complications. An in vitro study confirmed that better flushing of the terminal segment beyond the sideholes is achieved with the 6 sidehole catheter compared to the 12 sidehole catheter. It is anticipated that this feature will decrease the risk of thromboembolism associated with the use of the pigtail catheter.

72 dog kidneys were stored under hypothermia as described by COLLINS and SACKS between 24 and 72 hrs and then transplanted. The immediate function of the kidneys was measured by PAH and inulin clearances. 24 hrs proved to be the maximum safe preservation time with both methods. The immediate function of the kidneys stored under hypothermia could not be improved by the addition of furosemide to the flushing solution. These results were compared with those gained by mechanical perfusion of the organ: kidney function after 72 hrs of hypothermic mechanical perfusion was significantly better than after 24 hrs of hypothermic storage.

The syndrome of autonomic dysreflexia often occurs in quadriplegic subjects and is characterized by paroxysmal hypertension, headache, vasoconstriction below and flushing of the skin above the level of transection, and bradycardia. These attacks may cause hypertnesive encephalopathy, cerebral vascular accidents, and death. In five patients during crises, the mean arterial pressure changed from 95 to 154 mm Hg, heart rate 72 to 45 beats/min, cardiac output 4.76 to 4.70 L/min, and peripheral resistance 1650 to 2660 dynes.sec.cm-5. In eight subjects the control plasma, red cell, and total blood volumes were 19.1, 10.5, and 29.6 ml/cm body height, respectively, and when hypertensive, the plasma protein concentration increased by 9.9% and the hematocrit by 9.5%. Plasma volume was only reduced by an estimated 10-15%. At that time, arterial dopamine-beta-hydroxylase (DbetaH) activity increased 65% and prostaglandin E2 concentration by 68%. Thus, the augmented DbetaH activity presented primarily an elevated sympathetic tone and not hemoconcentration of that protein. The rise in prostaglandin may contribute to the severe headaches during hypertensive episodes.

We studied a 59-year-old man with transient paroxysms of hypertension, tachycardia, and flushing in whom pheochromocytoma was excluded. Although catecholamine excretion was normal, plasma catecholamine levels rose from normal basal levels (282 +/- 14 pg/ml) to increased levels (585 +/- 67 pg/ml; x +/- SEM; n = 4) at the peak of spells. Other hormones or substrates expected to rise with nonspecific "stress" did not increase after paroxysms. Therapy with clonidine (0.2 to 0.4 mg/day) suppressed basal catecholamines to undetectable levels and markedly reduced peak levels during spells (80 pg/ml). An epileptic pathogenesis was suggested by stereotypic olfactory and epigastric prodromata before spells, and abolition of paroxysms with the anticonvulsant carbamazepine. This patient represents a rare case of autonomic epilepsy with the seizure focus in the temporal lobe.

The effects of the calcium antagonist nifedipine on uterine activity and lower abdominal pain were studied during the first menstrual day in 10 women with severe primary dysmenorrhoea. Intrauterine pressure was recorded at three different levels by means of microtransducers. Nifedipine, 20 to 40 mg given orally, within 10 to 30 minutes effectively reduced the myometrial activity and relieved the pain. A moderate increase in heart rate, and a transient facial flushing were noted. In some patients receiving 30 or 40 mg this was associated with a slight headache. Otherwise no side effects were observed. It is suggested that calcium antagonists can be used to treat primary dysmenorrhoea and other conditions in which an inhibition of uterine activity is desirable.
