Rosacea should no longer be considered a follicular skin disease. It is a vascular disease of the face characterized by a significant evolution towards local complications such as telangiectasias, papular and aseptic pustular lesions, lupoid granulomas, chronic facial oedema and seboglandular hyperplasia. The basic abnormality seems to be a microcirculatory disturbance of the function of the facial angular veins directly involved in the brain-cooling vascular mechanism. The first clinic hallmark of this dysfunction is the occurrence of
flushing
, which may be spontaneous or induced by alcohol, intake of hot food, emotional stress and sudden variations in temperature. Tetracycline, metronidazole and isotretinoin are very useful for therapy but they only influence the cutaneous and ocular complications and do not act upon the basic vascular trouble. Current therapeutic research is directed towards drugs having an alpha-
sympathomimetic
activity and inhibiting the endogenous opioid mediators of
flushing
such as naloxone or clonidine.

Autonomic dysreflexia, a syndrome sometimes occurring in spinal cord injured (SCI) individuals, may be life-threatening. It involves, in varying degrees, hypertension, diaphoresis, headache, bradycardia, anxiety, and
flushing
and is believed due to unrestricted sympathetic activity below the level of the lesion in individuals with injuries above T4-6. The most frequent causes of the syndrome are urinary infections, rectal impaction, bladder distention, and decubitus ulcers. To our knowledge, medication has seldom been described as causal agent. We report here on an autonomic dysreflexic syndrome following use of an isometheptene combination (Midrin), to treat migraine. The individual involved is a C4-quadriplegic man with a long history of migraines. He was given a standard initial adult dose of the medication. Over a one-hour period, he was initially relieved of the headache, but then noted a new more severe headache, diaphoresis, and

. His vital signs showed progressive BP elevation to 210/130 and a relative bradycardia. Treatment over the next three hours was limited to elevation of the head of the bed and observation, during which his vital signs returned to baseline and he became asymptomatic. This experience reinforces the belief that
sympathomimetic
drugs in general, and isometheptene in particular, should be used in caution in patients with high-level SCI.

This case represents the development of dizziness, palpitation, tightness in chest,
flushing
, and tremor on consumption of a single dose of tapentadol (100 mg) for acute lower back pain. The patient was admitted in the intensive cardiac care unit for continuous monitoring. At admission, electrocardiogram showed tachycardia (140/min) along with ST segment elevation in second chest lead (V2). The patient was monitored and advised not to take further doses of tapentadol. He was discharged after 36 hours of admission. Tapentadol should be used cautiously in patients with cardiovascular diseases and receiving
sympathomimetic
drugs.
