Herpes infections continue to be prevalent, especially in immunocompromised patients. Some of these patients will develop resistant HSV infections. Therefore, it is important to explore new treatment options. Animal studies have shown cidofovir to be effective in the treatment and prevention of HSV infections. Human data are limited, with only one randomized, double-blind, placebo-controlled trial performed to date. The results from this study look promising; however, due to the small sample size, a larger clinical trial is warranted. The human data available as case reports are suboptimal in the quality of reporting time frames for resolution of lesions/symptoms and outcomes of therapy. Another problem with the case report data is that the TK status of the
herpes simplex
isolates was not reported. This would have helped substantiate the acyclovir resistance seen in these patients. It was evident in these case reports that acyclovir resistance can be overcome, as acyclovir-resistant strains became sensitive following cidofovir therapy. This may be because TK(+) viruses have been shown to establish latency more readily than do TK(-) viruses. This pattern suggests that alternating between acyclovir and cidofovir therapies may provide a strategy to manage the emergence of alternatively acyclovir-sensitive and -resistant infections. At present, only the intravenous formulation of cidofovir is commercially available. Advantages of the intravenous formulation include weekly dosing and efficacy. Disadvantages are the complexity of administration and the adverse effect profile. The most common adverse effects with this formulation include nephrotoxicity manifested as proteinuria (12%), and increased creatinine (5%) and neutropenia (15%). Administration of probenecid and NaCl 0.9% hydration are used to reduce the incidence and severity of nephrotoxicity in patients who are receiving cidofovir. Probenecid also has toxicities, including nausea, vomiting, headache, fever, and
flushing
. The topical formulation of cidofovir looks promising for mucocutaneous HSV infection because it is usually undetectable in the blood following topical administration. Therefore, systemic adverse effects should be minimized. A cidofovir gel product (Forvade, Gilead Sciences) is currently being reviewed by the Food and Drug Administration for the treatment of refractory HSV. Ultimately, more controlled clinical studies are necessary to determine whether routine cidofovir use can be justified in patients with acyclovir-resistant HSV infection.

In dentistry it is of primary importance to take into consideration microbial transfer due to the nature of the construction of rotating dental instruments. This aspect was the starting point for our research with the question whether or not sterilisation is fundamentally necessary for slow and high speed hand pieces to make them "safe" out of a virological point of view, or whether a thermal disinfection could also possibly be adequate for this purpose. In this context, we tested the efficiency of the cleaning and disinfection capacity of an automated steam disinfection and sterilisation unit (Sirona Hygiene Center, Siemens, AG, Bensheim) intended to the hygienic treatment of dental instruments with respect to viruses. In model tests the corresponding instruments were experimentally infected with
herpes simplex
virus type 1 (HSV) and simian vacuoling virus (SV40). As indicator systems we used for both cell cultures (measurement of the degree of infectiosity) and (for HSV) polymerase chain reactions (PCR; determination of viral nucleic acids). In the tests for (residual) infectiosity after thermal disinfection (as an isolated step of the Hygienic Centre) and also for a combination of cleaning and subsequent thermal disinfection (also after protein application), no infectious virus could be found in the interior of the slow handpieces and turbines tested. In opposite to this, infectious HSV and SV40 could be found after completion of every isolated cleaning program in the turbine (in all three ducts) and in the slow handpiece (only in the gearbox duct in the case of HSV, and in the case of SV40 also in the water and air ducts in very small amounts). The PCR analyses showed that no nucleic acids could be found in both instruments (in the air and water ducts) following a practice-relevant combination of cleaning and disinfection, but that PCR-positive signals were obtained for the larger-volume gearbox and drive and return air ducts in 1 or 2 of 3 test samples. The detection of viral nucleic acid proves that it is not a matter of complete removal of the infectious agents from the instruments, but rather that they are simply inactivated. Since the test samples did not show similar amounts of viral nucleic acid, it demonstrates the strong influence of the basic parameters (efficiency of the virus contamination,
flushing
conditions and the like). Our experimental model demonstrate that thermal disinfection may be adequate to prevent virus contamination of rotating dental instruments, while compulsory sterilisation is not mandatory. Further studies are necessary to demonstrate whether the present data are applicable for rotating dental instruments from other manufactures.

The author believes that psychocutaneous medicine has indeed come of age and is being incorporated into mainstream medical practice. Patients presenting to dermatologists today are more sophisticated and are frequently dissatisfied with traditional medical therapies. They actively seek alternative approaches and adjuncts to standard treatments. In contrast to many other "alternative" (or) "holistic" treatments offered through non-medical venues, dermatologists can assure their patients that controlled studies support the efficacy of psychocutaneous techniques in improving many dermatologic conditions. Psoriasis, rosacea,
herpes simplex
, body dysmorphic disorder, acne, eczema, urticaria, neurotic excoriations, excoriated acne, trichotillomania, dysesthetic syndromes, and delusions parasitosis are included in this incomplete list. The author believes it is helpful for both the patient and therapist to define concrete and realistic goals for psychocutaneous intervention. Concrete observable or measurable goals can help the patient and clinician gauge therapeutic progress and success. Specifically, goals can include reduction in pruritus (rating severity from 1-10), decreased scratching activity, decreased plaque extent or thickness, decreased number of urticarial plaques, decreased
flushing
, decreased anxiety, decreased anger, decreased social embarrassment, decreased social withdrawal, and improved sleep. More global goals can include an improved sense of well-being, increased sense of control, and enhanced acceptance of some of the inevitable aspects of a given skin disease. Cure should never be a goal, because most disorders amenable to psychocutaneous techniques are chronic in nature; thus, cure as an endpoint would only lead to disappointment. The author encourages dermatologists to align themselves with what he euphemistically calls "a skin-emotion specialist." The skin-emotion specialist may be a psychiatrist, psychologist, social worker, biofeedback therapist, or other mental health or behavioral specialist. Patients are more likely to accept a referral to a "skin-emotion specialist," because this term destigmatizes psychologic interventions. Incorporating these techniques and specialists into a clinical practice will expand therapeutic horizons and improve the quality of life of many of the patients afflicted with chronic skin disease. A final caveat must be offered about attempting to make prognostic statements regarding the likelihood of therapeutic success. Although all patients can potentially benefit from psychocutaneous interventions, those with severe psychopathology and poor pretreatment functional status are likely to be more difficult to treat and to achieve less optimal outcomes. Patients with personality disorders such as borderline, narcissistic, and schizotypal disorders, and patients with any active psychotic process certainly constitute a more resistant and difficult population with whom therapeutic success is less likely. These patients, however, are often the ones in the greatest subjective distress and certainly can profit from any of the described interventions. Quoting W. Mitchell Sams, Jr., "although the physician is a scientist and clinician, he or she is and must be something more. A doctor is a caretaker of the patient's person--a professional advisor, guiding the patient through some of life's most difficult journeys. Only the clergy share this responsibility with us." This commitment is and must always be the guiding force in the provision of comprehensive and compatient patient care.
