A patient with symptomatic urticaria pigmentosa who responded to nifedipine therapy is reported. Relief from cold-induced urtication and flushing was obtained with 10 mg taken orally three times daily. Calcium influx is an early step in the degranulation of mast cells. We hypothesize that the beneficial effect of nifedipine was due to calcium-channel blockade causing elevation of the mast cell threshold for degranulation.

The present studies were designed to achieve a good maintenance of energy metabolism as a means of improving the quality of liver preservation in rats. For this purpose, investigation were made whether initial warm ischemia may affect energy metabolism during preservation and oxygen supply during initial cold flushing process may protect energy metabolism. The following are the results. Compared with non-oxygenated groups, the oxygenated groups during the initial cooling process proved to be effective in, 1) reducing the precipitous fall in the ATP level of the liver during the initial cooling process, 2) maintaining the higher ATP level for 8 hours of hypothermic immersion storage, 3) maintaining energy production for 24 hours in intermittent perfusion storage, 4) obtaining a more rapid recovery of energy production in rewarming and reperfusion after 8 or 16 hours of hypothermic immersion storage, 5) obtaining the higher ATP level by high flow rate of flushing. It may be concluded that maintenance of energy metabolism in the preserved liver is related to the degree of interference with energy metabolism during the initial cold flushing and this interference can be prevented by flushing with the oxygenated perfusate of high flow rate.

No kidney transplant center responding to a kidney preservation questionnaire would accept a kidney flushed with an intracellular electrolyte solution and cold-stored for over 40 hours. This study from one center is a comparison of 50 primary cadaver kidney grafts preserved with an intracellular electrolyte flush followed by cold storage for 40 to 61 hours to 82 primary cadaver kidney grafts preserved by the same method for 9 to 24 hours. Kidneys cold-stored for over 40 hours had a significantly increased requirement for dialysis in the 1st week following transplantation (82% versus 34%) and a significantly increased 1-month serum creatinine nadir (2.3 mg/dL versus 1.7 mg/dL). Actuarial graft survivals and serum creatinine levels at 1, 2, and 3 years after grafting were not significantly different. Cadaver donor methylprednisolone (30 to 60 mg/kg) two to nine hours prior to kidney removal significantly reduced the requirement for 1st-week hemodialysis in the kidneys cold-stored for over 40 hours (60% versus 91%). Kidneys preserved by flushing with cold intracellular electrolyte solution can be successfully transplanted after over 40 hours of simple cold storage when the warm ischemia time is very short.

We compared the effects of inhaled glycopyrrolate (G), 1.3 mg, and atropine (A), 2.6 mg, and placebo on FEV1 and specific conductance (sGaw) before and after exercise in six men with exercise-induced asthma. Subjects exercised with cold air (-2 degrees C) 30 and 120 minutes after each aerosol treatment. Spirometry was performed and sGaw determined before aerosol treatment (baseline) and before and after exercise. Decreased airway tone was noted before exercising with A and G but not with placebo. The decreases in FEV1 and sGaw resulting from exercise were not significantly different among the three treatment groups at either exercise session. Postexercise FEV1 and sGaw were significantly higher after A and G compared to P. Dry mouth, flushing, and resting tachycardia were prominent with group A. Symptoms in G did not differ from those in P. This study suggests that A and G do not prevent bronchoconstriction induced by exercise and cold air but improve postexercise pulmonary function by achieving preexercise bronchodilation. Systemic side effects were minimal with G compared to A.

A 39-year-old nurse exhibited for one year an immediate-type asthmatic reaction with rhinorrhea and facial flushing and itching after ingestion of alcohol. The elimination of all alcohol-containing items from the operating theater brought relief from the daytime symptoms. Some dyspnea after salicylate ingestion and in cold weather persisted. Oral provocation tests with wine and pure ethanol and inhalation tests with ethanol vapours gave rise to all the known symptoms. Asthma could be prevented by prior inhalation of disodium cromoglycate, whereas facial itching and nasal reaction was prevented by oral ketotifen but not by cromoglycate.

Nickel analyses of tap water from several sources in Copenhagen gave up to 490 X 10(-6) g X 1(-1) in the first 250 ml portions. Hot water gave higher values than cold water. After flushing for 5 min, low values were found. Considerable variation from time to time and from tap to tap was found. Drinking of only the first portion in the morning might have an influence on nickel hand eczema.

Transplant teams have been reluctant to accept kidneys preserved with intracellular electrolyte flushing followed by simple cold storage, especially when retrieved by non-transplant surgeons or when preservation time exceeds 24 hours. This study from 1 center is a comparison of 40 primary cadaver kidney grafts preserved with Collins' C2 flushing followed by simple cold storage to 37 primary cadaver kidney grafts preserved with cryoprecipitated plasma on the MOX-100 machine. Cold storage time was 10 to 44.5 hours in the C2 group and 3.5 to 39 hours in the machine-perfused group, with a mean of 23 hours in each group. There was no significant difference between the 2 preservation methods no matter who removed the kidney with respect to 1) the incidence of acute tubular necrosis, 2) the 1-month serum creatinine nadir of surviving grafts and 3) the actuarial graft survivals up to 2 years. Among the 40 C2-preserved kidneys 17 were retrieved by community surgeons and 23 were retrieved by transplant surgeons. Human kidneys removed from beating-heart cadaver donors can be preserved satisfactorily with either Collins' 2 flushing followed by simple cold storage or pulsatile machine perfusion, even when preservation times exceed 24 hours.

The results of seven segmental pancreas transplantations in diabetic patients, using a jejunal Roux-en-Y loop for drainage of digestive enzymes, are presented. An initial case with pancreatic duct ligation is also included. The patients ranged in age from 30 to 45 yr, with duration of diabetes from 8 to 24 yr, and were incapacitated but not uremic. Immunosuppression was attempted with azathioprine, prednisone, and antilymphocyte globulin, and, in one patient, thoracic duct drainage was added. The pancreas tolerated at least 16 min of warm ischemia and at least 4 h of cold storage; flushing with a balanced electrolyte solution was optimal. Six of the grafts provided control of blood glucose for 7--51 days, and, in one patient, an intravenous glucose tolerance test was normal at 7 and 21 days. Five of the grafts failed due to rejection 7--51 days after transplantation, and one was removed at 14 days, while still functioning, due to bleeding. In one case, early detection of rejection by a rise in post-prandial blood glucose was treated and reversed by corticosteroid administration. Two failed in the immediate postoperative period from vascular thrombosis. Drainage of pancreatic secretions from a fistula was a common problem.

In 1976 a standardised hypertonic 'intracellular' perfusate (Euro-Collins solution) was recommended for renal preservation in all donor centres collaborating with the Eurotransplant Organisation. In order to investigate the effectiveness of this perfusate canine kidneys were initially flushed with Euro-Collins solution and subjected to 48, 72 and 96 h of cold storage and autotransplantated. Immediate contralateral nephrectomy was performed after autotransplantation. The serum creatinine served as a parameter of postoperative renal function. Under laboratory conditions successful preservation was achieved after minimal (less than 5 min) warm ischaemia for up to 96 h of cold ischaemia and after 15 min of warm ischaemia for up to 48 h of cold ischaemia. Our results suggest that initial flushing and hypothermic storage of kidneys using Euro-Collins solution is a satisfactory preservation method when human cadaver kidneys can be harvested under adequate conditions.

Isosine improved post-transplantation function of canine kidneys subjected to thirty minutes of normothermic ischemia followed by flushing and twenty-four-hour cold storage. Inosine was not able to improve renal preservation when cold storage was extended to forty-eight hours after thirty minutes of normothermic ischemia.
