Several cases are presented of extreme lateral rupture of the lumbar intervertebral disc in which symptoms of localized nerve root compression were not manifest and in which myelography was negative or misleading. Ordinary exploration techniques failed to disclose these lesions. The most striking pain patterns seen in these patients were
flank pain
, gluteal, groin, and sometimes upper anterior
thigh pain
, induced or aggravated by back motion. This pain, plus radiating leg pain, when it occurred, was generally exceedingly severe and disproportionate to the neurological deficit. Lumbar discography was useful in detecting these ruptures.

Three cases of primary hyperparathyroidism in pregnancy are described. Patient 1 developed left
thigh pain
and lower abdominal pain at 34 weeks' gestation. Patient 2 had right
flank pain
and lower abdominal pain at 32 weeks' gestation. Both patients accepted medical therapy initially, which resulted in poor control of hypercalcemia. Patient 1 delayed her parathyroidectomy until the postpartum period; she had maternal hypercalcemia and neonatal hypocalcemia. Patient 2 accepted parathyroidectomy at 32 weeks' gestation with an uneventful outcome for both mother and baby. Patient 3 was asymptomatic; her hyperparathyroidism was diagnosed postpartum after neonatal hypocalcemia and agreed to parathyroidectomy. All 3 patients had a parathyroid adenoma.

An 83-year-old woman was transferred to our cardiac intensive care unit with an acute non-Q-wave myocardial infarction and pulmonary edema. Enoxaparin was one component of the treatment regimen used. . The defining event occurred when the patient became acutely hypotensive and developed abdominal distention, peritoneal signs, intense left

, and a 3.3 g/dl hemoglobin decrease. Abdominal computed tomography showed a 9 x 6 x 20 cm left retroperitoneal hematoma. The hematoma was spontaneous, secondary to enoxaparin use. The patient died despite vigorous supportive care. Enoxaparin is being increasingly used in patients with acute coronary syndromes. Review of the medical literature revealed that this is the first reported case of a patient with an acute coronary syndrome who died as a result of an enoxaparin-induced, spontaneous retroperitoneal hematoma. This article reviews important clinical signs and symptoms, identifies high-risk patient populations, and discusses management strategies.
