A case of urothelial tumor with extremely high serum carcinoembryonic antigen (CEA) levels is described. A 68-year-old female presented with macroscopic hematuria and left flank pain. Laboratory examination revealed an extremely high serum level of CEA (194 ng/ml) and elevated levels of serum CA 19-9 (235 U/ml) and squamous cell carcinoma (SCC)-Antigen (10.7 ng/ml), while urine CEA remained within normal limits. No abnormal findings were recognized in gastrointestinal and respiratory systems, but left renal pelvic tumor (T4N2M0) was discovered. Nephroureterectomy with regional lymph node dissection was done. The pathologic anatomy was infiltrating non-papillary transitional cell carcinoma (TCC, G2 = G3, pT4N2M0). More than 30% of the tumor cells were positive for CEA by ABC-peroxidase staining. Levels of tumor markers remained higher than normal after the operation and were normalized after M-VAC (methotrexate, vinblastine, adriamycin and cisplatin) chemotherapy. However, 6 months after the operation, levels of tumor markers rose again and lung metastases appeared. She died 10 months after the operation.

We report a case of carcinoembryonic antigen (CEA)-producing renal pelvic and ureteral cancer. A 62-year-old man consulted a local hospital with the chief complaint of right flank pain. On ultrasonography and CT scan, right hydronephrosis with the renal pelvis and ureteral tumor were detected, and he was referred to our hospital. Both serum levels of CEA and CA19-9 were elevated to 36.9 ng/ml and 119 u/ml, respectively. Close examination of the gastro-intestinal tract did not detect any sign of digestive tumor. Right nephro-ureterectomy was performed, and the tumor was histologically diagnosed as TCC G2 > G3 pT3, and CEA was positive in the tumor cells immunohistochemically. CA19-9 was also positive both in the tumor cells and normal epithelium of the renal tubules. Postoperatively, multiple lung metastases developed despite chemotherapy and the patient died 4 months after surgery. CA19-9 had immediately decreased to the normal range after preoperative percutaneous nephrostomy. CEA had transiently decreased postoperatively, but then increased with lung metastases, apparently related to the state of cancer.

A 73-year-old man was admitted to our clinic with flank pain and gross macroscopic hematuria. Radiologic examination revealed a solid mass in the left kidney and additionally another mass in the ureteropelvic junction of the same kidney with severe hydronephrosis. Left nephroureterectomy with bladder cuff removel was performed, and histopathological evolution showed a Fuhrman grade 3 clear cell type RCC with low-grade TCC of the pelvis.
