The clinical and radiological features of five patients with renal
hydatid disease
communicating with the collecting system are described. Rupture into the renal pelvis of a
hydatid cyst
gives rise to acute
flank pain
followed by voiding of scolices or daughter cysts with or without hematuria and/or urinary obstruction. Intravenous urography demonstrates a renal mass frequently involving the collecting system; arteriography shows it to be avascular; sonography reveals a multicystic lesion of mixed echogenicity. When daughter cysts are present, computed tomography establishes the diagnosis.

18 patients with 18
hydatid cyst
of kidney, underwent surgical intervention from 1982 to 1994. There were 6 men in the series and 12 women who ranged in age from 7 to 65 years (mean 38 years).
Flank pain
was the chief complaint (55%). Abdominal mass was detected on physical examination in 38% of cases. Ultrasonographic examination, performed in all cases, made a correct diagnosis in 13 patients (72%). Excretory urogram (IVP), performed in 9 patients, showed signs of benign tumor process. All patients were operated on: the procedures performed included 12 resection of "protruding dome", 1 total nephrectomy, 3 partial pericystectomy, 2 total pericystectomy, 1 total nephrectomy. In postoperative course, there was one death (5.5%) and morbidity was 11%. We conclude that
hydatid cyst
of kidney still rare but possible in endemic areas; its treatment, usually surgical, still difficult because it should avoid recurrence of hydatic disease without sacrifice of the kidney.

Isolated extrarenal retroperitoneal and renal echinococcal cysts are rare manifestations of
hydatid disease
. We report 23 cases of renal and other retroperitoneal hydatid cysts diagnosed and treated in our clinic.
Flank pain
, abdominal mass and non-specific symptoms such as nausea and vomiting were the major symptoms. Indirect haemagglutination test was positive in 18 cases and eosinophilia in four cases. Among imaging studies computerized tomography was the most valuable diagnostic examination. A high index of suspicion is needed for preoperative diagnosis of

A 74-year-old man was admitted with right
flank pain
and discomfort lasting for 2 months. CT scanning revealed a large retroperitoneal cystic mass. There were no cysts elsewhere. Serological testing revealed
hydatid disease
. Preoperatively he was treated by Albendazole 400 mg for 1 month and then underwent laparotomy. The entire mass was excised en bloc and intact and right hemicolectomy was simultaneously performed for excision of the adhered ascending colon. He recovered uneventfully and was discharged on the eighth day, Albendazole was given and follow-up visits were arranged for every 6 months. Total cystectomy in case of active echinococcal cysts remains the treatment of choice.
