The clinical and radiological features of five patients with renal hydatid disease communicating with the collecting system are described. Rupture into the renal pelvis of a hydatid cyst gives rise to acute flank pain followed by voiding of scolices or daughter cysts with or without hematuria and/or urinary obstruction. Intravenous urography demonstrates a renal mass frequently involving the collecting system; arteriography shows it to be avascular; sonography reveals a multicystic lesion of mixed echogenicity. When daughter cysts are present, computed tomography establishes the diagnosis.

18 patients with 18 hydatid cyst of kidney, underwent surgical intervention from 1982 to 1994. There were 6 men in the series and 12 women who ranged in age from 7 to 65 years (mean 38 years). Flank pain was the chief complaint (55%). Abdominal mass was detected on physical examination in 38% of cases. Ultrasonographic examination, performed in all cases, made a correct diagnosis in 13 patients (72%). Excretory urogram (IVP), performed in 9 patients, showed signs of benign tumor process. All patients were operated on: the procedures performed included 12 resection of "protruding dome", 1 total nephrectomy, 3 partial pericystectomy, 2 total pericystectomy, 1 total nephrectomy. In postoperative course, there was one death (5.5%) and morbidity was 11%. We conclude that hydatid cyst of kidney still rare but possible in endemic areas; its treatment, usually surgical, still difficult because it should avoid recurrence of hydatic disease without sacrifice of the kidney.

Isolated extrarenal retroperitoneal and renal echinococcal cysts are rare manifestations of hydatid disease. We report 23 cases of renal and other retroperitoneal hydatid cysts diagnosed and treated in our clinic. Flank pain, abdominal mass and non-specific symptoms such as nausea and vomiting were the major symptoms. Indirect haemagglutination test was positive in 18 cases and eosinophilia in four cases. Among imaging studies computerized tomography was the most valuable diagnostic examination. A high index of suspicion is needed for preoperative diagnosis of hydatid disease of kidney and retroperitoneum.

We present the case of a 64-year-old female with a periadrenal paraganglioma. The mass was diagnosed during abdominal ultrasonography performed to investigate right flank pain. Magnetic resonance imaging showed a lesion 48mm in diameter with an irregular hyperintense central margin, thickened septa, centrally located cystic component, and contrast enhancing peripheral portions. These findings made us think it was an isolated adrenal hydatid cyst. The mass was excised via laparoscopy without complications. Histopathological examination was consistent with periadrenal paraganglioma. We believe that the paraganglioma can mimic the radiological appearance of an isolated adrenal hydatid cyst, which should be taken into consideration during diagnosis.

A 74-year-old man was admitted with right flank pain and discomfort lasting for 2 months. CT scanning revealed a large retroperitoneal cystic mass. There were no cysts elsewhere. Serological testing revealed hydatid disease. Preoperatively he was treated by Albendazole 400 mg for 1 month and then underwent laparotomy. The entire mass was excised en bloc and intact and right hemicolectomy was simultaneously performed for excision of the adhered ascending colon. He recovered uneventfully and was discharged on the eighth day, Albendazole was given and follow-up visits were arranged for every 6 months. Total cystectomy in case of active echinococcal cysts remains the treatment of choice.

Primary renal echinococcosis, a rare disease involving the kidney, accounts for 2-3% of human echinococcosis. A 64-year-old female patient from Uzbekistan presented with complaints of left flank pain. A CT scan revealed a cystic mass in the upper to midpole of the left kidney. We regarded this lesion as a renal malignancy and hand-assisted laparoscopic radical nephrectomy was performed to remove the renal mass. The mass consisted of a large unilocular cyst and multiple smaller cysts without any grossly visible renal tissue. The final pathologic diagnosis was a renal hydatid cyst. For patients from endemic areas, hydatid cyst should be included in the differential diagnosis. Here, we present a case of renal hydatid cyst in a female patient who relocated from Uzbekistan to Korea.

Echinococcosis of the kidneys in a renal transplant recipient is extremely rare and its occurrence being related to immunosuppression is a possibility which needs further characterisation. Ruptured renal hydatid in a renal transplant recipient is not reported so far to our best knowledge. We present a 42-year-old renal allograft receipient who presented one year after transplant with left flank pain, palpable left lumbar mass and gross hydatiduria. Investigations revealed a ruptured native hydatid kidney. Patient was managed with a combination of chemotherapy and left native nephrectomy and discharged in a satisfactory condition.

Echinococcus granulosus infection affects worldwide population, including countries of South and Central America, Middle East, sub-Saharan region, Russia and China. In cystic echinococcosis diagnosis is related to the presence of single or multiple well-delineated spherical primary cyst, commonly seen involving liver followed by lungs. Renal hydatid cyst is rare, comprising of about 2-3% of all locations, isolated renal involvement is even rarer. We present a case of hydatid disease in a 45 year male patient who presented with the history of vague flank pain, mild fever and hematuria. IVU showed a filling defect while USG and CT scan displayed multiloculated cyst in the upper pole of right kidney, correlating the clinical and radiological findings a differential diagnosis of cystic renal cell carcinoma and cystic nephroma was derived. Total nephrectomy was done but the histopathological examination of the lesion revealed hydatid cysts and final diagnosis of renal cystic echinococcosis was made.

Surgery is still the gold standard in the treatment of hepatic hydatid disease. One of the major problems associated with hydatid cyst surgery is biliary fistula. In this report, we aimed to describe a laparoscopic internal drainage method for the treatment of complicated hepatic hydatid cyst in order to prevent long-term biliary fistulas. A 44-year-old female was referred to our clinic with the complaints of flank pain. On computerized tomography of the abdomen, an 8-cm hydatid cyst was detected on the right lob of the liver. Laparoscopic exploration demonstrated a cysto-biliary communication, and laparoscopic cystojejunostomy was performed to prevent long-term bile leakage. The patient was discharged on postoperative day 5 without any problems. In 45th days, nearly complete obliteration of the cavity was observed. Laparoscopic cystojejunostomy can be an effective and safe surgical approach for the treatment of complicated hepatic hydatid cysts.

Isolated renal hydatid disease is very uncommon and is usually an unexpected disclosure during radiological imaging. Since it affects the kidney, renal mass, or colic, hematuria, pyuria, and dysuria are the predominant symptoms. We present a case of a 48-year-old diabetic postmenopausal female incidentally diagnosed with an isolated giant renal hydatid cyst while evaluating the cause of recurrent flank pain.
