A survey of the presence of MPD syndrome and bruxing habits among 899 dentists was carried out at the 1973 ADA annual session. The findings reported here suggest that MPD syndrome occurs less frequently in dentists than in dental patients, and there is no difference in the incidence of pathologic bruxing habits between dentists and non-dentists. Of additional interest was the observation that dentists tend not to seek treatment for the relief of MPD. The most frequent form of treatment for both MPD and bruxing habits was occlusal equilibration.

A follow-up study of 118 patients was performed to determine the effect of various conservative treatments for MPD after six months to 12 years. Of the patients, 82% were females; 62 were females younger than 40 years old. During treatment, patients were made aware of muscle spasm, and consciousness of the role of muscles in MPD was raised. Of 105 patients who were contacted, 65 had no further problems, 26 thought the problem was improved and under control, and 14 had not improved or had sought treatment elsewhere. Musculature and psychological factors play major roles in the MPD syndrome.

The slopes of the V/T curves of masticatory muscles were determined in patients with MPD syndrome, and the changes in the slopes were observed prior to and during electrical muscle stimulation and splint therapy. The slopes of the V/T curves in patients with MPD syndrome were significantly steeper than those of the healthy subjects. In patients with MPD the slopes were decreased by electrical muscle stimulation of moderate duration and by insertion of the splint. These findings suggest that the most simple procedure for the diagnosis of MPD syndrome is to compare the slopes in MPD patients with those of healthy subjects. The observation of the changes in slopes during electrical muscle stimulation and splint therapy permits a more quantitative diagnosis of MPD syndrome. It also permits a quantitative measurement of improvement of symptoms in patients with MPD syndrome.

One purpose of this clinical study is to establish a relationship between the hyper activity of the digastric muscles and predisposition of an individual to MPDS (myofacial pain dysfunction syndrome). If a population predisposed to MPD could be identified by an early diagnosis, intervention and treatment could eliminate potential pain in adulthood. Secondly, can the employment of electromyography to aid in the diagnosis of patients with MPD be helpful in establishing a program of prevention and treatment? Thirty-one patients, male and female, were randomly selected from among those routinely diagnosed as having myofascial pain dysfunction syndrome by the dental staff at the Long Island Center for Craniofacial Pain. Eighteen patients who did not experience any symptoms of facial pain comprised the control group in the study. This study demonstrated that the average trace readings which indicate the activity of the digastric muscles, as measured by the electromyogram from patients experiencing facial pain were significantly higher than those from patients without pain symptoms. In every instance, the correlation between facial pain and abnormal swallow patterns which are a cause of hyperactivity of the digastrics was confirmed.
