This article reviewed Definition of CFS proposed by CDC 1988. There are several issues in Definition for CFS of CDC. It is presented that other chronic clinical conditions have been satisfactorily excluded, including preexisting psychiatric diseases in (2) of major criteria. However,
fibromyalgia
can not be excluded from the fifth symptom of minor criteria,
myalgia
, and also depression from the ninth symptom. It is practically difficult to define impairment of average daily activity below 50% of the patient's premorbid activity level for a period of at least 6 months, as shown in (1) of major criteria, and it is not adapted for a first visit patient. Definition for CFS of CDC has been discussed on EBV infection, but not written on postviral fatigue syndrome and myalgic encephalomyelitis. Especially whether epidemic type of CFS is present or not was not discussed. Diagnostic criteria of CFS is necessary for clinical practice.

, however, has distinct features such as no antecedent inflammatory process and no endemics. Therefore, the syndrome has features distinct from, in addition to common features to CFS. It is also very difficult to distinguish CFS from depression. The above-mentioned features can be observed in depression. Now, study of brain blood flow or metabolism by PET or SPECT can be a possible tool for establishment of the CFS identity.

The aim of this study was to describe which complaints patients themselves regard as the cause of sickness certification. During one week in April 1986, 1,379 patients in Buskerud county, Norway, filled in a form after receiving an initial certificate of illness or a continuation certificate from a general practitioner. International Classification of Primary Care (ICPC) no. 1-29 was used to classify the patients' symptoms and complaints. More than half of the patients (53%) considered pain in the musculoskeletal system, particularly back pain (22%), as the reason for their sickness certification. Physical work load was assumed to be a contributory cause to the complaints by 66% of the patients certified sick because of back pain, 58% of those certified sick due to cervical spine and shoulder symptoms, and 72% of those with
myalgia
,
fibrositis
(ICPC no. L18, L19). 29% of the patients meant that their health problems could have been prevented, and 15% reported that they could have continued work if their jobs had been adjusted because of their condition.

Muscle pain
occurs in various neuromuscular disorders with characteristic physiological or biochemical abnormalities. There is, however, a group of patients in whom there is no clear physiological or structural basis for their pains. This syndrome has been called
fibrositis
or
fibromyalgia
. Sleep abnormalities have been reported in some of these patients, but have not been confirmed by others. We studied 8 patients with this disorder and found sleep abnormalities that were characterized by nocturnal myoclonus, alpha-delta sleep, and abnormalities compatible with depression. Polysomnography was, therefore, instrumental in helping direct the treatment of these patients. Therapeutic approaches aimed to correct the specific disorders were effective in improving the pain symptoms.

Virtually all the reports on muscle biopsy in chronic
muscle pain
conditions, except those of primary
fibromyalgia
syndrome (PFS) published recently, have inadequate methodologic information, particularly on definition of cases, and lack controlled materials, so that no firm conclusions may be made. A limited number of controlled studies of muscle biopsy in PFS suggest that results are negative by usual light microscopic, histochemical, and electron microscope examinations. The findings of abnormal rubber band-like structures and interconnecting network of reticular or elastic fibers in muscle fibers of certain PFS patients in a single study need to be confirmed by other centers by careful controlled and blinded observations. The findings that high-energy phosphate levels in tender PFS muscles are significantly reduced appear valid and important, and need to be independently confirmed by well-designed controlled and blinded studies. Finally, biopsy reports of unspecified or unclarified
muscle pain
syndromes should not be assumed to be those related to PFS, and such invalid references must be avoided.
