S-adenosylmethionine is a relatively new anti-inflammatory drug with analgesic and anti-depressant effects. Efficacy of 800 mg orally administered s-adenosylmethionine daily versus placebo for six weeks was investigated in 44 patients with primary
fibromyalgia
in double-blind settings. Tender point score, isokinetic muscle strength, disease activity, subjective symptoms (visual analog scale), mood parameters and side effects were evaluated. Improvements were seen for clinical disease activity (P = 0.04),
pain
experienced during the last week (P = 0.002), fatigue (P = 0.02), morning stiffness (P = 0.03) and mood evaluated by Face Scale (P = 0.006) in the actively treated group compared to placebo. The tender point score, isokinetic muscle strength, mood evaluated by Beck Depression Inventory and side effects did not differ in the two treatment groups. S-adenosylmethionine has some beneficial effects on primary
fibromyalgia
and could be an important option in the treatment hereof.

A 25-year-old woman was admitted in our hospital with back pain and both hip joint pain.
Pain
was abruptly occurred from the beginning of March 1990. Physical examination revealed wide spread
pain
(occipital area, both shoulder, lumber area, bilateral gluteal area, inguinal area, both Achilles-plantar area) and more than 12 tender points (occiput, trapezius, second rib, supraspinatus, gluteal, greater trochanter, hip joints, pubic bone). Laboratory examination showed no abnormal findings except ANF (1:160). Any examination including X-ray, bone scintigraphy, CT and MRI did not disclose spondylitis, sacroiliitis and enthesopathy. She was diagnosed as primary
fibromyalgia

fibrositis
syndrome. Treatment with maprotine hydrochloride (30 mg/day) and phenobarbital (120 mg/day) brought approximately 1/3 reduction of

and tenderness. Psychoanalysis revealed that she had psychological conflicts against her parents and her colleagues at the work. Functional disturbances, vegetative symptoms and psychopathologic changes are frequently connected with this. Laboratory investigations are normal. The diagnosis results from the combination of multilocular conditions of

- combined with the defined tenderness on pressure mentioned and as additional symptoms functional disturbances, vegetative symptoms and psychopathological changes. In the genesis of the disease probably reflex mechanisms on the part of the spinal column, partly also on the peripheral joints primarily play a role. Probably the generalisation is evoked above all by psychosocial factors which possibly cause the various symptoms by a continuous impulse on the reticular formation.

In 119 patients referred with suspected
fibromyalgia
, biopsies from the quadriceps muscle were analyzed for "rubber band" morphology, and isokinetic quadriceps strength was measured. Eighty-four fulfilled the criteria for
fibromyalgia
, 26 had chronic myofascial
pain
(CMP) and 9 had other diseases including 5 with concomitant
fibromyalgia
. Twenty-four CMP patients and 48
fibromyalgia
patients were randomly selected to match with regard to sex, age, smoking and drinking habits. "Rubber band" morphology was blindly graded on a biopsy score scale from 0 to 2. A statistically significant difference in biopsy score was found between the two matched groups (P = 0.003); median biopsy score in
fibromyalgia
was 0.42 and 0.25 in CMP. A cut-off value at 0.33 gave a specificity of 71% and a sensitivity of 63%. Isokinetic muscle strength did not differ in the
fibromyalgia
and CMP groups and was not related to the biopsy score. "Rubber band" morphology is seen more often in
fibromyalgia
patients than in CMP patients. The exact genesis of this phenomenon is still unknown but theories connected with the possible pathogenesis of the syndrome are presented.

Clinical features and psychological status determined by the Minnesota Multiphasic Personality Inventory (MMPI) in 103 patients with primary
fibromyalgia
syndrome (PFS) were analyzed by univariate and multivariate techniques to determine if clinical features were related to psychological status or were intrinsic to PFS per se. The central features of PFS, e.g., number of
pain
sites, number of tender points, fatigue, and poor sleep, were independent of psychological status. However, discriminant analysis identified 4 variables--patient-reported depression, anxiety, stress, and
pain
severity--which together predicted 3 MMPI subgroups with an accuracy of 55% (P less than 0.001); the only musculoskeletal feature--
pain
severity--alone provided an accuracy of only 34% (P greater than 0.05). These data suggest a new concept, that the central features of
fibromyalgia
are independent of the psychological status and are more likely related to the PFS itself. However,
pain
severity may be influenced by psychological factors.

remains unknown. In our patient, hypophysectomy may have played a role by disturbing endorphin secretion and

In a controlled study, 40 patients with refractory
fibromyalgia
were randomly allocated to treatment with either hypnotherapy or physical therapy for 12 weeks with followup at 24 weeks. Compared with the patients in the physical therapy group, the patients in the hypnotherapy group showed a significantly better outcome with respect to their
pain
experience, fatigue on awakening, sleep pattern and global assessment at 12 and 24 weeks, but this was not reflected in an improvement of the total myalgic score measured by a dolorimeter. At baseline most patients in both groups had strong feelings of somatic and psychic discomfort as measured by the Hopkins Symptom Checklist. These feelings showed a significant decrease in patients treated by hypnotherapy compared with physical therapy, but they remained abnormally strong in many cases. We conclude hypnotherapy may be useful in relieving symptoms in patients with refractory
fibromyalgia

A multidimensional evaluation of 78 patients with primary
fibrositis
/
fibromyalgia
syndrome (PFS) revealed no significant relationship between clinical measures of physical discomfort and psychological measures. This observation provided evidence against the notion that the
pain
of PFS has a psychological etiology. The same patients were randomized into 4 groups for treatment with ibuprofen and/or alprazolam in a randomized, double-blind, double-dummy, placebo-controlled pilot trial. Clinical improvement in patient rating of disease severity and in the severity of tenderness upon palpation was most apparent in the subgroup of patients who were receiving both ibuprofen and alprazolam. An 8-week, open-label study in which 52 patients received both drugs further documented improvement in outcome measures. These data indicate that treatment with a combination of ibuprofen and alprazolam can be beneficial for some patients with PFS.

Several past studies have evaluated the electromyographic activity of myofascial trigger points with conflicting results. This study was performed to determine whether spontaneous activity or motor unit activity was present in patients with focal myofascial
pain
or
fibromyalgia
. Using routine needle electromyographic techniques, we sampled reproducibly tender focal areas (tender points), similar tender areas with
pain
referral (trigger points), associated muscle bands and adjacent uninvolved musculature. Twenty-five subjects (14 females, 11 males, mean +/- SD age 43 +/- 14 years) were studied; twenty-one subjects with focal myofascial
pain
and four with
fibromyalgia
. Spontaneous fibrillatory or positive sharp wave potentials were not found in any muscle in the 25 subjects. There was no evidence of focal motor unit activity in the tender points, trigger points or associated muscle bands in either group. Motor unit recruitment was similar in all areas sampled. We conclude that no electrodiagnostic evidence of ongoing denervation or focal muscle spasm is found in association with focal myofascial
pain
or
fibromyalgia

The scapulocostal syndrome, a hitherto insufficiently understood condition, was clinically studied in 201 cases. The main findings were: (i)
pain
was the presenting symptom in all cases and was mainly cervicobrachial (90%); (ii) the syndrome is a definable entity within the wide spectrum of
fibromyalgia
(
fibrositis
); (iii) the
pain
originates mainly from an enthesopathy of the serratus posterior superior muscle; and (iv) physical degeneration was present in 76.5% of patients. Conservative treatment, successful in 95.9% of cases, consisted of an intralesional injection of a steroid-analgesic-mixture of 1 ml Celestone-Soluspan (Scherag) plus 1.8 ml Xylotox E80A (Astra), and physical rehabilitation. It was deducted that the dyskinesia was mainly due to an overload of the scapulocostal articulation, forcing the rib cage down to exert a stretching force on the serratus posterior superior muscle. The operation of 'serratotomy' (severing the serratus posterior superior muscle) was performed with excellent results in 6 patients in whom conservative treatment failed, and is described here for the first time.
