One hundred patients with chest pain and negative coronary arteriography were evaluated for musculoskeletal chest wall findings. Sixty-nine patients had chest wall tenderness. Typical chest pain was evoked by palpation in 16 patients. Tender areas were not found in a control group of patients without chest pain. A diagnosis of
fibrositis
could be made in five patients, including two in whom chest palpation reproduced typical chest pain. The sternal and xiphoid area, left costosternal junctions, and left anterior chest wall were the areas where tenderness was most common, but no significant differences were found comparing locations of tenderness in those with reproduction of typical
pain
. There was no significant difference in location, exacerbating factors, or other musculoskeletal symptoms among different groups of patients. Thus, most patients with noncardiac chest pain have chest wall tenderness that is not found in a control group without chest pain. However, reproduction of
pain
by palpation, a more specific diagnostic finding, is found in a minority of these patients.

The objective of the study was to quantify the voluntary dynamic muscular endurance (DME) in patients with primary
fibromyalgia
(PF) since easy fatigability of muscles is one of the major symptoms in this syndrome. Consecutive outpatients referred to a rheumatology clinic specializing in PF were investigated. Thirty-six patients with PF were compared with 18 patients with chronic myofascial
pain
(CMP) syndrome. Subjects were matched according to gender, age, height, weight, peak torque, and contractional work. The DME was defined as the number of repeated knee extensions necessary for the contractional work in two successive knee extensions to be equal to or below 70% of the initial value measured with an isokinetic dynamometer. A significantly lower DME was found in the PF group than in the CMP group: 11 (7-13) and 18 (11-25), respectively (median and quartiles), p less than .005. Patients with PF have a low voluntary muscular endurance compared to CMP patients.

To test the hypothesis that plasma tryptophan and/or its transport ratio is decreased in primary
fibromyalgia
(PF), we measured plasma tryptophan and its transport ratio in 29 patients with PF and 30 healthy controls without significant
pain
, in a blinded manner. Twenty-one other amino acids were also similarly analyzed among these study subjects. Transport ratio of tryptophan was found to be significantly (p less than 0.01) decreased in PF compared with the control group (0.09 +/- 0.02 vs 0.10 +/- 0.02). Plasma tryptophan level was lower in PF (45 +/- 10 nmol/ml) than in healthy controls (51 +/- 15 nmol/ml), showing a trend towards significance (p less than 0.09). Additionally, plasma histidine and serine levels were found to be significantly (p less than 0.01) lower in patients with PF than in controls. Our results suggest that a decreased brain serotonin level, as possibly reflected by a decreased transport ratio of plasma tryptophan, may play a pathophysiologic role in PF.

We used a simple quasiexperimental design to assess the efficacy of an inpatient cognitive-behavioral treatment program for persons with
fibromyalgia
syndrome (FS). Twenty-five participants were assessed on 3 occasions: (1) about 5 months before admission to the program, (2) on admission and (3) at discharge. In addition, the dependent variables (
pain
response measures) were divided into "target" (expected to change with treatment) and "nontarget" (not expected to change) to assess potential demand characteristics. Our results showed that, whereas no changes occurred in either target or nontarget variables between the initial assessment and admission, consistent changes were observed only in target variables between admission and discharge. Our data provide preliminary support for cognitive-behavioral treatment for FS.

In an epidemiological survey of females aged 20-49 years in Arendal, Norway, a prevalence of
fibromyalgia
according to the ACR criteria of 1990 of 10.5% was found. Thirty-four out of 40 women with
fibromyalgia
(85.0%) had, prior to the present population study, consulted a physician because of widespread
pain
and/or stiffness. Of these 34 women, 14 cases (41.2%) had been given a diagnosis of
fibromyalgia
. It is concluded that
fibromyalgia
represents one of the major causes of
pain
in the locomotor system, and unfortunately, the condition often remains undiagnosed for long periods.

In a cross-over study, the short-term efficacy of whole-body cold therapy and hot mud packs in patients with generalized tendomyopathy (
fibromyalgia
) was compared. As a
pain
assessment, visual analog scale and so-called
pain
score were measured; dolorimetry of the 24 tender points and eight control points was performed as well. Using these methods, we found that there is a significant improvement of all parameters examined during a 2-h period of measurements after cold application, and a marked improvement was also detectable 24 h after this therapy. In contrast, only
pain
score values showed a slight decrease immediately after hot mud-pack therapy, and no significant differences were found in visual analog scale and pressure tenderness as measured dolorimetrically. Central inhibition of nociceptors as a result of an activation of A-delta system as well as a blockade of gamma-motoneurons are discussed to be a mechanism of action of whole-body cold therapy, resulting in a decrease in muscle tonus. Long-term studies are needed to determine, if there is any enduring effect of whole-body cold therapy on
pain
in the patients with generalized tendomyopathy.

During the past year many studies have been published on
fibromyalgia
and chronic fatigue syndromes. Randomized clinical trials using current operational diagnostic criteria were reported, but no single therapy has been highly effective in either condition. The working case definition of chronic fatigue syndrome has been criticized and suggestions for a new case definition have been made. Further understanding of the overlap of these three common disorders will also require that uniform diagnostic criteria be tested in chronic fatigue syndrome and myofascial
pain
syndrome.

Trigger points (TP) are objectively demonstrable foci in muscles. They are painful on compression and trigger
pain
in a referred area. This area may be the only locus of complaint in humans. In dogs we cannot prove the existence of referred zones of
pain
. Therefore, we can only diagnose a TP-induced claudication if we cannot find bone, joint, or neurologic abnormalities, and we do find TP that disappear after treatment together with the original lameness. Several methods have been developed to demonstrate TP existence objectively. These are pressure algometry, pressure threshold measurements, magnetic resonance thermography, and histology. In humans, 71% of the TP described are acupuncture points. TP treatment consists of TP stimulation with non-invasive or invasive methods such as dry needling or injections. In the dog, ten TP are described in two categories of clinical patients. First, those with one or few TP reacting favorably on treatment (+/- 80% success in +/- 2-3 weeks). Second, those with many TPs reacting badly on treatment. Most probably the latter group are
fibromyalgia
patients.
