A multicentric follow-up study was conducted in general practice to assess disease characteristics which determine analgesic needs in non-articular rheumatism. Of 127 patients studied, those with chronic conditions such as brachial

tennis elbow
with a history extending over more than a month tended to require a longer course of treatment than those whose symptoms related to muscles of the trunk and spine. For most patients a one or two week course of Benoral tablets proved adequate in length and efficacy.

The purpose of the study was to report our experience in a preliminary series of patients who underwent arthroscopic treatment of
tennis elbow
in order to appreciate effectiveness of this method. Between September 2000 and February 2004, we treated arthroscopically fourteen patients with epicondylitis which failed to respond to medical treatment given for a mean duration of 15.8 months. We used the technique described by Baker in 1999. Briefly, the external capsule was sectioned followed by section of the extensor carpi radialis brevis and extensor digitorum communis insertions on the epicondyle. Section was continued until muscle fibers were visible. Mean patient age was 45 years (range 36-55). Five patients were victims of occupational accidents and one had an occupational disease. One patient suffered from
fibromyalgia
. All patients were reviewed at a mean follow-up of 11.5 months. We used the Mayo Clinic score to assess outcome which was excellent or good in nine patients, fair in one and poor in four. Despite these modest preliminary results, arthroscopy appears to be well indicated for this pathological condition. The ideal treatment remains a question of discussion.

Hyperalgesia to mechanical and thermal stimuli are characteristics of a range of disorders such as
tennis elbow
, whiplash and
fibromyalgia
. This study evaluated the presence of local and widespread mechanical and thermal hyperalgesia in individuals with knee osteoarthritis, compared to healthy control subjects. Twenty-three subjects with knee osteoarthritis and 23 healthy controls, matched for age, gender and body mass index, were recruited for the study. Volunteers with any additional chronic pain conditions were excluded. Pain thresholds to pressure, cold and heat were tested at the knee, ipsilateral heel and ipsilateral elbow, in randomized order, using standardised methodology. Significant between-groups differences for pressure pain and cold pain thresholds were found with osteoarthritic subjects demonstrating significantly increased sensitivity to both pressure (p = .018) and cold (p = .003) stimuli, compared with controls. A similar pattern of results extended to the pain-free ipsilateral ankle and elbow indicating widespread pressure and cold hyperalgesia. No significant differences were found between groups for heat pain threshold, although correlations showed that subjects with greater sensitivity to pressure pain were also likely to be more sensitive to both cold pain and heat pain. This study found widespread elevated pain thresholds in subjects with painful knee osteoarthritis, suggesting that altered nociceptive system processing may play a role in ongoing arthritic pain for some patients.
