Soft tissue rheumatism (extraarticular rheumatism) is a group of common disorders found separately as independent disorders or as a symptom of systemic joint or spine diseases. The most frequent soft tissue rheumatisms are tendinopathies, rheumatism of muscles with myofascial pains,
fibrositis
of subcutaneous tissue and, finally, bursitis and inflammations of tendon sheaths. The commonest therapeutic procedures are non-steroidal anatirheumatic drugs, local infiltrations of corticosteroids, and physiotherapy. In the acute stages cold packs are the best physical measure against subjective pain. In subacute and chronic cases the pain in subcutaneous tissues can be lessened by connective tissue massage and underwater jet massage, muscle pain by heat and active exercises, and tendinopathies by ultrasonics and electrotherapy but very seldom by heat. In chronic bursitis and tendovaginitis, iontophoresis with potassium iodine may be helpful. Reflex pains in muscles are due to lesions of the spine and should therefore be treated by spine extension and manipulations. Every pain in soft tissue has one source in the anatomic lesion of the tissue and the second in
psychogenic disorders
. The latter must be sought when soft tissue pains are found all over the body on the lines of a generalized
fibrositis
syndrome.

Somatoform disorders
occur frequently. The patients complain about numerous physical problems and/or pain for which, as a rule, an organic pathological cause cannot be not found. They are not willing to or cannot accept psychosomatic explanations. The real cause is faulty stress processing due to earlier biographical factors, for example, an early attachment disorder. A behavioral therapy could help the patient cope with the everyday routine; it influences the symptoms and signs, but does not fundamentally change them. Psychodynamic approaches are very promising. For comorbidity with anxiety or a depressive disorder as well as certain forms of
fibromyalgia
, psychopharmaceutical drugs should be additionally given.

Somatoform disorders
and functional body syndromes define a major, diagnostically heterogeneous group of patients with medically unexplained physical symptoms. Psychopharmacological approaches can be derived from the conceptualization of somatoform symptoms and syndromes within a biopsychosocial model. The survey presented focuses on randomized, double-blind and placebo-controlled studies. Antidepressants show a statistically and clinically relevant impact on many somatoform symptoms. In special reference to pain symptoms serotonergic and noradrenergic antidepressants seem to mediate a more favorable effect than selective serotonin reuptake inhibitors. For some functional body syndromes, e.g. irritable bowel syndrome and
fibromyalgia
, a major analgesic effect of antidepressants can be underlined as well. The empirical data for
fibromyalgia
, however, seem to be more convincing than for irritable bowel syndrome. Pregabalin holds an empirically well established position in the treatment of
fibromyalgia
. As yet there is no convincing psychopharmacological strategy for chronic fatigue syndrome. Probably due to the inherent relationships to anxiety, obsessive-compulsive and depressive disorders, both hypochondria and body dysmorphic disorder can be positively treated by serotonergic antidepressants as well.
