A review was conducted of 236 postmortem examinations of six species of captive and imported bustards in the United Arab Emirates for the period 1979-94. The most common causes of death in adult imported houbara bustards (Chlamydotis undulata macqueenii) were euthanasia due to Newcastle disease, aspergillosis, and post-transportation-related deaths. Helminth parasites were a common finding in imported houbara bustards, and large parasite burdens occasionally caused intestinal obstruction and death. The most common causes of death in adult captive houbara bustards were trauma-related deaths and euthanasia for or death following treatment for capture myopathy. Fatty liver change was an important postmortem finding of captive adult houbara bustards. The main cause of death in juvenile houbara bustards was yolk-sac infection and septicemia. The most common causes of death in adult kori bustards (Ardeotis kori) were capture myopathy and handling injuries or euthanasia following these injuries. The most common causes of death in adult rufouscrested bustards (Eupodotis ruficrista) were post-transportation deaths in imported birds and trichomoniasis and trauma in captive birds. Juvenile rufous-crested bustards appeared to be highly susceptible to gizzard impactions and foreign-body obstructions of the gastrointestinal tract. Nutritional bone disease is an important disease of all juvenile bustards, particularly kori and houbara bustards. Aspergillosis was also a common postmortem finding in captive and imported adult houbara bustards and also caused mortality in juvenile kori and houbara bustards.

Aspergillus terreus is emerging as a causative agent of life-threatening invasive aspergillosis. Prognosis for affected patients is often worse than for A. fumigatus infections. To study A. terreus-mediated disease, we developed 3 infection models. In embryonated hen's eggs and leucopenic mice, the outcome of invasive aspergillosis was similar to that described for A. fumigatus. However, 10(2)- and 10(3)-fold higher conidia concentrations were required for 100% lethality. In corticosteroid-treated mice, only 50% mortality was observed, although bioluminescence imaging revealed transient disease in all infected animals. In surviving animals, we observed persistence of ungerminated but viable conidia. Cytokine levels in these mice were comparable to uninfected controls. In contrast to A. fumigatus infections, all mice infected with A. terreus developed fatty liver degeneration, suggesting the production of toxic secondary metabolites. Thus, at least in mice, persistence and subclinical liver damage are unique features of A. terreus infections.

Aspergillus molds are ubiquitous environmental molds that can cause devastating invasive infections in immunocompromised patients. These infections often go unrecognized in critically ill patients. This case describes a 68 year-old female resident of a long-term nursing facility with history of dementia, nonalcoholic fatty liver disease with cirrhosis, chronic kidney disease stage III and insulin-dependent type 2 diabetes who presented with vomiting, diarrhea and leg swelling. She developed hypotension and was treated for sepsis but found to have negative routine infectious workup. Chest imaging showed nodular densities and bilateral opacities. She developed acute renal failure and hypoxic respiratory failure followed by acute decompensated cirrhosis with refractory volume overload and hypotension and was eventually transitioned to comfort care measures. Autopsy ultimately showed invasive pulmonary aspergillosis. Here we review the diagnosis and management of invasive fungal infections in critically ill patients without typical risk factors or clinical findings for invasive fungal disease. Invasive fungal infections are frequently missed and carry high mortality rates, therefore warranting consideration in critically ill populations.
