A new tensile
fatigue
test using a stainless steel screw was introduced. A transparent PMMA rod was selected as the test material. A
hole
was drilled and tapped into the rod; a screw was put into the rod with dental cement or without cementation. Three types of cyclic loads were applied to the test piece; the number of times the load was applied when the initial crack was observed and when the final fracture occurred were recorded. Surface observation showed that actual
fatigue
fractures occurred in the test piece. The preferred applied load waveform was the cyclic haver-triangles load because of the small coefficient of variance.
Fatigue
strengths of test pieces with and without cementation were different. Cementation improved the
fatigue
properties, and the type of cement had an effect on the
fatigue
properties. Although the number of final fractures minus the number of initial cracks was the constant regardless of the type of cement used.

Seven-
hole
3.5 mm broad and 5-
hole
4.5 mm narrow dynamic compression plates were applied to paired canine cadaveric tibias in a stable fracture model. Paired tibias were tested to acute failure in rotation and four-point bending, and to
fatigue
failure in four-point bending. Resistance to screw pullout was measured for three 3.5 mm cortical screws and two 4.5 mm cortical screws inserted in the configurations of the bone plates. All plate-bone systems failed by fracture of the bone through a screw
hole
. The 3.5 mm plate-bone system was stronger in acute failure in rotation and in four-point bending. There was no difference in stiffness, and no difference in the number of cycles to failure in
fatigue
testing. Three 3.5 mm screws had greater resistance to pullout than two 4.5 mm screws. Results indicate that the 7-
hole
3.5 mm broad dynamic compression plate has a biomechanical advantage over the 5-
hole
4.5 mm narrow dynamic compression plate.

A comparison of toluene and ethanol (EtOH) induced changes in central nervous system (CNS) function and symptoms were evaluated in two studies, and when possible the effects of toluene were expressed in EtOH equivalent units. The toluene concentrations were 0, 75, and 150 ppm, bracketing the American Conference of Governmental Industrial Hygienists threshold limit value (ACGIH TLV) of 100 ppm. The socially relevant EtOH doses were 0.00, 0.33, and 0.66 g EtOH/kg body weight, equivalent to two and four 3.5% 12 ounce beers. Forty two paid college students were used in each study. In the first study, subjects were exposed to toluene and an odour masking agent menthol (0.078 ppm) for seven hours over three days. In the second study EtOH or a placebo was administered at 1530 across three days also in the presence of menthol. Verbal and visual short term memory (Sternberg, digit span, Benton, pattern memory), perception (pattern recognition), psychomotor skill (simple reaction time, continuous performance, symbol-digit, hand-eye coordination, finger tapping, and critical tracking), manual dexterity (one
hole
), mood (profile on mood scales (POMS),
fatigue
(
fatigue
checklist), and verbal ability were evaluated at 0800, 1200, and 1600. Voluntary symptoms and observations of sleep were collected daily. A 3 x 3 latin square design evaluated solvent effects simultaneously controlling for learning and dose sequence. An analysis of variance and test for trend were performed on am-pm differences reflecting an eight hour workday and on pm scores for each solvent, in which subjects were their own control Intersubject variation in absorbance was monitored in breath. A 5 to 10% decrement was considered meaningful if consistent with a linear trend at p less than 0.05. At 150 ppm toluene, losses in performance were 6.0% for digit span, 12.1% for pattern recognition (latency), 5% for pattern memory (number correct), 6.5% for one
hole
, and 3% for critical tracking. The number of headaches and eye irritation also increased in a dose-response manner. The greatest effect was found for an increasing number of observations of sleep. A range of 2 to 7% decrements suggest the ACGIH TLV of 100 ppm toluene may be a good estimate of the biological threshold supporting a re-evaluation of the TLV. At 0.66 g EtOH/kg body weight symptoms and performance decrements were 6.6% for digit span, 9.2% for pattern recognition, 4.0% for continuous performance, 7.9% for symbol-digit, 16.5% for finger tapping, 6.2% for critical tracking, and 5.2% for the one
hole
test. The EtOH equivalents at 150 ppm toluene for digit span (0.56g EtOH/kg/body weight), the latency for pattern recognition (0.66 g EtOH kg body weight), and the one
hole
element "move" (0.37 g EtOH kg body weight) show that the first two measures would be affected at or above the 50 mg% blood alcohol concentration. This concentration is recognised as the lowest alcohol concentration associated with increased numbers of automobile accidents. The results suggest that EtOH may be a useful acute standard to compare the effects of various industrial solvents and support investigating an association between exposure to solvents and increased risk to safety in industry.

Since it is very rare that cardiac tamponade due to myocardial rupture caused by infective endocarditis, occurs we are reporting this case. A 62 year old man, who had underlying diseases of pneumoconiosis and hypertensive heart disease, visited Chikuho Rosai Hospital complaining of chest oppression and general
fatigue
on Feb. 7, 1987. He was diagnosed as having ischemic heart disease by electrocardiogram. Two days later, he suddenly had chills and a fever, and the laboratory data showed leukocytosis and a positive C-reactive protein (CRP). The echo cardiogram showed mitral regurgitation (MR) and aortic regurgitation (AR), but neither vegetation nor pericardial effusion was observed. On Feb. 16, he was admitted with shock, and he died the next day. The blood cultures grew gram-positive cocci, respectively. From the clinical symptoms, chest roentgenogram and electrocardiogram, we suspected a cardiac tamponade. On autopsy findings, though coronary arteries were intact, the aortic valves had severe valvular adhesions, calcifications and hypertrophies. The rupture
hole
was observed in the left ventricles, which was just under the aortic valve through the pericardiac space. It seemed that he died of a cardiac tamponade due to the outflow of blood from this
hole
. On histopathologic findings of the cardiac wall, gram-positive cocci and many of neutrophils were observed.

This study investigates, both behaviourally and biochemically, the action of 2-allophanyl-2-allyl-4-valerolactone (Valofan) in the mouse, after acute or repeated administration. The exploratory behaviour was measured in different experimental conditions by a
hole
-board test modified in observation length: 10 min instead of original 5. The variations in 5-HT and 5-HIAA levels were measured in cortex and brainstem. Acute administration of Valofan (50 to 500 mg/kg os) did not change exploration during the first 5 min period, while at higher doses (200-500 mg/kg) it did produce a significant increase in basal exploratory behaviour, measured by prolonging
hole
-board test to 10 min. These data were confirmed by the habituation test to he thole-board whereby, after three days of exposure, low exploratory baseline of mice was constant. The elevation of the exploration was consistant with a significant net increase in 5-HT levels (greater than 5-HT; less than 5-HIAA) in brainstem, and with a relative enhance in amine levels (= 5-HT; less than 5-HIAA) in cortex for higher doses of the drug. Modifications of serotonin mechanisms positively affect behaviour in an unfamiliar environment. Surprisingly repeated treatment (for 8 days) with Valofan did not change exploration in respect of controls. The biochemical pattern of repeated treatments showed that higher doses of drug increased 5-HT without affecting 5-HIAA levels in brainstem and cortex. Repeatedly handheld mice showed control values significantly higher than those of acute treatment. This increase in activity corresponded to a biochemical pattern similar to that obtained after acute administration of 500 mg/kg Valofan, indicating that handling affected the basal 5-HT content. Thus Valofan could stimulate exploration in mice with a low baseline, while it did not change the activity of animals with a higher baseline. Further evidence for a possible involvement of 5-HT mechanisms in the action of Valofan was given by the
fatigue
test. Mice, submitted to
hole
-board after 24 h of forced walking, showed, with 500 mg/kg, a significant decrease in exploration: action of Valofan seemed to potentiate the effects exerted by stress on serotonin turnover.

A stress analysis of a total hip acetabular component was performed using three-dimensional finite element modeling. The model consisted of 548 four-noded quadrilateral shell elements with 582 nodes. A worst-case support condition was assumed in which bony contact with small areas of the ilium, ischium, and pubis was represented by three-point support. Loads corresponding to the peak pressures developed in the hip when rising from a seated position--the activity contributing most to the
fatigue
of a prosthesis--were applied to the model. Peak stresses of 490 MPa are predicted around a screw
hole
in the region of greatest loading; according to the
fatigue
curve for porous-coated Ti-6Al-4V alloy, this translates into a component life of three years. When the area of bony support for the prosthesis is increased by a factor of 1.5, the
fatigue
life is more than doubled to seven years. The addition of restraints around the loaded
hole
to represent the placement of a screw extends the lifetime dramatically to 65 years. Substituting the material properties of cobalt-chromium alloy increases the predicted lifetime of the component to twelve years. We conclude that poor bone support can compromise the service life of titanium alloy acetabular components. Furthermore, we suggest that screw fixation be used to secure the prosthesis in the anterior-superior region if the prosthesis is not well supported by bone in the acetabulum.

In many industries workers perform manual assembly tasks with hands postured above the shoulders. Awkward shoulder and arm postures are often viewed as acceptable given costs of workplace modification, postural exertions which are in compliance with current design recommendations, ready availability of strong workers, and numbers of electromyographic studies which fail to detect significant signs of localized muscle
fatigue
(LMF). An experiment was conducted to: (a) study the onset and severity of (LMF) in the shoulder when performing a stylus-to-
hole
Fitts reciprocal movement task under a range of postures, hand loads, ratios of work-to-rest, and task durations, and (b) to evaluate the efficacy of three techniques (i.e., changes in EMG behaviour, postural tremor, and cross-modal matching estimates) in detecting and monitoring posturally-based LMF and discomfort in the shoulder complex. Experimental findings showed that posturing hands above shoulder level significantly increased the risk of LMF and postural discomfort even in light-weight manual assembly environs where postural exertions are small, and that cross-modal matching estimates and postural tremor were more sensitive metrics of LMF in the shoulder complex than EMG RMS voltage and mean spectral power frequency metrics. The basis for experimental findings, as well as potential application of LMF metrics in future postural stress investigations, are discussed. Recommendations for workplace posture are provided for job designers facing work height decisions in manual assembly environs.

An acute inhalation chamber study of 42 college students was performed to investigate the relation between exposure to 0, 75, and 150 ppm of toluene and changes in central nervous system function and symptoms. Paid subjects were exposed for seven hours over three days. Verbal and visual short term memory (Sternberg, digit span, Benton, pattern memory); perception (pattern recognition); psychomotor skill (simple reaction time, continuous performance, digit symbol, hand-eye coordination, finger tapping, and critical tracking); manual dexterity (one
hole
); mood (profile of mood scales (POMS];
fatigue
(

checklist); and verbal ability were evaluated at 0800, 1200, and 1600 hours. Voluntary symptoms and observations of sleep were collected daily. An analysis of variance and test for trend was performed on the difference and score for each concentration reflecting an eight hour workday where each subject was their own control. A 3 x 3 Latin square study design evaluated toluene effects simultaneously, controlling for learning across the three days and the solvent order. Intersubject variation in solvent uptake was monitored in breath and urine. A 5-10% decrement in performance was considered significant if it was consistent with a linear trend at p less than 0.05. Adverse performance at 150 ppm toluene was found at 6.0% for digit span, 12.1% for pattern recognition (latency), 5.0% for pattern memory (number correct), 6.5% for one
hole
, and 3.0% for critical tracking. The number of headaches and eye irritation also increased in a dose response manner. The greatest effect was found for an increasing number of observations of sleep. Overall, no clear pattern of neurobehavioural effects was found consistent with the type 1 central nervous system as classified by the World Health Organisation. Subtle acute effects, however, were found just below and above the ACGIH TLV of 100 ppm toluene, supporting the position that the guideline be lowered since the biological threshold of behavioural effects may be comparable with the TLV.

Spinal fixation employing transpedicular screws has recently been the focus of increased attention at various institutions throughout the world, but concerns about the safety and efficacy of transpedicular screws linger. This study was undertaken to address some of these concerns. The study included evaluation of the internal and external morphology of the vertebral pedicles, which revealed that adequate bone stock is generally available at T2, T7, T12, and L1-L5 spinal levels to accept screws in the 4-7-mm diameter range. The pedicle was observed to be composed of abundant cancellous bone internally with relatively thick cortical walls. The method of pilot
hole
preparation for pedicle screws was also examined. Screws inserted in pilot holes prepared with a 3.4-mm blunt probe (ganglion knife) resulted in higher pullout forces in eight of 10 trials as compared with those with pilot holes prepared using a 3.2-mm drill. Furthermore, the probes afford greater control of
hole
depth and alignment.
Fatigue
studies on three screw designs revealed a graduation of strength between a 7.0-mm pedicle screw, a 5.5-mm pedicle screw, and a modified 6.5-mm cancellous lag screw. The modified cancellous lag screw has an inherent stress riser that affected
fatigue
life. It was noted that extreme care must be exercised to prevent bending of the pedicle screws during implantation. If bending occurs one can expect a 50% reduction in the number of cycles to failure.

Twenty-three canine pelves were tested bilaterally to determine the stiffness and strength of intact ilium and stabilized oblique iliac osteotomies that simulated a common clinical fracture. Fixation systems tested were three 4.0 mm cancellous screws inserted ventral to dorsal across the osteotomy site and one laterally placed five
hole
3.5 mm dynamic compression plate. Specimens were mechanically tested to failure under torsional, axial, or axial plus bending loads. Lag screw fixation was stiffer and stronger than plate fixation in all testing modes. The differences were statistically significant (p less than .05) in the torsional and axial plus bending loading modes.
Fatigue
testing was performed on implanted specimens with low-level cyclic loading under axial plus bending loading conditions. Physiologic loading conditions failed to produce mechanical failure of either fixation system after 100,000 cycles.
