After discussion of the modern concepts of pathophysiology of ocular myasthenia the ocular symptoms such as ptosis and eye muscle palsies are discussed. As important diagnostic sign the Simpson lid fatigue test before and after application of Tensilon is described. For diagnosis of myasthenic eye muscle palsies electrooculography has a special significance especially in connection with the application of Edrophonium, which normalizes myasthenic hypometric saccades and transforms them even in hypermetric saccades. In doubtful cases of eye muscle palsies the electromyogram of the affected muscle in connection with the Edrophonium-test is extremely valuable. With regard to modern treatment apart from cholinesterase inhibitors (Pyridostigmine, Neostigmine) thymectomy, the application of corticosteroids, ACTH and especially also immune suppressive drugs (Imurel etc.) is discussed. Of great significance in ocular myasthenia is the local application of cholinesterase inhibitors like Eserine, Prostigmin or Phospholine Iodide.

Rosenblueth and Luco demonstrated in 1939 that, during prolonged stimulation of a motor nerve, neuromuscular fatigue is followed by a rise of tension that has been called the Rosenblueth Phenomenon. The purpose of this work was to investigate the Rosenblueth Phenomenon in a cat neuromuscular preparation in which the nerves were severed at different levels and stimulated at 60 Hz for several hours. It was demonstrated that in the longer nerve preparation the Rosenblueth Phenomenon starts earlier and its maximal tension is higher. Acetylcholine sensitivity was studied in the superior cervical ganglion preparation and no change was observed when tested before stimulation, during fatigue, and during the Rosenblueth Phenomenon. It is concluded that the onset and amplitude of the Rosenblueth Phenomenon depend on the length of the peripheral nerve stump: the longer the stump, the earlier and higher the response. It is suggested that the Rosenblueth Phenomenon is produced by an increase in the transmitter release which would be due to axonal progression of molecules along the nerve.

Authors report a case of intraventricular hemorrhage with hepatic insufficiency. A 36-year-old man was admitted following the sudden onset of coma. For 10 years before admission he had suffered general fatigue and jaundice, which were treated with medication as acute hepatitis. On the day of admission he began to suffer from a severe headache. Within one hour he was comatose and began to have vomiting, followed by seizures characterized by tonic movement of the right extremities. Lumbar puncture showed an initial pressure over 400 mmH2O, with grossly bloody spinal fluid. Numerous hemorrhages were noted in both optic fundi. Bilateral carotid angiography demonstrated slight enlargement of left lateral ventricle. Computerized tomography revealed that the lareral, third and fourth ventricles were dilated. There were discrete areas of increased absorption coefficient with values measuring between 30 to 35 in the Hounsfield scale in all ventricles. Two burr holes in both frontal areas were performed. About 50ml of blood clot at left ventricle and 30 ml of blood clot with liquor at right ventricle were removed. The patient died 7 days after operation. Autopsy revealed clotted blood in the whole ventricular system, mainly in right anterior horn of lateral ventricle, and a markedly cirrhotic liver with hepatoma. In our review of the literature, the relationship between intraventricular hemorrhage and bleeding tendency caused by hepatic insufficiency was discussed.

The contractile properties of frog sartorius muscles were evaluated during development of fatigue and recovery. Muscles were stimulated under anaerobic conditions at a rate of 30 stimuli/min. Twitch tension (Pt) decreased 59% and tetanic tension (Po) decreased 52% during 15 min of stimulation. Contraction time and one-half relaxation time increased markedly during the first 6 min and then leveld off. The peak rate of twitch tension development (dP/dt) progressively declined to 33% of its initial value. Maximum shortening velocity did not change significantly. Large increases in Pt and twitch dP/dt occurred during the first 15 s of recovery while Po did not increase significantly in this time. Thereafter, Pt, Po, and, dP/dt all progressively returned towards normal over 30 min. These results show that the initial rapid recovery in Pt is not due to an increase in the capacity to generate force. Po is a better indicator of the force-generating capacity of a muscle because, unlike Pt, it is not affected by changes in the rate of tension development.

Experiments were performed to determine how glycerol affects macromolecular syntheses and nucleoside triphosphate levels in a strain of Escherichia coli that lacks a functional sn-glycerol-3-phosphate dehydrogenase. The addition of glycerol to cultures of this strain, Lin 8, growing on a gluconeogenic carbon source causes immediate growth stasis (N. R. Cozzarelli, J. P. Koch, S. Hayashi, and E. C. C. Lin, J. Bacteriol. 90:1325-1329, 1965). Immediately after the addition of glycerol to cultures of Lin 8, the syntheses of DNA, RNA, and protein are completely inhibited. Phospholipid synthesis is not inhibited as severely by glycerol. The addition of glycerol to strain Lin 8 also results in a rapid decrease in its nucleoside triphosphate levels. The total intracellular concentration of ATP in strain Lin 8 was reduced by 85% within 30 s after the addition of glycerol. These results suggest that the glycerol-induced inhibition of growth and macromolecular syntheses may be a secondary consequence of the decreased energy supply in this strain. In addition, studies also suggest that phospholipid synthesis can continue (albeit at a reduced rate) under conditions of severe energy limitation.

Forty-four adult male mongrel dogs were used to compare segmental cortical freeze-dried allogeneic bone grafts with fresh autogenous, freeze-dried autogenous, and fresh allogeneic segmental cortical grafts. Group I consisted of bilateral fresh autografts as external controls; Group II, a fresh autograft on one side for internal control and a freeze-dried autogenous graft on the other side to evaluate the effect of freeze-drying on repair; Group III, a fresh autograft on one side and a fresh allograft on the other side to measure the differences between autogenous and allogeneic grafts; and Group IV, a fresh autograft on one side and a freeze-dried allogeneic graft on the other to see if freeze-drying altered the repair of allogeneic grafts. The grafts were analyzed qualitatively over a six-month period by the use of interval roentgenograms to determine the times of graft-host union and the incidence of fatigue fractures. Six months after operation, the repair processes in the four groups were compared quantitatively with respect to biological repair and physical strength using torsional stress-testing, tetracycline labeling, and microradiography. The results indicate both qualitatively and quantitatively that: (1) fresh bilateral segmental cortical autografts show reproducible characteristics, so that the canine fibula furnishes a satisfactory model (Group I); (2) freeze-drying does not inhibit the repair process per se (Group II); (3) fresh allografts are rejected in varying degrees of intensity (Group III); and (4) freeze-drying does not protect cortical allogeneic grafts from such rejection (Group IV).

A study is presented of 14 patients with hyperparathyroid crisis treated at the Massachusetts General Hospital between 1964 and 1978. These patients showed diverse clinical manifestations that were indistinguishable from those in patients with pseudohyperparathyroidism. Their symptoms varied from progressive fatigue, malaise, and weakness to those related to the gastrointestinal and urinary tracts. The one biochemical alteration commonly found among these patients was the rapid increase in the serum calcium. There was a concomitant rise in the BUN in 50% of the patients and in the creatinine in 80%. The diagnosis was established by an elevated immunoreactive parathyroid hormone (PTH) level in all eight patients (100%) who had the radioimmunoassay; by the presence of subperiosteal resorption of the phalanges in six of the eight patients (75%); and in three of four patients (75%) by the loss of the lamina dura of the teeth. The 12 patients who had surgery all survived; the two who did not died. Thirteen patients (93%) had a neoplasm--an adenoma in 12 and a carcinoma in one. One patient had hyperplasia (7%). Nine patients (64%) received hypocalcemic drug therapy. The serum calcium temporarily fell to 12 mg/100 ml in five patients (56%) but failed to budge in four (44%). Simultaneous treatment with saline infusion, furosemide and with hypocalcemic drugs over a prolonged period compounded the difficulty at operation by increasing interstitial edema. Our findings from this study show prompt surgical intervention as the ideal treatment for hyperparathyroid crisis, preferably, within 72 hours of the acute onset of symptoms.

Investigations into fatigue are very important for development of labour and sports physiology. Many occupations in modern industry do not often show apparent fatigue. This is the reason for the interest shown for occupational strain sgudies. Occupational strain has proved to have laws of optimum. In some occupational situations work is performed at a different than the optimum, level. It is important to take into consideration general and local strain as well as specific and unspecific components of strain. The fatigue level is a significant sign characterizing the occupational strain.

Complex motor habit (swimming in breast stroke style) revealed dissociation of the inner (electromyography) and ambient (chronogram of swimming) structure in conditions of emotional tension and fatigue. Some emotional states, however, induced consolidation of the habit due to the range of sportsmen and their psychological readiness for competition. The variety of emotional states seems to induce a variety of effects on the inner and ambient structures of the formed motor habit.

Autoscopy is a rare syndrome in which a person hallucinates a vision of himself, while retaining insight into the unreality of the phenomenon. Although a number of illnesses are associated with autoscopy, the most common causes are stress and fatigue. The medical, psychiatric, literary, and historical contributions to the understanding of autoscopy are reviewed and three recent cases presented. It is emphasized that illness, injury, and hospitalization are important precipitants of the syndrome, particularly in persons with highly developed visual memories.
