This study investigated the wear resistance of recently introduced nanofill (Filtek Supreme [FS], 3M-ESPE) and ormocer (Admira [AM], Voco) composites and compared their wear characteristics to microfill (Filtek A110 [AO], 3M-ESPE]), minifill (Esthet X [EX], Dentsply; Filtek Z250 [ZT], 3M-ESPE) and polyacid-modified (Dyract AP [DY], Dentsply) composites. Six specimens were made for each material. The specimens were conditioned for one week in distilled water at 37 degrees C and subjected to wear testing at 20 MPa contact stress against SS304 counter-bodies using reciprocal compression-sliding wear instrumentation. Distilled water was used as lubricant. Wear depth (microm) was measured using profilometry every 5,000 cycles up to 20,000 cycles. The results were analyzed using ANOVA/Scheffe's test (p<0.05). Wear of the materials was cycle and fatigue dependent. Although no significance in wear was observed between materials after 5,000 cycles of wear testing, significant differences were observed at 10,000 cycles and greater. After 20,000 cycles of wear testing, ranking was as follows: ZT > DY >AM > AO > FS > EX. Wear ranged from 39.90 microm for EX to 113.32 microm for ZT. The wear resistance of ZT and DY was significantly lower than AO, FS and EX In addition, ZT experienced significantly more wear than AM. Under the conditions of this in-vitro study, the wear resistance of nanofill and ormocer composites was comparable or superior to polyacid-modified, microfill and minifill composites.

The purpose of this study was to assess the dietary intakes and behaviors of male and female collegiate athletes. Athletes (n =345) at a NCAA Division I university completed an anonymous questionnaire. T-tests, chi(2) statistic, and ANOVA were used to assess gender and sport differences. Multiple linear regression was used to assess gender differences in nutrient intakes, controlling for energy intake and to examine the relationships between desired weight change, dietary behaviors, and nutrient intakes. Only 15 % and 26 % of athletes had adequate intakes of carbohydrate and protein, respectively, based on recommendations for athletes. Males were more likely to exceed the Dietary Guidelines for fat, saturated fat, cholesterol, and sodium than females. Sixty-two percent of female athletes wanted to lose at least 5 lbs compared to 23 % of males. The desire to lose weight was associated with decreased energy and macronutrient consumption, but not with inadequate micronutrient intakes.

The aim of this study was to investigate the flexural fatigue behavior of 11 packable composites. Of each material 30 rectangular samples (1.2 x 5 x 35 mm) for flexural fatigue test were prepared. The clamped fracture strength and flexural fatigue limit (FFL) of each material were determined using a custom-made fatigue machine, after storage of the samples for one month in water at 37 degrees C. All data were analyzed using one-way ANOVA and Bonferroni/Dunn's test for multiple comparisons (p<0.05). Regression analysis was used to evaluate the relationship between elastic modulus (Abe et al., 2001), clamped fracture strength or FFL and inorganic filler fraction (vol%). The fracture strengths of all but two materials were in between those of the compact-filled densified composites and the microfine ones. The FFL of the packable composites tested were significantly lower than those of the compact-filled densified composites. Three of the tested materials showed even significantly lower FFL than did the microfine composites. There were statistically significant relationships between both the elastic modulus and the volumetric filler fraction (R(2)=0.974, p=1.990 x 10(-7)). The great diversity in packable composites makes clinical assessment necessary with regard to a justified use in posterior teeth.

Myostatin inhibits myoblast proliferation and differentiation in developing muscle. Mounting evidence suggests that myostatin also plays a limiting role in growth/repair/regeneration of differentiated adult muscle by inhibiting satellite cell activation. We tested the hypothesis that myostatin mRNA expression would decrease after resistance loading (RL) with a blunted response in older (O) females (F) who have shown minimal hypertrophy [vs. males (M)] after long-term RL. As myostatin is thought to modulate cell cycle activity, we also studied the response of gene transcripts key to stimulation (cyclin B1 and D1) and inhibition (p21cip and p27kip) of the cell cycle, along with the muscle-specific load-sensitive mitogen mechano-growth factor (MGF). Twenty young (Y; 20-35 yr, 10 YF, 10 YM) and 18 O (60-75 yr, 9 OF, 9 OM) consented to vastus lateralis biopsy before and 24 h after a bout of RL (3 sets x 8-12 repetitions to volitional fatigue of squat, leg press, knee extension). Gene expression levels were determined by relative RT-PCR with 18S as an internal standard and analyzed by age x gender x load repeated-measures ANOVA. A load effect was found for four transcripts (P < 0.005) including myostatin, cyclin D1, p27kip, and MGF as mRNA levels decreased for myostatin (-44%) and p27kip (-16%) and increased for cyclin D1 (34%) and MGF (49%). For myostatin, age x load and gender x load interactions (P < 0.05) were driven by a lack of change in OF, while marked declines were noted in YM (-56%), YF (-48%), and OM (-40%). Higher cyclin D1 levels in OF led to a main age effect (36%, O > Y) and an age x gender interaction (66%, OF > YF vs. 10%, OM > YM; P < 0.05). An age x gender x load interaction (P < 0.05) for cyclin D1 resulted from a 48% increase in OF. Post hoc testing within groups revealed a significant increase in MGF after RL in YM only (91%, P < 0.05). Higher levels of cyclin B1 in O (27%, O > Y) led to a main age effect (P < 0.05). An age x load interaction for cyclin B1 (P < 0.05) was driven by a 26% increase in Y with no change in O after RL. No age or gender differences, or load-mediated changes, were detected in levels of p21cip mRNA expression. These data clearly demonstrate that RL downregulates myostatin expression and alters genes key to cell cycle progression. However, failure to reduce myostatin expression may play a role in limiting RL-induced hypertrophy in OF.

This study addressed the effects of nandrolone decanoate (ND) on contractile properties and muscle fiber characteristics of rats submitted to swimming. Male Wistar rats were grouped in sedentary (S), swimming (Sw), sedentary+ND (SND), and swimming+ND (SwND), six animals per group. ND (3 mg/kg) was injected (subcutaneously) 5 days/week, for 4 weeks. Swimming consisted of 60-min sessions (load 2%), 5 days/week, for 4 weeks. After this period, the sciatic nerve extensor digitorum longus (EDL) muscle was isolated for myographic recordings. Fatigue resistance was assessed by the percent (%) decline of 180 direct tetanic contractions (30 Hz). Safety margin of synaptic transmission was determined from the resistance to the blockade of indirectly evoked twitches (0.5 Hz) induced by pancuronium (5 to 9x10(-7) M). EDL muscles were also submitted to histological and histochemical analysis (haematoxylin-eosin (HE); nicotinamide adenine dinucleotide-tetrazolium reductase (NADH-TR)). Significant differences were detected by two-way ANOVA (p<0.05). ND did not change body mass, fatigue resistance or kinetic properties of indirect twitches in either sedentary or swimming rats. In contrast, ND reduced the safety margin of synaptic transmission in sedentary animals (SND=53.3+/-4.7% vs. S=75.7+/-2.0%), but did not affect the safety margin in the swimming rats (SwND=75.81+/-3.1% vs. Sw=71.0+/-4.0%). No significant difference in fiber type proportions or diameters was observed in EDL muscle of any experimental group. These results indicate that ND does not act as an ergogenic reinforcement in rats submitted to 4 weeks of swimming. On the other hand, this study revealed an important toxic effect of ND, that it reduces the safety margin of synaptic transmission in sedentary animals. Such an effect is masked when associated with physical exercise.

Controversy remains about the value of combined treatment with levothyroxine (LT4) and liothyronine (LT3), compared with LT4 alone in primary hypothyroidism. We compared combined treatment with LT4 and LT3 in a ratio of 5:1 or 10:1 with LT4 monotherapy. We conducted a double-blind, randomized, controlled trial in 141 patients (18-70 yr old) with primary autoimmune hypothyroidism, recruited via general practitioners. Inclusion criteria included: LT4 treatment for 6 months or more, a stable dose for 6 wk or more, and serum TSH levels between 0.11 and 4.0 microU/ml (mU/liter). Randomization groups were: 1) continuation of LT4 (n = 48); 2) LT4/LT3, ratio 10:1 (n = 46); and 3) LT4/LT3, ratio 5:1 (n = 47). Subjective preference of study medication after 15 wk, compared with usual LT4, was the primary outcome measure. Secondary outcomes included scores on questionnaires on mood, fatigue, psychological symptoms, and a substantial set of neurocognitive tests. Study medication was preferred to usual treatment by 29.2, 41.3, and 52.2% in the LT4, 10:1 ratio, and 5:1 ratio groups, respectively (chi2 test for trend, P = 0.024). This linear trend was not substantiated by results on any of the secondary outcome measures: scores on questionnaires and neurocognitive tests consistently ameliorated, but the amelioration was not different among the treatment groups. Median end point serum TSH was 0.64 microU/ml (mU/liter), 0.35 microU/ml (mU/liter), and 0.07 microU/ml (mU/liter), respectively [ANOVA on ln(TSH) for linear trend, P < 0.01]. Mean body weight change was +0.1, -0.5, and -1.7 kg, respectively (ANOVA for trend, P = 0.01). Decrease in weight, but not decrease in serum TSH was correlated with increased satisfaction with study medication. Of the patients who preferred combined LT4/LT3 therapy, 44% had serum TSH less than 0.11 microU/ml (mU/liter). Patients preferred combined LT4/LT3 therapy to usual LT4 therapy, but changes in mood, fatigue, well-being, and neurocognitive functions could not satisfactorily explain why the primary outcome was in favor of LT4/LT3 combination therapy. Decrease in body weight was associated with satisfaction with study medication.

Exercise-induced muscle damage (EIMD) is a common occurrence following activities with a high eccentric component. Alterations to the torque-velocity relationship following EIMD would appear to have serious implications for athletic performance, particularly as they relate to impairment of maximal intensity exercise. However, this has been studied infrequently. The purpose of this study was to assess the effects of EIMD on maximal intermittent sprint performance. Ten male participants (age 22.4 +/- 3.2 years, height 178.6 +/- 5.2 cm, mass 80.6 +/- 10.7 kg) performed 10 x 6 s cycle ergometer sprints, interspersed with 24 s recovery against a load corresponding to 0.10 kp/kg and 10 x 10 m sprints from a standing start, each with 12 s active (walking) recovery. All variables were measured immediately before and at 30 min, 24, 48 and 72 h following a plyometric exercise protocol comprising of 10 x 10 maximal counter movement jumps. Repeated measures ANOVA showed significant changes over time (all P<0.05) for perceived soreness, plasma creatine kinase activity (CK), peak power output (PPO), sprint time and rate of fatigue. Soreness was significantly higher (P<0.01) than baseline values at all time intervals (3.1, 4.9, 5.5 and 3.2 at 30 min, 24, 48 and 72 h, respectively). CK was significantly elevated (P<0.05) at 24 h (239 IU/l) and 48 h (245 IU/l) compared to baseline (151 IU/l). PPO was significantly lower (P<0.05) than baseline (1,054 W) at all time intervals (888, 946, 852 and 895 W, at 30 min, 24, 48 and 72 h, respectively). The rate of fatigue over the ten cycling sprints was reduced compared to baseline, with the greatest reduction of 48% occurring at 48 h (P<0.01). This was largely attributed to the lower PPO in the initial repetitions, resulting in a lower starting point for the rate of fatigue. Values returned to normal at 72 h. Sprint times over 10 m were higher (P<0.05) at 30 min, 24 h and 48 h compared to baseline (1.96 s) with values corresponding to 2.01, 2.02 and 2.01 at 30 min, 24 h and 48 h, respectively. Values returned to baseline by 72 h. The results provide further evidence that, following a plyometric, muscle-damaging exercise protocol, the ability of the muscle to generate power is reduced for at least 3 days. This is also manifested by a small, but statistically significant reduction in very short-term (approximately 2 s) intermittent sprint running performance. These findings have implications for appropriate training strategies in multiple sprint sports.

In order to evaluate a structured support intervention programme, symptoms and quality of life (QOL) were studied in 61 consecutive cancer patients with different tumour diagnoses in relation to intervention and follow-up. The majority of the patients were female. The Edmonton symptom assessment scale (ESAS), the Norwegian fatigue questionnaire and the hospital anxiety and depression scale (HADS) were used. Data were analysed according to ANOVA and Tukey's honest significant difference (HSD) test. ESAS sickness score (P=0.0001), depression (P=0.0001), anxiety (P=0.0001) and QOL (P=0.0009) improved and the improvements in depression and anxiety were still significant after 3 months (P=0.02 respectively). Aspects of fatigue also improved significantly in 7 of 11 questions after the intervention (P< or =0.04) but these improvements were not observed after 3 months. Anxiety according to the HADS questionnaire improved significantly after the intervention (P=0.0006). The majority of the patients highly appreciated the possibility of sharing thoughts and troubles with others with similar experiences (59%). The programme was therefore found to improve QOL, and physical and psychological functions. Randomized studies in relation to immunological changes and follow-up are in progress.

The purpose of this longitudinal study was to explore changes in symptom distress in newly diagnosed Taiwanese breast cancer patients during the initial 4-week postoperative period. The research instruments, including a demographic questionnaire and the Symptom Distress Scale, were used to obtain data on postoperative day 2 and at weeks 2, 3, and 4. In total, 39 patients with a mean age of 48 years participated in this study. Data were analyzed using descriptive statistics, t tests, one-way ANOVA, and repeated-measures ANOVA. Results revealed that the level of symptom distress significantly decreased from postoperative day 2 to week 4. Loss of appetite and a poor outlook increased; nausea frequency, fatigue, and insomnia decreased then increased; and frequency and the level of pain, coughing, tightness/tenderness in the chest wall, weakness, and numbness in the arm of the operative side all decreased over the 4-week study period. Age, stage of disease, and type of surgery were all related to symptom distress. Results of this study may provide reassurances about what can be expected after breast cancer surgery.

The aim of this study was to evaluate the mechanical properties, such as Young's moduli, fracture strengths (FS), and flexural fatigue limits of todays resin composite dental restoratives. All materials have been subdivided into flowable, aesthetic hybrid and nano-filled hybrid composites as marketed by dental manufacturers and analyzed in terms of the actual filler configurations. Specimen bars have been manufactured in reference to ISO 4049 standard, light-cured for 20 s, and stored in distilled water before testing. The elastic moduli (EM), FS, and flexural fatigue limits (FFL) were measured after 14 days storage by using the four-point bending test. The FFL was determined for 10(4) cycles. The fatigue data were analyzed by using the "staircase" approach and statistically treated by ANOVA analysis. Flowable materials with a reduced filler content exhibited the lowest Young's moduli, compared with those measured for higher filled materials. A linear relationship has been found between elastic moduli and filler loading (r(2) = 0.798). Correlations of FS and fatigue data to different filler fractions could not be proved. FS ranged between 61.3 and 124.9 MPa. After 10(4) cycles of fatigue loading, the FS suffered from a decrease between 45.2 and 61.7%. However, materials providing high initial strengths do not obviously reveal the best fatigue resistance. A marketing-based grouping of direct restorative materials has no meaning toward laboratory testing of mechanical properties.
