This study was to examine the effects of acupressure on
fatigue
and depression in HCC patients undergoing
TACE
. A quasiexperimental study design was used. Patients were evaluated at five time points: before treatment (T1) and 2, 3, 4, and 5 days after treating
TACE
(T2, T3, T4, and T5).
Fatigue
and depression were assessed by a VAS
fatigue
scale and a VAS depression scale at each time point. TFRS and BDI were administered at T1 and T5. Patients'
fatigue
and depression were significantly higher at T5 than at T1 in two groups.
Fatigue
and depression increased in both the experimental and control groups' patients over the five days of hospitalization during which
TACE
and chemotherapy were administered. The experimental group had significantly less
fatigue
than the control group, with lower subscale scores on physical, psychosocial, daily, and overall
fatigue
. There were no differences between the groups on depression. At posttest, the experimental group experienced lower physical, psychosocial, daily, and overall
fatigue
than the control group. Acupressure can improve
fatigue
in HCC patients during treatment with
TACE
but did not alleviate depression. Discharge planning should include home care for management of
fatigue
and depression.

As a novel vascular endothelial growth factor receptor-2 tyrosine kinase inhibitor (VEGFR2-TKI), apatinib has a certain anti-tumor effect for a variety of solid tumors. The present study evaluates its efficacy and safety in advanced hepatocellular carcinoma (HCC). In this study, 47 patients with advanced HCC were included.
TACE
monotherapy group included 22 patients that responded to
TACE
, while the group that received
TACE
and apatinib included 25 patients that progressed on
TACE
and were able to receive apatinib off label. Median overall survival (OS) was significantly improved in the apatinib plus
TACE
group compared with the
TACE
group. Similarly, apatinib in combination with
TACE
significantly prolonged median progression-free survival (PFS) compared with
TACE
monotherapy. Furthermore, there was a significant difference between combination therapy and monotherapy in both Barcelona clinic liver cancer (BCLC) B and BCLC C group. The combination therapy had a dramatic effect on OS and PFS for patients at both BCLC B and BCLC C level. The most common clinically adverse events of apatinib plus
TACE
group were
fatigue
, weight loss, hypertension, hand-foot syndrome and anorexia, which were manageable and tolerable. The efficacy analysis showed that there was no significant association of survival benefit with age, gender, Eastern Cooperative Oncology Group (ECOG) performance status, hypertension and hand-foot syndrome. Patients with macrovascular invasion and extrahepatic invasion showed worse survival benefits. In conclusion, apatinib combined with
TACE
revealed certain survival benefits for HCC patients who experienced progression following
TACE
, which can provide a promising strategy for HCC treatment.

Aim:
Doxorubicin-eluting beads transarterial chemoembolization (DEB-TACE) is reported to improve survival and tolerability when compared with conventional lipiodol-
TACE
(cTACE) for the treatment of hepatocellular carcinoma (HCC). The aim of this study was to evaluate tolerability and long-term survival in patients treated with cTACE or DEB-
TACE
in a real-life setting.
Methods:
Incidence of adverse events and overall survival in HCC patients treated with either cTACE or DEB-
TACE
at Karolinska University Hospital 2004-2012 were analyzed retrospectively. Median follow-up was 7.1 years. Patients were censored when transplanted or at the end of follow-up. Patients receiving both cTACE and DEB-
TACE
, or treated with resection or ablation post-
TACE
were excluded from the survival analysis.
Results:
A total of 202 patients (76 cTACE and 126 DEB-TACE) were eligible for analysis of adverse events, and 179 patients (69 cTACE and 110 DEB-TACE) were included in the survival analysis. cTACE patients were younger and had fewer tumors but higher BCLC stage than DEB-
TACE
. Child-Pugh and ECOG performance status were similar between groups. Adverse events (abdominal pain, nausea and vomiting, fever,
fatigue
) were significantly less common in the DEB-
TACE
group. Median survival was 17.1 months in the cTACE group and 19.1 months in the DEB-
TACE
(NS). In multivariate Cox regression analysis, portal vein thrombosis and tumor size were associated with increased, and sorafenib treatment post-
TACE
with decreased mortality.
Conclusion:
In this retrospective real-life analysis, DEB-
TACE
had better tolerability compared to cTACE, but overall survival did not differ between the two treatments. Portal vein thrombosis, tumor size and sorafenib treatment after
TACE
influence survival.
