The burnout syndrome denotes a constellation of physical fatigue, emotional exhaustion, and cognitive weariness resulting from chronic stress. Although it overlaps considerably with chronic fatigue as defined in internal medicine, its links with physical illness have not been systematically investigated. This exploratory study, conducted among 104 male workers free from cardiovascular disease (CVD), tested the association between burnout and two of its common concomitants--tension and listlessness--and cardiovascular risk factors. After ruling out five possible confounders (age, relative weight, smoking, alcohol use, and sports activity), the authors found that scores on burnout plus tension (tense-burnout) were associated with somatic complaints, cholesterol, glucose, triglycerides, uric acid, and, marginally, with ECG abnormalities. Workers scoring high on tense-burnout also had a significantly higher low density lipoprotein (LDL) level. Conversely, scores on burnout plus listlessness were significantly associated with glucose and negatively with diastolic blood pressure. The findings warrant further study of burnout as a predictor of cardiovascular morbidity and mortality.

Physiological-hygienic assessment has been made of the conditions of labour, the degree of difficulty, tension of labour, fatigue of miners during the work shift, indices of health status and sociopsychological climate under the new conditions of the team form of labour organization of miners. It has been found out, that transition to the contract and piece-rate and premium system of labour organization lead to higher labour productivity, longer usage of mining equipment, stability of the collective. However, alongside with this, labour conditions are getting worse, the work becomes more difficult and tense, which leads to more rapid development of fatigue of miners during the work shift, to chronic morbidity. Prophylaxis should be aimed at rationalization of regimes of labour and rest, normalization of the psychological climate in brigades, strengthening of the treatment-prophylaxis work.

Acute hypoglycemia provides a reproducible method of investigating the effect of biological changes induced during hypoglycemia on mood states. Hypoglycemia was induced twice using a hyperinsulinemic glucose clamp in 24 nondiabetic human participants; a euglycemic placebo control study was also performed. Serial changes in mood were assessed using the UWIST Mood Adjective Checklist before, during, and after 60 min of controlled hypoglycemia (2.5 mmol/l). Hypoglycemia induced a significant reduction in hedonic tone (p = .001), a significant increase in tense arousal (p < .0005), and a significant decline in energetic arousal (p = .01) in comparison with the euglycemia control study. Profound changes in mood were observed in nondiabetic participants during acute hypoglycemia, and a state called tense tiredness persisted for at least 30 min after restoration of euglycemia.

Two epidemiological surveys of suicidal ideation were conducted among adolescents, aged 15-18, in France and the Canadian province of Quebec. The results suggest that, in both countries, suicidal ideation was linked to drug use (especially tobacco, illicit drugs, and psychotropic medicine), nonspecific somatic complaints (especially tiredness, sleep difficulties, depression, and feeling tense), a lack of self-esteem (pessimism, instability, boredom), and dissatisfaction with family relationships. There was no relationship between suicidal ideation and sports activities in either country. The implications for prevention are discussed.

The Freiburg Complaint List (Freiburger Beschwerdenliste FBL-R) is a self-report questionnaire which consists of 80 somatic complaints. A survey was conducted in 1993 and a sample of N = 2070, representative for the German adult population of 16 years and above, was obtained. Factor analysis, cluster analysis and item analysis were employed to develop nine scales: General Condition, Tiredness, Cardiovascular, Gastrointestinal, Head-Throat, Tenseness, Emotional Reactivity, Pain, Sensory and Total Score. Regression analyses revealed substantial effects of gender, age group, and, to a lesser extent, social class on FBL-R scores. Furthermore, highly significant correlations were found between FBL-R scores and indicators of illness, health concern, and utilization of health services. The standardization provides normative data (stanine norms) that account for gender and age groups. This representative survey provides essential data, i.e. base rates of somatic complaints in the general population and subsamples. These base rates are relevant to the future development of criteria for diagnosis of somatization disorder.

Hypoglycaemia provokes unpleasant symptoms and sensations in patients with insulin-dependent (Type 1) diabetes mellitus (IDDM). There is much interest in, and information on, the cognitive effects of acute insulin-induced hypoglycaemia. However, the effects of hypoglycaemia on brain function extend to important, non-cognitive aspects of psychological functioning, which are reviewed here. Acute hypoglycaemia induces changes in mood which result in a transient state of 'tense tiredness', a decrease in happiness, an increase in tense arousal, and decreased energetic arousal. Appraisals of life problems are affected adversely. Frequent exposure to hypoglycaemia is associated with heightened fear of hypoglycaemia, which can be quantitated in individuals. Personality may also influence behavioural responses to hypoglycaemia and the ability of an individual to cope with diabetes. The adverse effects of hypoglycaemia on mood, behaviour, personality, social function and management of diabetes in individual patients may be profound and need to be identified and addressed appropriately.

Daily hassles, mood changes, and sleep quality in the 3 days preceding migraine attacks were prospectively studied and compared with migraine-free control days. Nineteen female migraine patients, aged 20 to 49 years, kept a 10-week diary four times per day and produced complete data on daily hassles (incidence and stressfulness), mood (alert, tense, irritable, annoyed, depressed, and tired), sleep quality, and migraine. Significant results indicated increased hassles, particularly in the 24 premigraine hours; psychological arousal (increased irritability, annoyance, and tenseness), predominantly from 60 to 24 hours before the attack; repeated fatigue in the 60 premigraine hours, with a peak immediately before the attack; and a sharp decrease in sleep quality in the night preceding the attack. The authors also found that those attacks that came on during phases of heightened activity--particularly in the afternoon--were preceded by more substantial and more significant elevations of hassles and psychological arousal than were attacks that occurred during unwinding phases, particularly at night.

Occupational specialists are well aware of the lack of objective measurements for the diagnosis and the functional evaluation of workers affected by thoracic outlet syndrome (TOS). Subjects affected by TOS refer pain, sensation of fatigue in the arm frequently aggravated by overhead work. A "stick-up" position tends to close the costoclavicular space and tense the neck and shoulder muscles to bring into play the abnormal compression mechanism that may affect the brachial plexus nerves and the subclavian vessels. In a previous study, we demonstrated the reliability of an isokinetic hand grip endurance test. In this pilot study we asked 7 industrial workers affected by TOS to perform a 30-second isokinetic endurance test for hand gripping with a Lido WorkSET dynamometer. Tests were performed in two positions (resting and "stick-up" position) before and after a 3-week intensive rehabilitation program. In addition, the subjects performed daily aerobic exercise with an arm crank ergometer. Objective indexes of posttreatment improvement were found. Subjects were able to generate higher grip forces during the isokinetic hand grip endurance test and to sustain higher mechanical workload with the arm ergometer at the same intensity level for longer duration after completion of the rehabilitation program.

This study used the biological model of experimental hypoglycaemia to examine the effect of a manipulation in mood-state on appraisal. Controlled hypoglycaemia was achieved using the hyperinsulinaemic glucose clamp technique. Mood, appraisal, and personality traits were assessed using well validated questionnaires. Our findings 1) reaffirm the existence of multiple arousal systems in the generation of moods, 2) show that the induction of a negative mood state does lead to more negative appraisals of a life situation, and 3) show that personality traits remain stable during the experience of negative emotions and cognitions. We conclude that hypoglycaemia, by inducing a state of tense tiredness in some individuals, may lead to more negative appraisals of a life situation but does not alter people's reporting of behavioural dispositions.

Personality is complex and unique, in that people differ greatly from one another in multiple components of personality. Nevertheless, personality can be measured and dissected into a number of quantitative traits. One of the more recent tools is the Temperament and Character Inventory (TCI), which is a self-report personality questionnaire based on Cloninger's psychobiological model of personality (Cloninger et al., 1994). This model consists of four dimensions of temperament and three dimensions of character. Temperament dimensions (novelty-seeking, harm avoidance, reward dependence and persistence) refer to automatic emotional responses to stimuli and are moderately heritable, relatively stable throughout life, and invariant despite sociocultural influences (Svrakic et al., 1996). Individuals high in novelty-seeking tend to be quick-tempered, excitable, exploratory, curious, enthusiastic, impulsive and disorderly. Individuals high in harm avoidance tend to be cautious, careful, fearful, tense, apprehensive, nervous, discouraged, insecure, negativistic and pessimistic, even in situations which do not worry other people. People scoring high on reward dependence seek social contact and are open to communication with other people; they are tender-hearted, sensitive, dedicated and sociable. Highly persistent individuals tend to be industrious, hard-working, persistent, and stable, despite frustration and fatigue. The character dimensions (self- directedness, cooperativeness and self-transcendence) involve differences in social goals and values and seem to be moderately influenced by family environment (Cloninger et al., 1994). Cloninger's psychobiological model of personality shows correlations to other factor models of personality, like the NEO five-factor model. The TCI dimension of novelty-seeking shows positive loading on NEO conscientiousness (Cloninger et al., 1994).
