In many industries workers perform manual assembly tasks with hands postured above the shoulders. Awkward shoulder and arm postures are often viewed as acceptable given costs of workplace modification, postural exertions which are in compliance with current design recommendations, ready availability of strong workers, and numbers of electromyographic studies which fail to detect significant signs of localized muscle
fatigue
(LMF). An experiment was conducted to: (a) study the onset and severity of (LMF) in the shoulder when performing a stylus-to-hole Fitts reciprocal movement task under a range of postures, hand loads, ratios of work-to-rest, and task durations, and (b) to evaluate the efficacy of three techniques (i.e., changes in EMG behaviour, postural tremor, and cross-modal matching estimates) in detecting and monitoring posturally-based LMF and
discomfort
in the shoulder complex. Experimental findings showed that posturing hands above shoulder level significantly increased the risk of LMF and postural
discomfort
even in light-weight manual assembly environs where postural exertions are small, and that cross-modal matching estimates and postural tremor were more sensitive metrics of LMF in the shoulder complex than EMG RMS voltage and mean spectral power frequency metrics. The basis for experimental findings, as well as potential application of LMF metrics in future postural stress investigations, are discussed. Recommendations for workplace posture are provided for job designers facing work height decisions in manual assembly environs.

The purpose of this study was to determine the effect of syndesmotic screw fixation on ankle motion. Eight unpaired osteoligamentous cadaver ankles were tested. The study quantified ankle flexion, talar tilt, ankle drawer, and tibiotalar rotation for each specimen using an MTS biomechanical testing system. Each specimen was tested under a 15-kg and a 70-kg axial load. The testing was repeated after the placement of a syndesmotic screw, a 4.5-mm cortical screw, in accordance with AO technique. There was a statistically significant decrease (P less than .05) in tibiotalar external rotation. There was no statistical difference in ankle flexion. There was a significant decrease in the anterior and the posterior drawer tests with the foot in plantar flexion. These were the most significant results. Syndesmotic screw fixation is used for the internal fixation of certain unstable ankle fractures. Opinions differ as to whether the screw should be removed at 6 weeks (prior to weightbearing) or left in place indefinitely. It was concluded that the syndesmotic screw should be removed prior to the return to full activity. Leaving it in place will contribute to abnormal ankle motion; this may result in local

A modified version of the McCorkle & Young Symptom
Distress
Scale, based on a linear analogue self-assessment scoring system, was used to assess symptom distress in a heterogeneous sample of 53 cancer patients. The scale was simultaneously completed by the nurses caring for those patients, who were asked to rate the patient according to how they perceived he was feeling with regard to each particular symptom. The scores were compared for congruency. This preliminary study suggests that, although nurses appear able to estimate the degree of distress due to changes in mobility and appearance or the presence of diarrhoea, constipation and

, they are less effective in perceiving the degree of distress due to the less 'visible' symptoms such as pain, nausea, anorexia, sleeping disturbances, concentration and mood. Perhaps surprisingly, the trend was for nurses to overestimate the degree of distress when this was compared with the patients' self-assessment.

Sixty-eight medical, social, and occupational history variables were analyzed in a general population of 442 men and 478 women, aged 30, 40, 50, and 60 years to identify possible indicators for first-time experience and recurrence or persistence of low-back trouble (LBT) during a 1-year follow-up. Variables that in univariate analyses showed statistically significant indications for future LBT were subjected to stepwise logistic regression analyses. The most important indicators for recurrence or persistence of LBT thus identified were, for men, intermittent claudication, restlessness, or other

We have conducted a Phase I and pharmacological study of flavone acetic acid, one of a series of novel flavonoids. The drug was administered i.v. weekly for 4 weeks, with a 2-week rest and then repeated. Flavone acetic acid was given initially in a 1-h infusion, but at the 3900-mg/m2 dose level, the infusion time was lengthened to 3 h. A total of 31 patients were treated with 9 different dose levels, ranging from 330 to 6400 mg/m2. Dose-limiting toxicity was acute hypotension that began after about one-third of each drug dose had been infused and rarely lasted more than a few minutes after the infusion was discontinued. In addition, subjective
fatigue
and asthenia causing unacceptable patient
discomfort
was dose limiting. A significant side effect noted that was not dose limiting was diarrhea during the infusion. This drug exhibited nonlinear pharmacokinetic behavior. Plasma levels exceeded 300 micrograms/ml during the infusion at the maximally tolerated dose. After the infusion ended the principal half-life was about 2 h. In 24-h urine collections 27% of the flavone acetic acid dose was recovered as intact drug and an additional 37% was recovered as a metabolite. The maximally tolerated dose determined in this study is 6400 mg/m2 given i.v. over 3 h.

and endurance during repeated maximum isokinetic manoeuvres were studied in five sprinters and five marathon runners, all males.
Fatigue
and endurance were expressed in terms of output (contractional work, CW) and input (integrated electromyograms, iEMG), and was related to muscle fibre composition of m. vastus lateralis and to cross-sectional area (CSA) of m. quadriceps. The initially performed contractional work was positively related to the calculated absolute type II fibre area of m. vastus lateralis, but not to the cross-sectional area of m. quadriceps. During the first 25 contractions, sprinters fatigued, with a slope of decline in contractional work that was correlated to the absolute type II fibre area, also after correction of performed work for CSA of m. quadriceps. As the electromyographic activity decreased less than the contractional work, a drastic fall in the CW/iEMG ratio was found in the sprinter group. Three out of five sprinters could perform less than 50 knee extensions, due to subjective
discomfort
in tested thighs. In contrast, all marathon runners could, after a minor similar decline in electromyographic activity and contractional work during the first part of the experiment, maintain a steady-state endurance level of CW and iEMG throughout the test (200 knee extensions). In conclusion,

Although the symptoms may seem to appear after infections and various physical and psychological stresses, neurocirculatory asthenia is most often encountered as a familial disorder that is unrelated to these factors, although they may aggravate an existing tendency.
