A 13-year-old girl presented to the emergency department with
fatigue
, headaches and
muscle stiffness
after returning from a family camping trip. Within 24 h, she was transferred to ICU with general oedema and low saturations, where she had a cardio-respiratory arrest and was placed on veno-arterial extracorporeal membrane oxygenation (ECMO). The patient was successfully supported with ECMO for profound myocardial dysfunction and haemofiltration for rhabdomyolysis and acute renal failure. Patients who present with profound myocardial dysfunction and myoglobinuria as a consequence of viral infection can be successfully supported with ECMO.

The purpose of the present study was to compare vertical jump performance after 2 different
fatigue
protocols. In the first protocol, subjects performed consecutive sets of 10 repetitions of stretch-shortening cycle (SSC) contractions. In the second protocol, successive sets of 10 repetitions of isometric contractions were performed for 10 s with the knee at 90 degrees of flexion. The exercises were stopped when the subjects failed to reach 50% of their maximum voluntary isometric contractions. Maximal isometric force and maximal concentric power were assessed by performing supine leg presses, squat jumps, and drop jumps. Surface EMG was used to determine changes in muscle activation before and after
fatigue
. In both groups, the
fatigue
exercises reduced voluntary isometric force, maximal concentric power, and drop jump performance. Kinematic data showed a decrease in knee muscle-tendon stiffness accompanied by a lengthened ground contact time. EMG analysis showed that the squat and drop jumps were performed similarly before and after the
fatigue
exercise for both groups. Although it was expected that the stiffness would decrease more after SSC than after isometric
fatigue
(as a result of a greater alteration of the reflex sensitivity SSC), our results showed that both protocols had a similar effect on knee

The aim of this study was to investigate changes in mechanomyographic (MMG) and the surface electromyographic (EMG) signals during and after fatiguing shoulder eccentric contractions in a group consisting of 12 males and 12 females. Exerted force, MMG, EMG, pain and rate of perceived exertion were assessed before, during and after repeated high-intensity eccentric exercises. Bouts of eccentric contractions caused a decrease in the exerted force for males (P<0.05) and an increase in the rate of perceived exertion and pain for both genders (P<0.05). During eccentric exercise, the root mean square (RMS) values of the MMG signal increased (P<0.05). The mean power frequency (MPF) values of the EMG signal decreased at the end of each eccentric bout for both genders (P<0.05); the decrease was higher for females compared with males (P<0.05). Immediately after eccentric exercise in static abduction of the upper limbs, the MMG RMS and MPF values increased (P<0.05). The present study showed that (1) neuromuscular changes associated with pain and changes in

and (2) changes in motor units strategy during

Trauma not only affects individuals who are primarily present, but also those with whom they discuss their experience. If an individual has been traumatised as a result of a cancer diagnosis and shares this impact with oncology nurses, there could be a risk of vicarious traumatisation in this population. However, although Thompson [2003. Vicarious traumatisation: do we adequately support traumatised staff? The Journal of Cognitive Rehabilitation 24-25] suggests that vicarious traumatisation is a broad term used for workers from any profession, it has not yet been empirically determined if oncology nurses experience vicarious traumatisation. This purpose of this paper is to introduce the concept of vicarious traumatisation and argue that it should be explored in oncology nursing. The review will highlight that empirical research in vicarious traumatisation is largely limited to the mental health professions, with a strong recommendation for the need to empirically determine whether this concept exists in oncology nursing.

A 59-year-old male was referred to our hospital with chief complaints of general
fatigue

muscle stiffness
of the shoulders. His hormonal data were total testosterone 0.05 ng/ml (normal: 2.01-7.5 ng/ml), free testosterone less than 0.6 pg/ml, leuteinizing hormone (LH) 1.1 mIU/ml (2.2-8.4 mIU/ml), follicle stimulating hormone (FSH) 2.4 mIU/ml (1.8-12 mIU/ml) and prolactin (PRL) 13.1 ng/ml (4.3-13.7 ng/ml). Though both his genital stage and pubic hair stage were Tanner V, his testis volume was 12 ml on the right and 10 ml on the left. A gonadotropin releasing hormone (GnRH) stimulation test revealed low responses of LH and FSH. Magnetic resonance imaging of the head revealed pituitary tumor (15 mm). Our diagnosis was acquired hypogonadtropic-hypogonadism-related pituitary tumor. Transsphenoidal hypophysectomy was performed and pathological diagnosis revealed epidermoid cyst. Three months after the operation, his total testosterone was elevated to 1.17 ng/ml and his complaints were improved.

The organ most frequently affected in mitochondrial disorders is the skeletal muscle (mitochondrial myopathy). Mitochondrial myopathies may be part of syndromic as well as non-syndromic mitochondrial disorders. Involvement of the skeletal muscle may remain subclinical, may manifest as isolated elevation of the creatine-kinase, or as weakness and wasting of one or several muscle groups. The course of mitochondrial myopathies is usually slowly progressive and only rarely rapidly progressive leading to restriction of mobility and requirement of a wheel chair or even muscular respiratory insufficiency. Autosomal recessive limb-girdle muscular dystrophy type 2G (LGMD2G) is an adult-onset myopathy characterized by distal lower limb weakness, calf hypertrophy and progressive decline in ambulation. The disease is caused by mutations in Tcap, a z-disc protein of skeletal muscle, although the precise mechanisms resulting in clinical symptoms are unknown. To provide a model for preclinical trials and for mechanistic studies, we generated knockout (KO) mice carrying a null mutation in the Tcap gene. Here we present the first report of a Tcap KO mouse model for LGMD2G and the results of an investigation into the effects of Tcap deficiency on skeletal muscle function in 4- and 12-month-old mice. Muscle histology of Tcap-null mice revealed abnormal myofiber size variation with central nucleation, similar to findings in the muscles of LGMD2G patients. An analysis of a Tcap binding protein, myostatin, showed that deletion of Tcap was accompanied by increased protein levels of myostatin. Fibromyalgia is a common disorder occurring in approximately 2-5% of most populations, with female patients outnumbering males by up to nine to one. The two essential components of fibromyalgia, experienced in conjunction, are widespread pain and widespread abnormal bodily tenderness, although other common features of the syndrome include

The clinical features result from complex changes of pain-related neurophysiological function in the brain and spinal cord, particularly through the neurophysiological process of sensitization. Management of fibromyalgia is directed to the inputs to this process, through a combination of education, exercise and psychological strategies. A number of drugs can also target this mechanism. With use of selected combinations of these strategies, the prognosis of fibromyalgia is now much improved.
