Hearing loss induced in thirty female workers because of exposure to continuous noise was studied in a plastic bag plant in Samutprakarn Province, the largest industrial zone in Thailand. The sound level in this plant was 98.5 dBA., 94.0 dBA. and 93.0 dBA. in the weaving, winding and warping sections, respectively. Results of an audiometric test showed a significant relationship between high noise level and hearing loss at frequency 4,000 Hz. A questionnaire survey found that a relatively high number of workers had various symptoms such as: 76.7% general
fatigue
, 70% headache, 63.3% ear distension and 56.7%
vertigo
. Concerning the usage of ear protective devices, it was found that 80% of the workers have never used such devices, 16.7% occasionally used them, and 3.3% have always used cotton wool to reduce the high noise level. Their reasons for non-use of ear protective devices were 1) not provided by the employer (86.7%), 2) not necessary (83.3%), 3) accustomed to the noise (63.3%), 4) nobody uses (56.7%), and 5) no loud noise (16.7%). These results point to the need for improving the work conditions and welfare services of the workers at this plant. Also, hearing conservation programmes can be instituted in developing countries through cooperation among the safety inspectorate, the employer and the workers.

A 30-year-old man was admitted because of

and 2 episodes of melena. Endoscopy showed a tumor in the gastric antrum which appeared benign, as was also its appearance on barium meal. The source of the bleeding was the mucosa overlying the tumor. Antrectomy was performed with Billroth I anastomosis. Histopathological examination revealed the tumor to be an ectopic pancreas in the gastric wall.

Orbital venous vasculitis has been suggested to cause characteristic periorbital pain in patients with pathologic changes in their orbital phlebograms. The orbital pain is characterized by being unilateral, not shifting side, boring and pressing, but not throbbing, increasing on eye strain, exposure to cold, or weather changes, and resistant to analgesics. It is ameliorated by steroids. Fifty patients with symptoms of orbital venous vasculitis were investigated for other symptoms that could be related to the vasculitis. When the 32 female patients were compared with a randomly selected age- and sex-matched control group, there was a significant increase of symptoms of chronic

, cold feet, gut problems such as constipation and/or diarrhea, arthralgia, memory impairment, rotatory

, spontaneous ecchymoses (all, p less than 0.0001), back pain (p less than 0.012), and thrombophlebitis (p less than 0.022) in the patient group. These symptoms, although commonly occurring, seem in these patients to be related to the vasculitis. Blood tests of the fifty patients showed signs of inflammation which did not disagree with the hypothesis of an immunologic cause of the orbital venous vasculitis.

Two groups of 15 women with moderately severe post-partum hypotension were assigned at random to receive treatment with either 200 mg dimetophrine or placebo, orally, over a period of 10 days. Systolic blood pressure increased steadily and significantly during the first 5 days of treatment, from 100.0 +/- 1.2 mmHg to 128.0 +/- 1.2 mmHg with dimetophrine; with placebo, the increase from 99.8 +/- 1.1 mmHg to 104.7 +/- 1.1 mmHg was significantly less. Similar results were observed in diastolic blood pressure measurements. Overall, all 15 patients responded to dimetophrine but only 4 spontaneously recovered on placebo. At the same time, heart rate moved towards normal with dimetophrine (from 82.4 +/- 2.0 to 75.5 +/- 1.1 beats/min); with placebo, significantly less recovery was observed (from 79.5 +/- 2.3 to 78.6 +/- 1.5 beats/min). Concomitant with the recovery of perfusion pressure, the associated symptoms (asthenia, paleness,

The therapeutic efficacy and safety of oxiracetam (ISF 2522), a new nootropic cyclic GABA derivative, were investigated in a double-blind, placebo-controlled study in 40 patients with organic brain syndrome in late life. The psychopathology was characterized by memory deficits, intellectual dysfunction, lack of drive, and disturbance of affectivity. Patients were randomly assigned to a 4-week treatment with either 2 X 400 mg oxiracetam capsules t.i.d. or identical placebo capsules in the same dosing schedule. Evaluation of the psychopathology and side effects was carried out at weeks 0, 1 and 4; laboratory tests (hematology, blood chemistry and urinalysis), a battery of psychometric tests and quantitative EEG investigations were done at weeks 0 and 4. In the oxiracetam group a slight but significant improvement in global symptomatology was observed within 1 week, with further improvement after 4 weeks. In the placebo group, an improvement was seen only in the 4th week. Evaluation of the detailed psychopathology by means of the Sandoz clinical assessment geriatric scale (SCAG) showed in the oxiracetam group significant improvements in loss of appetite and

after 1 week and in short-term memory, anxiety, emotional lability,

after 4 weeks. In contrast, not a single item improved significantly during placebo treatment. In extrapulmonary foci the search for mycobacteria is to try by swab, puncture, control of urine and menstrual blood. It is possible, that a histologic corroboration will be necessary. Unclear fever, headache and vomiting with or without dyspnoea, cyanosis and diaphragmatic lowness indicate a ocular reflection, liver biopsy and, in special case, a lumbar puncture without delay. Sooner or later the course of an unrecognized phthisis can result in death. It is necessary to fill up the gap between welltime diagnosis and death by unknown tuberculosis. That means: Thorough knowledge of matter, insight into the disease-course and inducement of all necessary diagnostic possibilities.

In a Finnish general practice 120 patients with psychosomatic disorders, manifest as syndromes of tension headache, cardiac neurosis, dizziness or muscular tension, were randomly allocated to treatment over a 4-week period with either flupenthixol (1 to 2 mg per day) or diazepam (5 to 10 mg mg per day). The 4 syndromes and 12 associated symptoms (anxiety,
fatigue
, depression, pain, asthenia, muscle fatiguability, tension, dyspnoea, restlessness, palpitations, sleep disorders, and

) were rated on a 4-point scale on entry, at 2 weeks and at 4 weeks. Both drugs reduced significantly the average total scores for syndromes and single symptoms after 2-weeks' treatment. Flupenthixol was the more effective in relieving
