In order to determine the causal factors of
low back pain
(
LBP
) which prevails among freight-container tractor drivers, vibration measurements of the seat of ten freight-container tractors and one heavy truck, a survey on the daily working hours of 240 tractor drivers for a month, and a time study of work on 28 person-days were conducted. Vibration measurement was made under routine conditions on paved public roads for 40 km under laden and unladen conditions. For the evaluation of vibration exposure, ISO 2631 was used. With the freight-container tractors, the mean vibration levels were high in X and Z directions, particularly in X direction. In the heavy truck, the vibration level was consistently higher in Z direction. According to the evaluation made by ISO 2631, the X component of the vibration was found to be serious. The magnitude of the vibration level was greatly influenced by the condition of the road surface with bridges tending to produce a high level of vibration. Vibration levels of the new model tractors were not necessarily always lower than that of the old models. It was estimated from the foregoing results and the time study that in more than 90% of the drivers the daily exposure time (i.e. driving hours) exceeded the allowable exposure time of
fatigue
-decreased proficiency boundary (FDP), and in about one-third of the drivers the allowable exposure time of exposure limit (EL) was exceeded. These results suggest that long exposure to severe vibration during work is one of the possible causal factors of
LBP
of freight-container drivers.

display a different pattern of mood fluctuations across days than matched healthy controls and whether these mood states are related to pain occurrence and/or magnitude using a prospective design. The questions addressed are whether mood states prior to a pain episode are associated with the episode or whether mood relates to pain as a secondary reaction. patients and an equivalent group of asymptomatic controls matched for age, sex, socioeconomic status, and reported activity level monitored mood state (anxiety, tension, depression, anger, vigor,

, confusion) and pain before breakfast, at 4 p.m. and at bedtime for 14 consecutive days. Groups were successfully matched. Analyses revealed significantly higher levels of tension, anxiety and
fatigue
and lower levels of vigor in the pain cases. No mood state was predictive of pain onset but

was associated with pain 24 h following pain, indicating
fatigue
as secondary to pain. While mood state recorded prior to or following pain was unable to predict magnitude of pain,

was associated with the level of pain experienced during the pain episode itself. The findings reveal a pattern of anxiety, tension and

Five cases of gastric carcinoma (one woman) were investigated. The average age of the patients was 44 and the clinical manifestation were

. Two cases were Borrmann III, and other were Borrmann IV, IIc and IIc advanced, respectively. Three were signet-ring cell carcinoma and the other two were moderately differentiated and well-differentiated adenocarcinoma, respectively. All patients showed leukoerythroblastosis. Bone marrow specimens revealed myelofibrosis in three cases, while adenocarcinoma cells were also found in three. MHA or DIC was found in three patients, all of whom died of bleeding tendency. Mean survival time was 2.6 months. Thus, bone marrow carcinomatosis proved to be very severe.

A case of advanced gastric cancer associated with metastatic carcinomatosis of the bone marrow, microangiopathic hemolytic anemia (MAHA) and disseminated intravascular coagulation (DIC) was reported. A 71-year-old woman complained of

and general

. At the time of admission, besides anemia, jaundice and a tendency to bleeding, the laboratory data showed, hyperbilirubinemia, elevated FDP and hemolytic anemia with fragmented red cells. The bone marrow was infiltrated by carcinoma, and an upper GI series examination showed Borrmann II type gastric cancer on the greater curvature of the antrum. After remission of both MAHA and DIC by mitomycin C, neothramycin and FOY, the patient successfully underwent subtotal gastrectomy. However, she died of cerebral hemorrhage on the 96th postoperative day.

The effect of a "pure" cyclic flexion bending moment on the three-dimensional load-displacement behavior of fresh ligamentous lumbar spine was investigated. The load-displacement behavior, for 11 L1-sacrum specimens, pre- and post-cyclic
fatigue
bending tests were quantified using a Selspot II system. A special fixture was designed to mount the specimen within the MTS system to administer "pure" cyclic flexion bending, under displacement control, for 5 hours. The testing was accomplished in a 100% humidity chamber at 0.5 Hz. The maximum cyclic bending moment, based on the literature dealing with loads experienced by the spine during activities involving lifting, was set at 3.0 Nm. An increase in motion of the order of 10% in the extension loading mode was observed. The increase in motion in other loading modes was not significant. In the extension loading mode, the increase in the anteroposterior displacement (retrodisplacement) in general was higher than the corresponding rotation component. The results suggest that the bending moment of low magnitude, usually experienced by the spine during activities of daily living, alone may not trigger the mechanical failure processes in the disc. The presence of high axial compressive loads on the disc seems to be the main contributing factor in this process. The presence of bending moments and axial twist along with axial compressive load may accelerate the unstable processes leading to
low back pain

A clinical Phase I study of recombinant human interferon alpha A (Ro 22-8181) was performed in patients with malignant tumors; twenty of them received an American product and seven others a domestic product. Both products were administered in single intramuscularly injected doses of 18, 36, 50, 75 and 100 X 10(6)U. Main side effects included fever and influenza-like symptoms (headache, chill/shivering, general
fatigue

lumbago
), and digestive symptoms (anorexia, nausea/vomiting). Numbness of fingers or limbs and somnolence were also observed in higher dose groups, but these symptoms all disappeared on the day of administration or by the 3rd day after administration. Abnormal laboratory findings included leukopenia, granulocytopenia, lymphocytopenia, thrombocytopenia and increased GOT/GPT/LDH, but these returned to normal by the 10th day after administration. The peak blood concentration was correlated with the dose, falling to the base line 72 hr after administration. The American product and the domestic product were nearly comparable in the type and incidence of their side effects, and also produced generally comparable blood concentrations. Furthermore, increased anti-IFN-alpha antibody titer was not observed in any of the patients; and the Prick Test proved negative in all of them. No significant changes were observed in any immunological parameters, either.

Twenty-eight outpatients with chronic severe lumbar pain participated in a double-blind comparative trial on the clinical efficacy of orally administered piroxicam and indomethacin. Half of the patients received indomethacin, 25 mg t.i.d.; the other half received piroxicam 20 mg in the morning and a placebo at lunchtime and before dinner for six weeks. The patients were examined four times at two-week intervals for their capability to do daily tasks and for total lumbar mobility, forward bending, raising of both legs straight and the subjective assessment of pain. Side effects were recorded on a questionnaire and with laboratory tests. The overall results of both groups were similar. Thus, piroxicam, 20 mg daily, matches indomethacin, 25 mg t.i.d., in the treatment of
low back pain
. The side effects were slight. Treatment with indomethacin was stopped in one case (erythema and conjunctivitis). In the piroxicam group diarrhea, constipation, and pain in the tongue were reported, whereas in the indomethacin group gastrointestinal irritation and
tiredness
were typical symptoms.

Furthermore, the bowing posture for strawberry picking was compared with that of eggplant picking using newly devised posture-pattern recording equipment. More than 50% of strawberry or eggplant farmers complained of
fatigue
in the lower back and shoulders. The prevalence of
low back pain
was significantly higher among strawberry farmers than among eggplant farmers, probably due to the deep bowing posture of the farmer during picking work. The new posture-pattern recording equipment proved useful for investigating changes of work postures.
