We tested whether behavioural manifestations of
mental fatigue
may be linked to compromised executive control, which refers to the ability to regulate perceptual and motor processes for goal-directed behaviour. In complex tasks, compromised executive control may become manifest as decreased flexibility and sub-optimal planning. In the study we use the Wisconsin Card Sorting Test (WCST) and the Tower of London (TOL), which respectively measure flexibility (e.g., perseverative errors) and planning. A simple memory task was used as a control measure.
Fatigue
was induced through working for 2 h on cognitively demanding tasks. The results showed that compared to a non-fatigued group, fatigued participants displayed more perseveration on the WCST and showed prolonged planning time on the TOL.
Fatigue
did not affect performance on the simple memory task. These findings indicate compromised executive control under
fatigue
, which may explain the typical errors and sub-optimal performance that are often found in fatigued people.

Fatigue
in tumor patients is caused both by the tumor and/or its treatment, and is accompanied by loss of drive,
lack of energy
, depressive mood and loss of vitality; it is unresponsive to rest and sleep. A differentiation is made between
mental fatigue
, drive
fatigue
and psychological
fatigue
. In particular cytokines produced by the cancer cells, but also by the immune system, play a pathophysiological role. Such cytokines as interferon gamma and interleukin are also involved in the development of cachexia in tumor patients, and may also lead to changes in sleep pattern. An additional factor contributing to the loss of performance and exhaustion in cancer patients is the frequently deficient oxygen supply to the muscles, as also anemia, which is often seen in advanced tumor disease. Mental
fatigue
is also closely associated with depressive mood as well as the noxious influences of cytostatic agents, steroids and painkillers on the CNS. The causes of
fatigue
should always be investigated before appropriate causal treatment can be instituted.

Fatigue
is commonly reported among adults with cerebral palsy (CP) and in the general population. The aim of this study was to examine the prevalence of perceived
fatigue
in adults with CP compared with the general Norwegian population. A multidimensional questionnaire, containing among other instruments the
Fatigue
Questionnaire, was sent to persons with CP aged 18 years or older with no intellectual disabilities documented in their medical records. Results were compared with recently published results of a study of
fatigue
in the general population in Norway. There were 406 respondents (209 males, 197 females; mean age 34 years, SD 11 years; age range 18 to 72 years). Distribution of the different types of CP coincided with international epidemiological studies on CP. Adults with CP reported significantly more physical, but not more
mental fatigue
, than the general population. The strongest predictors associated with
fatigue
were bodily pain, deterioration of functional skills, limitations in emotional and physical role function, and low life satisfaction.
Fatigue
was not significantly associated with type of CP but was most prevalent in persons with a self-reported moderate grade of CP. Results of the present study indicate that physical
fatigue
is a pronounced problem in adults with CP, which has an impact on the preservation of functional skills and life satisfaction.
Fatigue
and factors related to
fatigue
should be addressed specifically in follow-up programmes for persons with CP.

Promoting attentional health is an important opportunity to support and empower women to actively participate in their own health care and function effectively in multiple roles. Attention, the ability to focus or concentrate, is a necessary cognitive resource used throughout the life span in participating in self-health care. However, women can inadvertently overuse this essential capacity, especially when responding to life's multiple demands, resulting in mental or attentional
fatigue
. When women experience this
mental fatigue
, many daily activities, including self-health care, are more difficult. An attentional perspective in women's health allows approaches to improve daily functioning and reduce the risk of attentional
fatigue
. In this article we present the conceptual dimensions of directed attention and its relevance to women's health; practice and research implications are identified.

The aim of the present study was to investigate how "double-shifts" (15.5 hours) affects sleep,
fatigue
and self-rated health. The study was carried out on male construction workers of which 80% were long-distance commuters. The schedule involved two work periods and each work period involved two double shifts in a row. The subjects filled in a sleep/wake diary at 8 times across a year and a questionnaire at 3 times. They also wore an actigraph during one shift cycle. The results showed that sleepiness, and to a certain extent,
mental fatigue
increased during double shifts and accumulated across days. The short rest time (8.5 hours) between days caused insufficient sleep and approximately 5.5 hours of sleep was obtained between double shifts. Questionnaire data showed that complaints of insufficient sleep, exhaustion on awakening and pain symptoms increased across the year. It was concluded that a shift system involving double shifts has a negative effect on
fatigue
, recovery and health-related well-being.

Fatigue
is a major complaint among advanced cancer patients. Several instruments are available for measuring
fatigue
. The EORTC QLQ-C30 is one of the most frequently used health-related quality of life (HRQOL) instruments, and it includes a three-item
fatigue
subscale. Limited knowledge exists about the validity, performance and sensitivity of EORTC QLQ-C30
fatigue
scale as compared with a
fatigue
-specific instrument. The aim of the present study was to validate the EORTC QLQ-C30
fatigue
scale (FA) against the
Fatigue
Questionnaire (FQ). The FQ is frequently used and was developed to measure
fatigue
in both cancer and noncancer populations. The FQ measures physical (PF, seven items) and
mental fatigue
(MF, four items). The study population included two different cohorts: A) patients with advanced metastatic cancer included in a prospective randomized study of palliative radiotherapy (n = 238); B) patients with leukaemia and malignant lymphoma curatively treated with stem-cell transplantation and high-dose chemotherapy (n = 128). The analysis demonstrated that the FA correlated higher with the PF scale (r = 0.67-0.75) as compared with the MF scale (r = 0.49-0.61). The item scale correlations between FA items and the PF scale were consistently higher than between FA items and the MF scale. A factor analysis including all the items within the FA and the FQ identified two factors. All FA items loaded on a PF factor (0.70-0.85). A floor/ ceiling effect, indicating a high number of respondents with lowest, respectively, highest scores was observed more frequently in the FA as compared with the FQ. The PF discriminated better between diagnostic groups with different levels of
fatigue
than the FA did. In conclusion, the EORTC QLQ-C30
fatigue
scale is measuring physical
fatigue
. A floor/ ceiling effect seems to appear for the EORTC QLQ-C30
fatigue
scale. The validity of the EORTC QLQ-C30
fatigue
scale is to be questioned for use in palliative care patients. In studies with
fatigue
as a defined end point, a domain-specific instrument should, therefore, be added.

We investigated the impact of
mental fatigue
on exploration in a complex computer task. The exploration behavior of participants who underwent a
fatigue
manipulation (N = 36) was compared with that of a control (nonfatigued) group (N = 32). A distinction was also made between participants with high or low levels of general computer experience. Results showed that fatigued participants used significantly less systematic exploration and made more errors than did nonfatigued participants.
Fatigued
participants with low computer experience also showed significantly more rigid behavior than did the other participants. No differences were found on the number of subtasks solved. Compared with low-experience participants, highly experienced participants showed significantly more systematic exploration, less unsystematic trial and error, solved more subtasks, and made fewer errors (marginally significant p = .056). Findings were interpreted as the result of reduced task engagement under
fatigue
and reduced involvement of executive control on behavior. Actual or potential applications of this research include guidelines to prevent detrimental effects of
mental fatigue
on exploration behavior.

Free tryptophan (Trp), which is augmented by liberated free fatty acids (FFA) from adipose tissue, can induce
mental fatigue
via serotonin during exercise. Since an attenuation in FFA has been observed with omega-3 fatty acid (n-3fa) use, our purpose was to examine the effect of n-3fa supplementation on free Trp availability and exercise
fatigue
. Ten recreationally trained men ( n=5) and women ( n=5), with maximal oxygen consumption (VO(2max))of 51.6 (3.0) and 44.3 (1.4) ml kg(-1) min(-1), respectively, were studied on two occasions following an overnight fast, before and after n-3fa supplementation (4 g day(-1) for 4 weeks). The exercise trials consisted of a 75-min treadmill run at 60% VO(2max) followed immediately by a high-intensity incremental bout to
fatigue
. Measurements included exercise monitors, plasma volume (PV), triglycerides (TG), FFA, glycerol, lactate, and glucose. Free Trp and branched-chain amino acids (BCAA) were measured and correlated with time to
fatigue
; all blood variables were corrected for PV. Free Trp, lactate, glucose, FFA, and glycerol were not significantly different between trials, but TG ( P<0.001) and the free Trp/BCAA ratio were significantly lower after n-3fa use [1.76 (0.18)x10(-2) microg ml(-1)] versus before supplementation [2.17 (0.22), P=0.033]. There was a non-significant increase in time to
fatigue
after supplementation [10.2 (0.3) min] versus before n-3fa use [9.7 (0.2), P=0.068], and a tendency for higher BCAA levels after supplementation, P=0.068. However, neither free Trp nor the free Trp/BCAA ratio significantly predicted time to
fatigue
. In conclusion, n-3fa supplementation did not diminish free Trp concentrations or significantly improve endurance performance during a maximal bout of exercise.

This study was aimed at confirming the cross-validity and construct validity of the short version of the Japanese version of the Zarit Caregiver Burden Interview (J-ZBI_8) developed by Arai and colleagues. A total of 169 family members caring for disabled elderly were assessed by a self-administered questionnaire which included the Japanese version of the Zarit Caregiver Burden Interview (J-ZBI). Cronbach's alpha of the J-ZBI_8 was 0.88, indicating a high reliability of this instrument. A confirmatory factor analysis revealed that the two factor model of J-ZBI_8 fitted the data. Pearson's correlation coefficient between J-ZBI_8 and J-ZBI was 0.92; and the correlation coefficient between J-ZBI_8 and item 22 (a single global burden) was 0.66. These data indicated that J-ZBI_8 had a high concurrent validity. The J-ZBI_8 was found to be significantly correlated with Barthel Index, duration of caring, hours of caregiving/day, physical
fatigue
and
mental fatigue
among caregivers, but not with the duration of caring (months). Caregivers of disabled elderly who did not have any behavioral disturbances had a significantly lower J-ZBI_8 score than those who were caring for elderly with such behaviors. Also, caregivers who declared that they did not abuse disabled elderly had a significantly lower J-ZBI_8 score than those who replied they abused them, when compared by t-test. These results indicated that J-ZBI_8 had a high construct validity. It is concluded that the J-ZBI_8 had a high cross-validity and construct validity. This instrument can thus facilitate the assessment of family caregiver burden in clinical settings.

Effects of a combination of caffeine and glucose were assessed in two double-blind, placebo-controlled, cross-over studies during extended performance of cognitively demanding tasks. In the first study, 30 participants received two drinks containing carbohydrate and caffeine (68 g/38 mg; 68 g/46 mg, respectively) and a placebo drink, in counter-balanced order, on separate days. In the second study 26 participants received a drink containing 60 g of carbohydrate and 33 mg of caffeine and a placebo drink. In both studies, participants completed a 10-min battery of tasks comprising 2-min versions of Serial 3s and Serial 7s subtraction tasks and a 5-min version of the Rapid Visual Information Processing task (RVIP), plus a rating of '
mental fatigue
', once before the drink and six times in succession commencing 10 min after its consumption. In comparison to placebo, all three active drinks improved the accuracy of RVIP performance and both the drink with the higher level of caffeine in first study and the active drink in the second study resulted in lower ratings of
mental fatigue
. These results indicate that a combination of caffeine and glucose can ameliorate deficits in cognitive performance and subjective
fatigue
during extended periods of cognitive demand.
