The prevalence of perimenstrual symptoms usually is based on reports for one menstrual cycle; the consistency of symptoms across cycles is ignored. The purpose of this investigation was to determine perimenstrual symptoms reported concordantly for two menstrual cycles in a group of 63 presumably healthy women reporting symptoms in health diaries over 2 months. There were only nine symptoms for each of the menstrual and premenstrual phases reported by the same woman across both cycles. Furthermore, concordance of perimenstrual symptom reporting across the two cycles was significant only for
backache
(kappa = .636, p less than .0001), headache (kappa = .849, p less than .001), and cramps (kappa = .899, p less than .001) in the menstruum and for
backache
(kappa = 0.123, p less than .0001), cold sweats (kappa = .500, p less than .0001),
fatigue
(kappa = .135, p less than .0001), depression (kappa = .268, p less than .0002), and tension (kappa = .320, p less than .0001) in the premenstruum. Several symptoms showed high prevalence during the remainder of the cycle which might contribute to the lack of concordance. These data imply that prevalence estimates based on only one menstrual cycle may be inaccurate overall and inadequate as baseline or followup estimates by which to evaluate therapeutic intervention.

In order to clarify the characteristics of the low-
back pain
which is prevalent among freight-container tractor drivers, a medical examination, composed of orthopedic tests and questions about health conditions, was carried out on 231 tractor drivers. Prevailing complaints were: dullness or stiffness of the shoulder (71%); dullness of the neck (69%); dullness of the low back (62%); low-
back pain
(42%); dullness of the lower limbs (39%) and dullness of the back (36%). The rates of complaints of dullness or pain of the neck, shoulder, upper limbs, back or lower limbs, and disturbance of daily activity were associated with the grade of low-
back pain
evaluated from the subjective symptoms. Orthopedic examination revealed
fatigue
signs of muscles i.e. tenderness of the body trunk and limbs, tenderness or percussion pain on the spinous processes, and poor results in some tests on muscle strength. These findings were seen more frequently in the group with subjective symptoms, but even among the group without symptoms, they were evident to some degree. These findings were considered to originate from the fact that the freight-container tractors had many ergonomic problems and the daily driving hours of many drivers were estimated to exceed the allowable vibration exposure time of the ISO.

An ergonomic checklist was prepared to examine safety and comfort of freight-container tractors. With this checklist, 533 tractor drivers were asked to evaluate the tractors to which they were exclusively assigned. In order to check the objectivity and the validity of the results, the correlation between the evaluation of the items on the checklist and the corresponding dimensions on 10 tractors was studied. Fourteen patients with degenerative spondylolisthesis and three patients with degenerative scoliosis, all of whom experienced low-
back pain
, lumbar radiculopathy, and/or intermittent claudication were treated with posterolateral fusion and correction of deformities using a new instrumentation system. This new spinal fixation system combines the advantages of a rod for scoliotic deformities and a plate for sagittal plane disorders. The combination rod-plates can apply multiple forces to facilitate correction of complex deformities as well as enable indirect neurologic decompression. The system allows segmental rigid fixation via transpedicular screws that is limited only to the abnormal vertebral levels to preserve the maximum number of uninvolved lumbar motion segments. The physiologic lumbar lordotic curvature is also preserved. The minimum follow-up period was 1 year. Satisfactory results were obtained in 15 patients (88%). No intraoperative complications occurred. Screw

A multiple crossover research study was used to evaluate the effect of dialyzer re-use on fever, blood leaks, serum urea and creatinine values and symptoms. Each of 6 crossover periods consisted of 4 weeks on either single-use or re-use, 1 week washout, 4 weeks on the alternative treatment and 1 week washout. The re-use consisted of 6 uses of each dialyzer and the washout weeks consisted of 3 single-use sessions. Analysis of paired observations within rather than between patients showed no effects of time (i.e. among crossover periods 1 through 6) or number of re-uses (i.e. among uses 1 through 6). There was no significant difference for temperature change during dialysis, blood leak rate, or the serum urea and creatinine values before the first dialysis of each washout period. There were no differences for symptoms of pruritus, cramps, nausea, headache, chest pain,
backache
or
fatigue
. There were no clinical advantages or disadvantages associated with dialyzer re-use.

Eighteen patients with solid tumours were treated with human recombinant interferon-gamma at escalating dose levels starting at 1 X 10(6) units/m2 per infusion and rising through 3 X 10(6), 6 X 10(6), 9 X 10(6) and 22 X 10(6) to a maximum of 110 X 10(6) units/m2 per infusion. The IV infusions were given three times a week over a 4-week period. Side effects were seen in all patients, but were mild except at the highest dose. Acute dose-related effects included pyrexia,
tiredness
, thirst, chills and rigors. Chronic dose-related effects included anorexia, lethargy, weakness, disorientation, a trace of proteinuria and minimal rises in liver enzymes. In addition, effects were observed which were not related to dose. These included headache, nausea and vomiting,

, myalgia, flatulence and a mild, transient reduction in neutrophils and erythrocytes. At the highest dose level dose-limiting toxicity was observed, consisting in severe
tiredness
and anorexia, hypotension, disorientation and changes on the electrocardiograph. Overall, toxicity was similar to that seen with preparations of interferon-alpha, except that no tolerance to the effects of interferon-gamma was noted. We observed less hepatic and haematological toxicity, but also recorded flatulence, handcramps and electrocardiograph changes, which have not been reported with interferon-alpha. When given according to this regimen, doses of 22 X 10(6) units/m2 per infusion of recombinant interferon-gamma were generally well tolerated by the patients.

. The effects of isodynamic fatiguing of flexion and extension trunk movements on the movement patterns and the motor output of the trunk were investigated. Twenty male subjects with no history of low-
back pain
for the past 6 months volunteered for the study. A triaxial dynamometer was used that simultaneously provided measurement of torque, angular position and velocity of each axis. Resistances were set independently for each axis by an interfaced computer. The subjects performed trunk flexion and extension movement against a sagittal plane resistance equal to 70% of their maximum isometric extension strength in the upright position. The minimum resistances in the coronal and transverse planes were set up at 7 Newton meters. The subjects were asked to perform trunk movement as quickly and as accurately as possible while exerting the maximum efforts until exhaustion. Analysis of variance, the MANOVA procedure with a repeated measure design, was performed among the selected parameters of the first, middle and last three repetition cycles. The selected parameters are the trunk motor output and movement patterns; the total angular excursion, range of motion, maximum and average torque and angular velocity of the trunk. All the selected parameters were significantly reduced in the sagittal plane. Subjects displayed significantly less motor control and greater range of motion in the coronal and transverse planes in performing the primary task of flexion and extension. The reduction in the functional capacity of the primary muscles performing the required task is compensated by secondary muscle groups and the spinal structure is loaded in a more injury prone pattern, as identified by finite element models. In addition it is suggested that the fatigued muscles would be less able to compensate any perturbation in the load or position of the trunk. The repetitive loading results in a weakening of the viscoelastic passive elements of the spineless structure. The loss of ability to protect these weakened passive elements makes the spine susceptible to industrial and recreational injuries.

Orbital venous vasculitis has been suggested to cause characteristic periorbital pain in patients with pathologic changes in their orbital phlebograms. The orbital pain is characterized by being unilateral, not shifting side, boring and pressing, but not throbbing, increasing on eye strain, exposure to cold, or weather changes, and resistant to analgesics. It is ameliorated by steroids. Fifty patients with symptoms of orbital venous vasculitis were investigated for other symptoms that could be related to the vasculitis. When the 32 female patients were compared with a randomly selected age- and sex-matched control group, there was a significant increase of symptoms of chronic
fatigue
, cold feet, gut problems such as constipation and/or diarrhea, arthralgia, memory impairment, rotatory vertigo, spontaneous ecchymoses (all, p less than 0.0001),

(p less than 0.012), and thrombophlebitis (p less than 0.022) in the patient group. These symptoms, although commonly occurring, seem in these patients to be related to the vasculitis. Blood tests of the fifty patients showed signs of inflammation which did not disagree with the hypothesis of an immunologic cause of the orbital venous vasculitis.

Fifty-four patients were treated by a standardized single stage anterior opening wedge and a posterior closing extension wedge osteotomy for
back pain
associated with postoperative loss of lumbar lordosis (iatrogenic flat back syndrome). Presenting complaints were
fatigue
, pain and a stooped posture. Etiological factors were, in descending order of frequency, distraction instrumentation with the lower end at the L5 or S1 vertebra, thoracolumbar junction kyphosis greater than 15 degrees, especially if associated with a hypokyphotic thoracic spine, and degenerative changes above and below a previous fusion. Kostuik-Harrington instrumentation was used anteriorly for the opening wedge and Dwyer cables and screws together with a midline plate were used posteriorly for the closing extension osteotomy. Malunion occurred in three patients, one requiring recorrection. Pain relief occurred in 48 (90%). Neurological complications occurred in two patients, one with permanent deficient. Follow-up averaged 4 years. Average preosteotomy lordosis L1-S1 was 21.5 degrees and was restored to 49 degrees (equal to the lordosis before the initial surgery) for an average correction of 29 degrees, (range 24 degrees to 63 degrees). Prevention of this complication can be accomplished by maintaining normal lordosis at the time of initial surgery.

Computed tomographic (CT) findings of cerebral and cerebellar calcification are described in three American adults with raised serum lead levels and known exposure to lead for 30 or more years. Calcification patterns were punctiform, curvilinear, speck-like, and diffuse and were found in the subcortical area, basal ganglia, vermis, and cerebellum. Admission serum lead levels ranged from 54 to 72 micrograms/dl (normal, 0-30 micrograms/dl).
