The first autopsy of a case of multiple sclerosis from the District of Hokuriku was reported. The patient, a 50-year-old house-wife, born in Toyama Prefecture, had noticed a paresthesia of her face,
fatigue
, numbness and weakness in the right limbs, dimness of vision and gait disturbance at ave 44. Furthermore, in the course of the disease, she had suffered from visual disorder, tetraplegia, hyperreflexia, pyramidal signs and cerebellar syndroms such as dysarthria, nystagmus, intention tremor and ataxia. She also showed symptoms of euphoria and
dementia
. After a course of six years she died of bronchopneumonia. Remissions and exacerbations were noted four times during her clinical history. Histopathologically, there were many recent and old demyelinating lesions of varying sizes and shapes in all parts of the central nervous system, namely the cerebrum, brainstem, spinal cord and optic nerve. In contrast to the clinical symptoms, the cerebellum itself revealed less plaques than the other areas of the brain. According to the observed distributions of the lesions, our case can be classified as belonging to the optico-cerebro-spinal type in the Ikuta and Zimmerman classification. The demyelinated lesions were characterized by a perivenular distribution of the plaques, lack of tissue necrosis, paucity of inflammatory reaction and marked fibrous gliosis of varying degrees.

Health conditions of primary caregivers for elderly aged 65 and over living at home were surveyed. Subjects were 167 caregivers in Osaka prefecture. Results obtained were as follows. 1) The primary caregivers were largely associated with and varied with the sex of the cared elderly. The caregivers for the female elderly were almost daughters-in-law or daughters. The majority of persons with
dementia
are cared for in the home by family and friends. The goals of treatment in this setting are to maximize the quality of life of the demented person and minimize burdens on the caregiver. Behavioral problems are common with
dementia
and can lead to significant caregiver burden. Behaviors that are most common or most serious to caregivers include behaviors related to memory disturbances, restlessness and agitation, catastrophic reactions, day/night disturbances, delusions, wandering, and physical violence. A general method for clinicians to manage these problems involves the identification of the behavior and its antecedent and consequent events. Stressors that may cause behavioral problems include
fatigue
, a change of routine, excessive demands, overwhelming stimuli, and acute illness or pain. Caregivers can be taught to identify these stressors in order to prevent or alleviate troublesome behaviors. When behavioral techniques are not successful and the behaviors are particularly dangerous or burdensome, therapy with low doses of high-potency neuroleptics has been suggested. Measures such as these to help caregivers in the management of
dementia
at home can be instrumental in improving the quality of life for the person with
dementia

Trichosanthin, a ribosomal inhibitor protein, blocks HIV replication in lymphocytes and macrophages. This agent was used to treat 51 patients with advanced HIV disease in a dose-escalation study in which three injections were administered over a 9-21-day period in a dose range of 10-30 micrograms/kg per injection. The maximum tolerated dose was estimated to be 30 micrograms/kg. Reversible but severe
fatigue
and myalgias were the major dose-limiting side-effects; mild leucocytosis and elevations in serum transaminases were noted and were reversible. Non-dose-related reversible mental status changes were seen in six patients and were considered to be associated with the drug. This was usually manifest as
dementia
, but progressed to coma in two patients. This reversed, but the sequelae resulted in death in one patient. Decreases in serum p24 antigen levels were noted 1 month after the first infusion in 10 of 18 patients who entered the study with elevated levels; one converted to negative. Values usually remained low to the end of the study period (2 months). In those patients with CD4+ cell levels greater than 50 x 10(6) cells/l significant decreases in sedimentation rate and increases in CD4+ cell numbers were also noted. These changes were found at all dose levels but only in patients receiving three infusions.

Among 78 carefully diagnosed
dementia
of Alzheimer type (DAT) patients, periodic synchronous discharges (PSD) on electroencephalograms (EEGs) were observed in 12 advanced cases. Although several case studies have been made since 1970, it is not widely recognized that PSD can be observed in advanced DAT patients. In this study the clinical picture, the nature of PSD, background activity of EEGs and 2 autopsied cases have been clinicopathologically investigated. The mean duration of the disease in the 12 cases was 6.7 years. The mean age of onset of the disease was 67 years of age. Among the 12 cases, 9 patients presented akinetic mutism or symptoms closely resembling akinetic mutism, who were completely bed-ridden with their extremities fixed in flexion. The other 3 patients were severely demented. They presented various degrees of Parkinsonian syndromes or paratonic rigidity accompanied by some infection and general
fatigue
. In one of these 3 patients, PSD were observed during the transient apallic state and the PSD themselves were also transient. The basic rhythm in EEGs were 4-6 c/s: middle amplitude theta waves dominant with delta waves on frontal leads. Triphasic delta, theta and sharp waves of PSD were dominant on centro-parieto-occipital leads. Background activity was not suppressed differing from that of Creutzfeld Jacob disease (CJD). According to the interval histograms of PSD, the majority presented an interval of 1-2 s (1.5 s mean) with some lability which was relatively slower than that of CJD. In view of some controversies still existing about the real efficacy of ergot derivatives in the management of
dementia
, a double-blind, randomized, parallel group trial extending up to 6 months was carried out to compare the effects of nicergoline, 60 mg daily, and placebo in 315 patients suffering from mild to moderate
dementia
. Clinical evaluation was performed by the SCAG scale. The trial, which included a 1-month placebo run-in period, showed that both placebo and nicergoline were associated with some degree of improvement. The effect of nicergoline, however, was significantly greater and more sustained, steadily increasing with time. In particular, the difference between nicergoline and placebo in mean total SCAG score was 5.5 at 3 months (95% confidence interval: 3.6-7.4) and increased to 9.8 at 6 months (95% confidence interval: 7.8-11.8). A comparison of nicergoline versus placebo in the frequencies of changes in each item of the SCAG showed also a significant difference at 6 months, the percent of patients displaying an improvement by at least 2 points ranging from 13.5 (bothersome) to 30.2 (disorientation) in nicergoline group, against 4.1 (self-care) to 14.3 (
fatigue
) in placebo group. The safety of nicergoline, as judged by hemodynamic changes and drug-related adverse reactions, was quite satisfactory.
