Prospective data on 86 pregnant women were analysed to investigate the relationship between the pregnancy symptoms of nausea and vomiting and various demographic, social and psychological factors. In contrast to previous research findings no significant differences were discovered along demographic or social dimensions between women reporting no nausea, mild nausea and severe nausea during the first trimester of pregnancy. and irritability. Women having both nausea and vomiting during the first trimester, reported significantly more unplanned, undesired pregnancies and negative relationships with their own mothers, than first trimester groups experiencing nausea only or no symptoms. Finally, those with continuing nausea and vomiting in the third trimester, when compared with those having no symptoms, tended to have reported significantly more psychological/psychiatric problems in the first trimester and were significantly more negative in their assessment of their own maternal relationship.

Twenty-five adults with a deviated nasal septum, who complained about excessive daytime sleepiness, chronic

, and nocturnal insomnia, were studied for one to two nights in a sleep laboratory. Recordings disclosed disordered breathing during sleep in the form of pronounced periodic breathing of alternating hypopneas and hyperpneas, isolated hypopneic episodes and central apneas and periodic sighs, all combined with electrophysiologic "microarousals," and a mixture of alpha and delta EEG wave activities. Surgical treatment of the deviated septum in 14 patients resulted in a subjective improvement in the level of diurnal alertness and in the quality of nocturnal sleep in 12 patients. Follow-up sleep recordings in seven of the patients who reported subjective improvement in sleep disclosed notably less waking and abnormal breathing during sleep. Nocturnal myoclonus has been identified as a cause of

. To our knowledge, no therapy has heretofore been available to adequately control this problem. We identified this problem clinically in two patients; in one, the phenomenon was observed with a polygraphic sleep recording. Small doses of clonazepam, 1 mg, before retiring were found to specifically control the myoclonus and allow sleep patterns to return to normal.

Out of a regional traumatic spinal cord injury population consisting of 379 individuals, 353 (93.1%) participated in the present study. Subjects were individually interviewed using semi-structured protocols. In addition, previous medical records were available for over 96% of subjects, and were used in all these cases to minimise recall bias. Cause of injury, prevalence of present medical symptoms and occurrence of medical complications in the post-acute, post-discharge phase were recorded. Neurological classification was verified by physical examination according to ASIA/IMSOP standards. Many subjects had experienced complications since discharge from initial hospitalisation, especially urinary tract infections, decubitus ulcers, urolithiasis, and neurological deterioration. Prevalence of medical symptoms was also high. More than 41% of subjects with spastic paralysis reported excessive spasticity to be associated with additional functional impairment and/or pain. Almost two-thirds of subjects reported significant pain, with a predominance of neurogenic-type pain. Bladder and bowel dysfunction were each rated by nearly 41% of subjects as a moderate to severe life problem. As expected, sexual dysfunction was also commonly reported. . However, lack of adequate normative data precludes comparison with the general population. The goal of the present study was to detect signs and symptoms of mental fatigue in nurses working in the hospital with shift schedule. The signs and symptoms of mental fatigue were measured by a subjective indicator, composed of a check list and two objective indicators called critical frequency of light fusion (Flicker) and Reaction Time to auditive stimulus (RTs). During fifteen consecutive days, morning, afternoon and night shifts of 12 nurses working in three different units of a university hospital were analyzed. The great rotation in work schedule, when in just one week a nurse can work up to three different shifts, is hazardous to the health, social, familiar and professional life of those nurses, who were not satisfied with the work schedule adopted by the institution and showed signs and symptoms of mental fatigue. We have performed a study assessing the prevalence of sleep-disordered breathing in a large US trucking company using a validated portable monitor (MESAM-4) and a validated symptom questionnaire. Three hundred eighty-eight drivers with a mean age of 36 years filled out the questionnaire. One hundred fifty-nine drivers with a mean age of 35 years spent the night at the terminal hub where they underwent monitoring for identification of sleep-disordered breathing. The drivers also had blood pressure recorded while awake, seated, and after 15 min of quiet rest. Seventy-eight percent of the drivers had an oxygen desaturation index (ODI) > or = 5 per hour of sleep; 10% had an ODI > or = 30 per hour of sleep. There was a significant difference in the body mass index (BMI) between drivers with ODI < 5 and drivers with ODI > or = 5 (25.7 +/- 6.0 kg/m2 in drivers with ODI < 5 vs 29.0 +/- 6.3 kg/m2 in drivers with ODI > or = 5, p < 0.001). Sixteen percent of all drivers tested were hypertensive. Twelve percent were unaware of their hypertension. Hypertensive drivers were significantly more overweight (p < 0.0001), slept more restlessly (p < 0.04), took more naps (p < 0.03), and woke up more frequently during the night (p < 0.005). In a randomized, placebo-controlled, double-blind study, 39 patients with depression with somatic symptoms were treated with hypericum extract LI 160. The therapy lasted for 4 weeks; the dosage was 300 mg three times daily. At the onset of the study as well as after 2 and 4 weeks, the following criteria were analyzed: HAMD, B-L, CGI, and vegetative symptoms. The results show a significant improvement in the active treatment group at the 5% level as compared to placebo. Seventy percent of the patients treated with LI 160 were free of symptoms after 4 weeks. Typical symptoms of the depression such as lack of activity,
