from carbon paper is an exceptional rarity; the reported causes are dyes and plasticisers. But carbon paper is gradually being replaced by pressure-sensitive or carbonless copy paper, particularly for business forms. The method depends on mechanical pressure or chemical transfer. For the latter, colour formers are held in microcapsules, which rupture under pressure, and released when they produce the colour. Some office workers have claimed that the handling of used carbonless papers are responsible for symptoms affecting the skin, eyes, and upper respiratory tract, as well as systemic symptoms including headaches, drowsiness and
fatigue
. Patch tests are negative. If the symptoms are attributable to carbonless paper it is suggested that the colour former solvents are most likely to be responsible.

Eleven nickel-hypersensitive patients with chronic, dyshidrotic hand eczema aggravated by oral challenge with 0.6-2.5 mg nickel were treated with 100 mg tetraethylthiuramdisulfide (Antabuse) two to four times daily for 4-10 weeks. Nine of the patients experienced a flare of the dermatitis shortly after initiation of the treatment. During the course of treatment the dermatitis of seven patients cleared, improvement was seen in two patients, and in two the dermatitis remained unchanged. Flare was seen in six patients when the treatment was discontinued. Seven patients experienced side effects such as
fatigue
, headache and dizziness. The treatment of four patients was discontinued due to side effects. During the treatment high levels of nickel were found in the serum and urine.

A literature review of experimental and human exposure studies of skin permeation and cutaneous hypersensitivity reactions evoked by chromium was carried out to provide a basis for making a risk assessment of chromium as a soil contaminant. In vitro and in vivo studies demonstrated that 1 to 4% of the applied dose of hexavalent and trivalent chromium to guinea pig skin penetrated skin within 5 to 24 hr after application. Ultrastructural investigations showed that hexavalent chromium localized intracellularly and extracellularly in the upper layers of guinea pig epidermis. Only minute quantities of hexavalent chromium are required to elicit a positive hypersensitivity reaction in susceptible individuals; using a patch dose of 20 micrograms, only 2 micrograms were required to evoke a positive skin reaction in hypersensitive subjects. The potential of hexavalent chromium to produce a skin sensitization reaction is readily demonstrated using animal models. The incidence and characteristics of chromium-induced skin hypersensitivity as a clinical entity are described. A double-blind, double-dummy, parallel-group trial that used a predominantly Hispanic patient population with mild-to-moderate hypertension was designed to compare the efficacy, safety, and acceptability of monotherapy with either transdermal clonidine applied once a week or terazosin tablets taken once a day. Of 44 patients admitted, 20 of 22 in the transdermal clonidine group and 15 of 21 in the terazosin group (one patient was lost to follow-up) met the response criteria. These criteria included the completion of 1 full week of therapy with the smallest dose that resulted in a seated diastolic blood pressure of <90 mm Hg and the subsequent entrance into an 8-week maintenance therapy phase. At the end of the 8-week maintenance phase the mean seated diastolic blood pressure remained <90 mm Hg in the clonidine cohort but not in the terazosin cohort, yet the difference was not statistically significant. There was a significantly better response to clonidine when all patients who received treatment were considered. No adverse first-dose effects were experienced with terazosin. One patient who received transdermal clonidine developed
contact dermatitis
and withdrew from the study prematurely. The most common side effects associated with clonidine were dry mouth and

while those associated with terazosin were headache and
fatigue
. Compliance was excellent in both groups. Seventy-nine percent of patients found transdermal clonidine preferable to the oral regimen.

Breast cancer most often presents as a palpable mass or with an abnormal mammogram. Much less commonly, breast cancer may present as a paraneoplastic syndrome. Dermatomyositis (DM) is a rare disease most often considered a complement-mediated idiopathic inflammatory myopathy manifested by classic skin findings and proximal muscle weakness. However, DM may also be due to a paraneoplastic syndrome associated with an underlying malignancy. The authors present a case report of a woman with presumed

who was diagnosed with breast cancer in the setting of progressive

and muscle weakness. DM was subsequently diagnosed. Treatment of DM simultaneous with treatment of the breast cancer led to regression of DM. The diagnosis of DM in an adult should raise suspicion of an underlying malignancy. Breast cancer is a common disease that may rarely present with uncommon features that may divert attention from the underlying malignancy.

Claims of benefits of aromatherapy for cancer patients include reduced anxiety levels and relief of emotional stress, pain, muscular tension and
fatigue
. The objective of this paper is to provide an updated descriptive, systematic review of evidence from pre-clinical and clinical trials assessing the benefits and safety of aromatherapy for cancer patients. Literature databases such as Medline (via Ovid), the Cochrane database of systematic reviews, Cochrane Central were searched from their inception until October 2010. Only studies on cancer cells or cancer patients were included. There is no long lasting effect of aromatherapy massage, while short term improvements were reported for general well being, anxiety and depression up to 8 weeks after treatment. The reviewed studies indicate short-term effects of aromatherapy on depression, anxiety and overall wellbeing. Specifically, some clinical trials found an increase in patient-identified symptom relief, psychological wellbeing and improved sleep. Furthermore, some found a short-term improvement (up to 2 weeks after treatment) in anxiety and depression scores and better pain control. Although essential oils have generally shown minimal adverse effects, potential risks include ingesting large amounts (intentional misuse); local skin irritation, especially with prolonged skin contact; allergic

; and phototoxicity from reaction to sunlight (some oils). Repeated topical administration of lavender and tea tree oil was associated with reversible prepubertal gynecomastia.

There has been little epidemiological evidence that has comprehensively clarified whether alterations in lifestyle, such as sleep quality and dietary intake, explain changes in the skin condition of healthy young adults. Therefore, the aim of our study was to elucidate the association between skin condition and lifestyle behaviors such as diet and sleep, after statistically controlling confounding factors. The subjects were 54 participants who were aged 20-32 y, and who attended college in the Kanto region (in Japan). guidelines for TEWL measurements. Self-administered questionnaires on demographic characteristics, dietary habits, and health status, such as sleep condition, depression, and

, were handed to participants on the day of measurement. We found that lower sleep efficiency was significantly associated with higher TEWL (p=0.023), while other demographic factors, dietary intakes, and eating behaviors were not (p>0.05). This significant association remained unchanged after controlling for confounding variables, such as sex, BMI, and dietary intake. Our findings may have important implications in the development of valuable health strategies that may suggest behavior modifications for young to middle-aged men and women.
