We evaluated diagnostic X-ray and endoscopic examination findings in 486 patients with
esophageal achalasia
. Concerning the association between the duration of disease and the X-ray dilatation type, the duration was 1-4 years in more than 50% of patients with the Sp type, a mean of 8.5 years in those with the F type, and frequently more than 10 years in those with the S type. Endoscopy is generally used to determine the presence or absence of abnormal movement. In
achalasia
, the endoscope can be inserted into the stomach despite resistance at the stenotic site, and the mucosal surface is normal. Squamous cell carcinoma as a complication was observed in 21 patients (4.3%). The carcinoma complication rate was higher with a longer duration of disease and a longer observation period. The mean total course including the postoperative course was 27 years. Long-term and periodic X-ray and endoscopic observation of the disease course is important, and
iodine
staining is indispensable for early detection of esophageal cancer.

Squamous cell cancer is the most common neoplasm of the oesophagus worldwide, with an enormous variation in its global incidence. Several risk factors, such as
achalasia
, Plummer-Vinson syndrome, coeliac disease and nutritional factors, have been identified. The surveillance of patients, especially those with tylosis or caustic ingestion, has been recommended. Vital staining with
iodine
may improve the diagnosis of early cancer. The endoscopic management of early cancer and dysplasia by minimal invasive techniques such as photodynamic therapy or mucosal resection has become attractive for many of these patients with co-morbidity.

Endoscopic mucosal resection of the esophagus was found to be safe and easy to perform. Efforts must be made to detect early m1 to m2 cancers, which are indicated for EEMR. It is necessary to perform periodic endoscopic examination. During endoscopic examination, it is important to wash the inside of the esophagus with water and perform careful observation. Also, in high-risk patients and patients with abnormalities, such as erythema, turbidity, or hypervascularity,
iodine
staining should be performed frequently. Patients at high risk for esophageal cancer include (1) men more than 55 years old who are heavy smokers and drinkers; (2) patients with cancer of the head and neck region; and (3) individuals with a family history of cancer and those with
achalasia
, corrosive esophagitis, or Barrett's esophagus.

The present study involved a total of 25 patients presenting with
oesophageal achalasia
who had undergone balloon cardiodilation. The complex of rehabilitative measures concluded the application of an ultrahigh-frequency electromagnetic fields (decimeter wave (DMW) therapy) to the collar region and general
iodine
bromide baths. The treatment resulted in the elimination of dysphagia syndrome during consumption of solid food in 80% of the patients. Simultaneously, the oesophagogastroscopic study revealed the improvement of the state of oesophageal mucosa. Moreover, the thyrotropin level was normalized. The positive effect of such rehabilitative treatment persisted during 6-8 months.

We present a case in which we used a thoracoscopic approach for resection of multiple esophageal carcinomas diagnosed 33 years after surgery for
esophageal achalasia
. A 68-year-old Japanese man had been diagnosed with
esophageal achalasia
and underwent surgical treatment 33 years earlier. He was examined at our hospital for annual routine checkup in which upper gastrointestinal endoscopy showed a "0-IIb+IIa" lesion in the middle esophagus.
Iodine
staining revealed multiple irregularly shaped
iodine
-unstained areas, the diagnosis of which was esophageal carcinoma. Thoracoscopic subtotal esophagectomy was performed. Esophageal carcinoma may occur many years after surgery for
esophageal achalasia
, even if the passage symptoms have improved. So, long-term periodic follow-up is necessary for detection of carcinoma at an earlier stage.
