A case of a 47-year-old male admitted to our department due to acute myocardial infarction, treated effectively with primary angioplasty, is presented. Three weeks earlier the patient was admitted to another hospital with a suspicion of infective
endocarditis
. Echocardiography performed in our department after the patient recovered from acute phase of MI revealed the presence of mitral valve abscess which caused valve perforation. The patient was transferred to another institution for cardiac surgery.

The aim of this study was to establish the indications to use of pulmonary autograft in the treatment of aortic valve disease in the adolescents and adults; early and mid-term an evaluation of autograft's function and function of left ventricle after Ross operation. In this procedure the patient's own pulmonary valve is removed together with the main trunk of the pulmonary artery and used to replace the aortic valve. Pulmonary homograft is used to reconstruction of right ventricular outflow tract. Between February 1994 and march 2001 in 1st Cardiac Surgery Department in 43 patients over 14 years of age Ross operation have performed. Mean age was 24.9 years. Indications for operation were predominant aortic stenosis in 29 patients, aortic incompetence in 7 patients, and complex aortic valve disease in 7 patients. Hospital mortality was 4.6%. In mid-term observation 41 patients left. All survivors remained in New York Heart Association functional class I and were free of complications and medications. In the all patients autograft's valve have functioned excellent (without or small regurgitation). Echocardiographic parameters of left ventricular function improved in the all patients. The indications to pulmonary autograft insertion in the adolescents and adults are small aortic ring, infective
endocarditis
and contraindications to anticoagulants using. The early and mid-term results confirm a high efficacy Ross operation in this age's group with low mortality. It completely relieves the abnormal loading conditions of the left ventricle, resulting in a complete recovery of left ventricular function.

A case of a 75-year-old male with a history of aortic valve disease hospitalised due to pyrexia of unknown origin is reported. Initially the patient was diagnosed with infective
endocarditis
due to persistent pyrexia, history of valve disease, single positive blood culture and echocardiographic scan suggesting infective
endocarditis
. Treatment with two strong antibiotics did not bring expected amelioration. Patient's hemodynamic status remained unchanged. After several weeks of hospitalization X-ray scan revealed involved hilar lymph nodes, which was confirmed by CT scan of the chest. The patient was diagnosed with Hodgkin's disease. He died three months after initiation of chemotherapy.

, persistent atrial fibrillation requiring pacemaker implantation and total a-v node ablation as well as ischaemic stroke which occurred one year before present hospitalisation. This time the patient was admitted to the hospital due to progressive heart failure. Transesophageal echocardiography showed a cyclic intermittent opening of both prosthetic valves full opening was present during every second cardiac cycle. The same phenomenon was documented using plethysmographic recording of a pulse wave from a finger. The patient underwent prosthetic valve replacement. Intraoperatively, a fibrous tissue ingrowth was detected.

A case of a 5-year-old girl with severe dysfunction of aortic valve in Kawasaki disease coexisting with

, is described. The role of Ross operation in the treatment of this condition is discussed. The 18-months follow-up showed good function of aortic valve and "Contegra" conduit (bovine jugular vein), but long-term follow-up of patients with "Contegra" conduit remains unknown. In conclusion, a Ross operation using "Contegra" conduit in pulmonary position could be effective method in the treatment of dysfunction of aortic valve in Kawasaki disease coexisting with
endocarditis
in children.

A case of 47-year old woman operated on because of a chronic left ventricular false aneurysm caused by Staphylococcus aureus septicemia and
endocarditis
8-years earlier is described. After septicemia was cured, clinical status improved so markedly that the patient refused to undergo recommended operation until onset of heart failure (NYHA III). She was operated on from the median sternotomy with the use of cardiopulmonary bypass. After pericardial adhesions were dissected free, the large left ventricular false aneurysm with severely calcified wall was found. The aneurysm was excised completely and its orifice closed with non-absorbable monofilament 3-0 suture. Postoperative course was complicated by epileptic attack accompanied by loss of consciousness and left hemiplegia on 4th day after surgery. Neurological symptoms regressed within 48 hours and on 12th postoperative day she was discharged from a hospital in a good clinical status.
