The authors report one observation of endocarditis due to Hemophilus para-influenzae associated with mitral valve prolapsus. This germ is difficult to isolate and is found late in cultures using standard techniques. The dicovery of germs resistant to ampicillin requires the search for the presence of a betalactamase. When resistant germs are found, the antibiotic of choice is chloramphenicol or cefamandole. This type of endocarditis different from the others on account of the risk major of embolism (60-85 p. 100) which justifies valve replacement when the echography reveals persistant vegetations.

The authors report a case of septicemia due to Cardiobacterium Hominis. This commensal germ of the respiratory system is rarely the cause of pathologic diseases. All the organic manifestations described are included in endocarditis due to Cardiobacterium Hominis but the above mentioned observation is characterized by the absence of endocarditis.

A new case of septicemia (without endocarditis) due to Actinobacillus actinomycetum comitans is described. The patient was a 59-year-old man with an intraventricular pacemaker. He was successfully treated by a combination of gentamicin and ampicillin. Human infections due to this demanding and slow-growing micro-organism, which is not related to actinomycosis, are only exceptionally reported. In 73% of the cases the endocarditis is subacute. The study of the literature demonstrates the prevalence of male patients and the usual absence of leukocytosis.

Three cases of endocarditis due to Neisseria mucosa are reported. The pathogenic role of these commensal microorganisms is reviewed. A survey of the literature found a dozen cases of endocarditis due to commensal Neisseria before the antibiotic era, and fourteen more since then (not including our three observations). Some of distinctive features can be delineated from the study of cases seen during the second period. Patients mean age is fairly low (34 years), incidence of cutaneous manifestations is high, and vascular complications (mainly embolism) are common (52, 1% of the cases). Overall prognosis is good and the recovery rate (82 %) is similar to that seen in non-D Streptococcus endocarditis.

Five cases of Brucella melitensis endocarditis are reported. Common features included subacute course, enhanced humoral immunity, and deficient cellular immunity. Appropriate antibiotic therapy led to recovery in all cases. In two patients, valvular replacement was necessary.

The authors report a case of endocarditis due to Corynebacterium diphtheriae. THe bacteriological study is difficult since this micro-organism can be undistinguishable from a saprophytic diphtheroid. The observed case is compared to previously published observations. All have in common the great severity of the illness, and the high mortality rate. Despite the clinical symptoms and the cerebrospinal fluid (CSF) abnormalities, meningitic involvement could not be demonstrated.

A change in any of the parts of the mitral valve apparatus may cause systolic leakage: the mitral ring, the large and small valve, the papillary muscles and the chordae tendinae, hence the possibility of a large number of anatomical and physiological mechanisms and many possible etiologies in the onset of mitral incompetency. For each of the four anatomical constituents of the mitral valve these mechanisms are successively reviewed and their etiologies considered, as various as the sequelae of rheumatic or infective endocarditis, metamorphic calcifications, mucoid degeneration, ischemic or other myocardial changes, constitutional abnormalities. Each type of mitral incompetency has a variable effect on the heart cavities and the pulmonary circulation.

Streptococci remain the bacterial group the most frequently associated with microbial endocarditis. This paper points out the ability of the different species to cause endocarditis. 128 streptococci are isolated from patients with microbial endocarditis (127 from blood cultures, 1 from a valve). Isolates are speciated by biochemical tests and the determination of their group antigen. The viridans streptococci most frequently isolated are S. sanguis I (27 p. cent), S. mitis (16,5 p. cent) and S. sanguis II (9,6 p cent). A comparative survey of the results is rather difficult due to the methodological and taxonomical divergences.

Two new cases of spondylitis associated with endocarditis are reported. Approximately 30 cases have already been published in the medical literature. Although this association is infrequent, the possibility of its occurrence calls for repeated auscultation in patients with spondylitis and focal roentgenograms in patient with bacterial endocarditis who develop vertebral pain.
