The incidence of significant bacteremia following routine replacement of an IUD was assessed in a sample of 23 female patients recruited from 1 medical practice. The replacement device was a Gravigard in 12 cases and a Nova T in 11 cases. Blood samples were collected at the start of the procedure, after removal of the old IUD, and at 2, 5, 7, and 10 minutes after insertion of the new coil. Nontouch aseptic technique was used throughout. Transient bacteremia resulting from vaginal organisms (
Gardnerella
vaginalis and Bacteroides) was found in 3 (13%) women 4-6 minutes after insertion of the new device. No significant associations were noted between bacteremia and parity, the duration of use of the old IUD, or the type of the new device. Previous studies that failed to demonstrate bacteremia have looked at either 1st insertion or removal of IUDs, not replacement. Study findings suggest a need for antibiotic prophylaxis for IUD insertion and replacement in women with prosthetic heart valves, previous cardiac surgery, or a history of drug abuse. A history of pelvic inflammatory disease or infective
endocarditis
should be an absolute contraindication to IUD insertion.
