The use of anti-tumor necrosis factor agents such as infliximab as treatment modalities of inflammatory joint diseases has widely spread over the past few years. However, increasing numbers of reports of infectious complications during TNF-a blockade have also highlighted the fact that an increased rate of sometimes life-threatening complications may be the price paid for superior therapeutic efficacy. We report the first case report of Listeria
endocarditis
associated with infliximab use and the second published case of Listeria infection associated with infliximab in patients with
psoriatic arthritis
. We also summarize the literature regarding the association of Listeria infection with use of infliximab. Further studies are needed to elucidate the contribution of anti-TNF-a therapy to development of listeriosis. Physicians should be aware of the possibility of Listeria infection in individuals receiving anti-TNF therapy.

Whipple's disease is a chronic condition that is characterized by diarrhea, weight loss and arthropathy, and caused by infection with the fastidious bacterium Tropheryma whipplei. Although once rare, Whipple's disease is being increasingly described owing mainly to advances in molecular genetics and an improved isolation of the organism. Whilst cardiac Whipple's disease occurs less commonly, especially in the absence of gastrointestinal symptoms, it has become apparent that some cases of culture-negative
endocarditis
may well be attributable to T. whipplei. The case is reported of a patient with Whipple's disease
endocarditis
in association with
psoriatic arthritis
and lumbar discitis.

Many rheumatic diseases show changes and are visible in the hands. The pattern of distribution in the relevant joints, soft-tissue changes, skin manifestations, neurological and vascular symptoms and clinical findings provide fundamental information. Imaging and lab results provide diagnostic support. In this review, common diseases are presented in terms of their clinical expressions in the hands: osteoarthritis, rheumatoid arthritis, gout, calcium pyrophosphate dihydrate deposition disease,
psoriatic arthritis
, reactive arthritis, systemic sclerosis, dermatomyositis/polymyositis and systemic lupus erythematosus. Furthermore, we discuss pathological findings of the hands as a result of diabetic cheiroarthropathia,
endocarditis
, secondary hypertrophic osteo-arthropathy and chronic regional pain syndrom.

The paper describes the case of a patient affected by a combination of genetic and autoimmune diseases (multiple sclerosis,
psoriatic arthritis
, Leiden V mutation, and sicca syndrome) and hypertension. The
psoriatic arthritis
was treated with celecoxib and multiple sclerosis with fingolimod. The patient developed high fever and
endocarditis
, resulting in severe mitral regurgitation, atrial fibrillation, and congestive heart failure. Evidence is suggestive of adverse effects of potent immunosuppressive and anti-inflammatory therapies with biologic agents and the cardiovascular system. Fingolimod increases susceptibility to infections and induced
endocarditis
resulting in severe mitral regurgitation, atrial fibrillation, and congestive heart failure. Managed care systems limit the contact among basic care physicians and specialists. However, the process by which the optimal decision may be reached for a patient with a complex pathology is shared decision making, where the risk of severe complications and medical expenses may be reduced.
