Austrian syndrome is a rare condition caused by invasive Streptococcus pneumoniae, comprising a triad of pneumococcal meningitis,
endocarditis
, and pneumonia. Herein, we report a 59-year-old male patient who presented with fever and tenderness of the right shoulder. Although the initial diagnosis was acromioclavicular joint septic arthritis, the present case showed a reduced level of consciousness, pulmonary infiltrates, cerebral infarcts, and destruction of the mitral valve. This case suggests that acromioclavicular joint arthritis could be an initial presentation of pneumococcal infection inclusive of Austrian syndrome, especially in patients with some risk factors of invasive pneumococcal infections, such as
chronic alcoholism

in the United States (USA). Historically it was linked with trench fever, but cardiac involvement seems to be more prevalent recently. There are some known risk factors associated with
Bartonella quintana
endocarditis
such as human immunodeficiency virus (HIV) infection,
alcoholism
, homelessness and poor hygiene. We report a case of 37-year-old African man, with culture negative
endocarditis
, emboli and rising
B. quintana
and
B. henselae
IgG titers.

DNA was subsequently detected from the mitral valve sample with 16S rRNA gene and ribC primer sets. Eventually, blood culture for
B. quintana
was positive after 21 days. Patient was successfully treated with doxycycline and gentamicin. There have been a few cases of
B. quintana
endocarditis
in the USA and most of them were associated with HIV infection, homelessness or
alcoholism
. The case reported here highlights the importance of high clinical suspicious for
Bartonella species
in blood culture negative
endocarditis
in the USA in appropriate setting and will help to increase awareness among physicians for early diagnosis and treatment.

CASE REPORT This case report involves the presentation, diagnosis, and treatment of a 69-year-old female who presented with new rapidly progressing cavitary lung lesions in the setting of copious administration of systemic steroid use. Given the patient's past history of
alcoholism
and environmental exposure, her case was not straight forward in regard to a diagnosis. Ultimately, she was diagnosed with chronic cavity pulmonary aspergillosis in the setting of chronic immunosuppression secondary to systemic steroid administration. Due to her convoluted medical history and the poor differential diagnosis list, there was a delay in final diagnosis. CONCLUSIONS This case report and clinical review aims to prevent anchoring when the patient's presentation is not straight forward and aims to remind the clinician of the importance of a differential diagnosis.
