Many reports of diabetic ophthalmoplegia have been published from the clinical points of view. However, there have been only three autopsied cases in which the ocular nerves were investigated histopathologically. A 72-year-old housewife was diagnosed to have glycosuria at the age of 67, but no medical treatment was done. She admitted to the hospital, because of acute onset of right eyelid drooping and diplopia for previous four days. She showed complete eyelid ptosis, moderate dilatation of right pupil, loss of light reaction, and extraocular muscle palsy except abduction on the right. Blood pressure was normal. A glucose tolerance test was diabetic and HbA1c was moderately increased. Her diabetes was fairly well-controlled with a diet therapy and injection of lente insulin. Two and a half months after admission, the course of illness became regressive. Seven months later, external ophthalmoplegia was disappeared and only slight anisocoria was seen. She readmitted to the hospital one year and eleven months later, because of anorexia and emaciation. She died of adenocarcinoma of the stomach without chemotherapy. The duration from onset of ocular symptoms to death was two years and one month. At postmortem examination, stomach cancer infiltrated extensively to the abdominal and pelvic viscera, but no metastasis to the nervous system or intraorbital tissues was found. There were mild to moderate atherosclerotic changes in the small-and middle-sized arteries of the kidneys, pancreas and adrenal glands corresponding to her age. A case of selenium poisoning involving eighty approximately nine-week-old piglets is reported. Feed and water were supplied with a dry feed hopper with nipple in the through. Clinical symptoms appeared within ten days after the feed-intake. The symptoms consisted in loss of condition (emaciation), drawn-in thin bellies, long hair, loss of appetite, hard, dark faeces, cerebral symptoms and mortality. Autopsy was performed on a total number of four piglets, in which severe hepatic degeneration was detected. When pig prestarter (creep feed) was substituted for piglet-raising feed (weaner-diet), the condition was gradually restored to normal within a few days.

Ten patients with adrenal cortical carcinoma were treated from 1966 to 1986. There were 7 males and 3 females. The typical clinical manifestations, marked increase of 17-ketosteroid, 17-hydroxycorticoids and DHA, and negative dexamethasone suppression test were essential for the diagnosis. Of the ten patients, eight had secretive function and their 17-ketosteroid and 17-hydroxycorticoids varied from 36.8-93 mg% and 32.5-150 mg%, respectively. DHA was measured in 5 cases with the result of 6.95-44mg%. Those without secretive functions or obvious endocrine disturbances were usually misdiagnosed as kidney tumor, splenomagaly, liver tumor or pancreatic mass. Wood had summarized that nonsecretive ACC patients commonly had fever, pain, exhaustion syndrome (emaciation, fatigue, perspiration, anorexia), mass and distant metastasis. Adrenal scan, IVU, abdominal aortic arteriography, retroperitoneal pneumography and CT were helpful in localization. The differential diagnosis between ACC and adenoma by pathology was difficult. It is generally agreed that if the mass is larger than 100 grams, capsulated, having blood or lymphatic vessel invasion, hemorrhage, necrosis and calcification or even distant metastasis, malignant tumor should be considered. Surgical removal of the tumor is the only effective treatment. For advanced or recurrent lesions, selective adrenal artery thrombosis could be used. One of the ten patients was thus treated by this facilitated subsequent surgery. Postoperative chemotherapy, such as O.P-DDD, might be used in some cases.

Nine dogs with primary gastrointestinal disease had clinical and laboratory findings resembling hypoadrenocorticism. The dogs had histories of anorexia, weakness or lethargy, diarrhea, vomiting, and weight loss. Hypothermia, dehydration, and emaciation also were detected on physical examination. Hyponatremia, hyperkalemia, and abnormally low Na/K ratios were found on laboratory evaluation, but results of ACTH-response tests were not compatible with hypoadrenocorticism. The primary diagnoses were trichuriasis and salmonellosis in 2 dogs, trichuriasis in 5 dogs, and perforated duodenal ulcer in 2 dogs. Most dogs responded to medical or surgical treatment of their primary gastrointestinal disease, and the original electrolyte abnormalities resolved. These findings emphasize the importance of the ACTH-response test in the diagnostic evaluation of dogs with clinicopathologic findings similar to those of hypoadrenocorticism.

Polybrominated biphenyls (PBB) were used as a fire retardant. In common with other halogenated hydrocarbons, PBBs are lipophilic and resistant to chemical and metabolic degradation. Cattle on about 25 Michigan farms were exposed to as much as 250 g per head of PBB when it was accidentally mixed in cattle feed in 1973 to 1974. Livestock exposures several orders of magnitude lower occurred on several hundred other farms because of carryover and equipment contamination in feed mills. Approximately 85% of the Michigan population received some exposure to PBB because dairy product marketing involves mixing milk from many farms. A few cases of high human exposure, which may have been as great as 10 g, occurred when residents of the more highly exposed farms consumed their own products. Although numerous clinical signs and pathological changes were reported in exposed cattle, only anorexia, lacrimation, emaciation, hyperkeratosis, and kidney damage were confirmed in controlled studies. The acute toxicity of PBB in laboratory animals is low, but a variety of subacute effects have been reported. Induction of microsomal enzymes, enlargement and histopathological changes of the liver, fetotoxicity, and immunosuppression are among the more significant. Epidemiological studies of exposed humans have revealed no pattern of clinical signs or symptoms that were related to PBB exposure. A complete evaluation of the human consequences of exposure to PBB await the conclusion of long-term epidemiological studies.

One hundred newborn swamp buffalo calves (Bubalis bubalis) from three villages in North-East Thailand were divided equally into treatment and control groups. Treated calves received two doses of a piperazine/thiabendazole mixture (196.6 and 50 mg/kg bodyweight respectively) at three, and again at ten weeks of age. Control calves received placebo treatments at the same times. Egg excretion rates for the roundworms Strongyloides papillosus and Toxocara vitulorum were recorded as high as 85% and 58% respectively during the first three months of life. During the trial period, four calves in the treated group died and one was treated for coccidiosis, compared to seven deaths and nine additional anthelmintic treatments required in the control group. All calves that either died or required additional anthelmintic treatment showed signs of severe enteritis, anorexia and emaciation that was attributed to parasitic infestation. Calves receiving emergency anthelmintic treatment all recovered uneventfully. All except four of the calves that died or became sick did so between 25 and 40 days post partum, with the remainder developing clinical signs of parasitosis before day 50. There was no difference in weight gains between treated and untreated calves over the trial period (16 weeks). Thus, the advantage of anthelmintic treatments as described at three and ten weeks of age in buffalo calves under village conditions was seen as a reduction in the combined morbidity/mortality rate from 32% to 10% (P less than 0.05). From the pattern of infection observed in this study it is suggested that further gains could be made by giving the second treatment at six (instead of ten) weeks of age, and by increasing the dosage of anthelmintic used.

The historic, physical, laboratory, and histologic findings for 74 cats with chronic renal disease were reviewed. Most cats were older, and no breed or sex predilection was detected. This most common clinical signs detected by owners were lethargy, anorexia, and weight loss. Dehydration and emaciation were common physical examination findings. Common laboratory findings were nonregenerative anemia, lymphopenia, azotemia, hypercholesterolemia, metabolic acidosis, hyperphosphatemia, and isosthenuria. The most common morphologic diagnosis was chronic tubulointerstitial nephritis of unknown cause. The other pathologic diagnoses were renal lymphosarcoma, renal amyloidosis, chronic pyelonephritis, chronic glomerulonephritis, polycystic renal disease, and pyogranulomatous nephritis secondary to feline infectious peritonitis.

A 12-year-old Thoroughbred mare, with a history of anorexia, dramatic weight loss, fluctuating pyrexia and intermittent diarrhoea after an episode of colic, was presented for examination with depression, emaciation and ataxia. Thoracic and abdominal paracenteses yielded copious quantities of inflammatory exudate. Palpation per rectum revealed an enlarged spleen. The primary alterations in haematology included a severe leucocytosis with a left shift, and a hyperproteinaemia characterised by hypoalbuminaemia and hypergammaglobulinaemia. Post-mortem examination revealed a low grade pleurisy and peritonitis with fluid accumulation in both cavities. A suppurative gastritis with full thickness perforations of the stomach wall associated with Gasterophilus intestinalis larvae had extended to the juxtaposed organ initiating an extensive suppurative splenitis. Streptococcus zooepidemicus was cultured.

It is rare to find primary multiple teratomas among intracranial tumors. We report a case of multiple teratomas combined with embryonal carcinoma. A 22-year-old man was admitted to our hospital because of loss of appetite and emaciation. Neurological examination was negative except bilateral optic nerve atrophy and generalized hyperreflexia. The endocrinological study showed panhypopituitarism and partial DI. CT scan revealed irregularly enhanced abnormal masses involving suprasellar cistern and the septum pellucidum. The tumors were removed by the transcallosal approach, and the histopathological study showed mature teratoma which contained cartilage, epithelium of GI tract, ciliated epithelium, smooth muscle, fat tissue. Within the tumor of the septum pellucidum, nests of markedly pleomorphic epithelial tumor cells with necrosis, nuclear pleomorphism and mitosis were identified, embryonal carcinoma. At necropsy, no carcinomatous lesions were identified in other organs. Multiple teratomas from nonmetastatic, separate origins have been rate by reported in the literature. In our case, histologically, the mass in the suprasellar cistern was diagnosed as mature teratoma and the mass of the septum pellucidum was mature teratoma combined with embryonal carcinoma. It seemed likely that each of the tumors was developed from different origins.

A 65-year-old female with general malaise, anorexia and marked emaciation was studied by secretion stimulation tests on 6 anterior pituitary hormones. Only ACTH showed no response and the other 5 hormones responded normally. The basal value of antidiuretic hormone was normal. She was found to be suffered from primary hypothyroidism. Though neither antithyroid antibodies nor other autoantibodies were found, Hashimoto's thyroiditis was confirmed by a thyroid open biopsy. Neoplastic lesions of the hypophysis were ruled out by various X-ray and CT examinations of the sella turcica as well as the brain. The case was concluded to be isolated ACTH deficiency associated with Hashimoto's thyroiditis. The substitution therapy using small doses of adrenocortical hormone and thyroid hormone has kept her well and she has been living normally for these five years.
