Twenty-six cases of leiomyoma of the esophagus, presenting at four centers of Thoracic Surgery were reviewed. Dysphagia was a presenting symptom in 42% but was the only symptom in 15%. Sixty-two percent of patients presented with dyspepsia and 50% had other upper gastro-intestinal tract disorders. Half the lesions were identified correctly on contrast radiography and seen at endoscopy. Tumors were more or less equally found in the lower and middle thirds of the esophagus and varied in size from 2 cm to 13 cm in diameter. All were removed, 88% by enucleation, but the 3 patients who had esophageal resection all developed postoperative reflux esophagitis. Leiomyomas may cause no important symptoms and do not always cause dysphagia even when large. If treated, they should be enucleated.

Pardridge, MD, Professor of Medicine. Several specialists have recently recognized that gastrointestinal reflux causes complications resulting in significant disease. It causes discomfort, indigestion, esophagitis, Barrett's esophagus, and carcinoma of the esophagus. Pediatricians attribute many early pulmonary problems, and even some sudden deaths in infants, to the reflux of gastric contents. Otolaryngologists now recognize that many cases of nonbacterial, nonspecific pharyngitis and laryngitis are due to the reflux of gastrc acid secretions. Contact granuloma and cancer of the larynx may, in some instances, be secondary to nocturnal reflux. Thoracic surgeons and pulmonologists believe chronic tracheobronchitis and some cases of pulmonary disease are attributable to recurrent bathing of the respiratory epithelium by aspirated gastric contents. An awareness of the many complications of gastrointestinal reflux should lead to a multidisciplined attack on the factors responsible for these diseases.

Thoracic aortic dissection (TAD) has a very high mortality rate and is often missed due to the atypical presentation of patients. We present a case of a man with chronic hypertension, atrial fibrillation (AF) (on regular warfarin) and a previous endovascular aneurysm repair (EVAR), who presented with dyspepsia and was incidentally found to have a ruptured distal thoracic aneurysm on imaging with no obvious clinical signs on examination, nor abnormalities on admission chest x-ray (CXR).
