Pepsin 1, the ulcer-associated pepsin, occurred significantly more frequently in the gastric juice of those patients with duodenal ulcer who did not secrete A, B, or H antigens into gastric juice than in those secreting these antigens. Tests of gastric secretion are seldom diagnostic in the investigation of gastric and duodenal ulceration, but they can provide evidence that is helpful in arriving at a diagnosis. Mecillinam is a new amidinopenicillin which is taken orally as its ester pivmecillinam. 10 healthy volunteers were each given 200 mg of pivmecillinam hydrochloride four times daily and all urine was collected and cooled to -20 degrees C to minimize antibiotic degradation. 43% of the ingested antibiotic was recovered from the urine in its microbiologically active form, during the first 12 h and 34% during the subsequent 24 h. The corresponding figures for 10 volunteers receiving double this dose of antibiotic was 30 and 34%. The mean half life of mecillinam in urine due to spontaneous breakdown at 37 degrees C was 17 h. The symptom complex of gallstone
dyspepsia
is defined and then analysed before and after cholecystectomy in 108 patients. Only 46% of patients were symptom-free after operation and 30% were no better. When pyloric function was studied patients with these symptoms before or after cholecystectomy and those with normal radiographs showed duodenogastric reflux, often precipitated by intraduodenal fat. Symptomless matched control subjects showed no reflux. Synchronous radiology and pressure recordings demonstrated that the pylorus in these patients failed to contract in response to a duodenal contraction, whereas the normal pylorus could prevent the reflux produced by an isolated duodenal contraction. The interrelationship of gastrointestinal bleeding, acute perforation, and pyloroduodenal stenosis was examined in a hospital series of 1,457 patients with duodenal ulcers. The proportion of each complication among the total number of ulcer patients after various periods of ulcer symptoms was determined. There was little variation in the proportion with bleeding as the duration of symptoms increased, whereas the proportion with perforation decreased and the proportion with stenosis increased with increasing duration of ulcer

A review has been made of the hospital case notes of 32 patients with indomethacin-associated peptic ulceration seen over a four year period in Dunedin. Ulceration attributed to indomethacin therapy was found to particularly involve elderly women, in contrast to a predominance of males and a younger mean age at diagnosis in non-drug associated ulceration seen over the same period. The indomethacin-associated ulcer patients were more frequently complicated by haematemesis or melaena (two-thirds of cases compared to one-quarter for the group not on drugs) and relative hypochlorhydria was frequently noted at the time of gastroscopy in this group as compared to the other cases. It is suggested that patients warranting prolonged treatment with indomethacin require regular reassessment for any evidence of

48 patients with rheumatic diseases underwent long-term treatment with a new antirheumatic compound, tolfenamic acid pINN. The dosage was 2 capsules of 100 mg 3 times daily. At the time of summing-up 9 patients had been treated for one year, 41 for 6 months and 7 had been eliminated after 1 month of treatment, because of side-effects in the form of diarrhoea,

, vomiting and 1 ulcer patient got an attack of duodenal ulcer. Of the 41 patients who completed the 6 month trial 33 reported good therapeutic effect. A significant fall in the erythrocyte sedimentation reaction (p less than 0.01) was observed. 19 patients reported side-effects in the trial period, but at the end of the trial only 5 complained of side-effects. In the male patients occasional slight dysuria was the most common side-effect. Of the 9 patients who were treated for one year all reported a good effect from the preparation and none of them complained of side-effects after 1 year of treatment. Apart from eosinophilia in 2 patients, who were eliminated from the trial because of diarrhoea, none of the laboratory values showed any signs of the preparation having any toxic effects.

The clinical features of the Mallory-Weiss syndrome were prospectively documented in 130 of 1 667 patients submitted to endoscopy for gastro-intestinal haemorrhage, an incidence of 7,8%. The important clinical features found in these patients were: alcohol abuse (21%); retching or vomiting (38%); salicylate ingestion (36%);
dyspepsia
(39%). The correct clinical diagnosis was therefore often difficult to make. Additional lesions were found in 40% of patients at endoscopy. Blood loss was relatively small, and surgery was not required in any patient. The 2 deaths that occurred were not attributable to haemorrhage. A high index of suspicion and early endoscopy are required to establish the diagnosis.
