-hypersensitive patients with chronic, dyshidrotic hand eczema aggravated by oral challenge with 0.6-2.5 mg
nickel
were treated with 100 mg tetraethylthiuramdisulfide (Antabuse) two to four times daily for 4-10 weeks. Nine of the patients experienced a flare of the dermatitis shortly after initiation of the treatment. During the course of treatment the dermatitis of seven patients cleared, improvement was seen in two patients, and in two the dermatitis remained unchanged. Flare was seen in six patients when the treatment was discontinued. Seven patients experienced side effects such as fatigue, headache and
dizziness
. The treatment of four patients was discontinued due to side effects. During the treatment high levels of
nickel
were found in the serum and urine.

Thirty-eight workers from a factory producing
nickel
-cadmium and other types of batteries came to us for medical evaluation. They included 21 women and 17 men (seniority 2-20 years, age range 31-63 years), and represented a self-selected subset of 700-900 ever-employed and 200+ recently or currently employed workers in the factory. Thirty-four worked on the
nickel
-cadmium assembly line. Symptoms and signs included: headache in 34; weakness, fatigue and lassitude in 26;
dizziness
in 16; pruritus and skin eruptions in 37; gingivitis, teeth loss and caries in 34; nasal congestion, nosebleeds and anosmia in 30; cough, phlegm production, wheezing and shortness of breath in 26; "asthma" in 14; bone pain in 18; urinary frequency, beta 2 microglobulinuria and kidney stones in 17; and sterility or multiple abortions (33) in 8 of 21 women. One additional patient had died from an "amyotrophic lateral sclerosis-like syndrome", while CT scans in six workers revealed brain atrophy. One other worker had leukemia, and two had died from cancer (lung and pancreas). Those who had worked for more than 10 years had more symptoms and signs than shorter-term employees, especially neurological illness, bone pain and urinary tract problems, including beta 2 microglobulinuria. Past blood and urinary cadmium levels were in the range of 1.6-8.7 micrograms/dl and 8-306 micrograms/l, respectively. Our findings indicated that: a) health risks for workers were not confined to the
nickel
-cadmium assembly line or to older workers, b) hazardous exposures still existed and illness appeared in new workers after a clean-up and intervention program, and c) exposures involved increased risks for renal disease and cancers. Finally, there is a need to control exposures and determine health risks in the full cohort of those ever employed, in the workers' children, and in the surrounding environment (air, ground, water) due to the dumping of waste from the plant.

Allergy Syndrome (SNAS). The objective of the study was to describe the nosologic framework of the syndrome and to compare sensibility and specificity for SNAS diagnosis between two different low
nickel
diets - BraMa-Ni and the usually prescribed list of forbidden foods - along with patient adherence to diet. One hundred forty-five patients with suspected SNAS (by history and benefit from
nickel
dietary restrictions) were selected and orally challenged with
nickel
for a definite diagnosis. Specificity and sensibility of the diets were calculated in relation to the results of
nickel
challenges. The nosologic framework of SNAS was deduced from the clinical pictures of 98 patients with positive
nickel
challenge and characterized essentially by skin and gastrointestinal symptoms, whereas all other symptoms (
dizziness
, headache etc.) were never elicited by the oral
nickel
challenge. The specificity and sensibility of BraMa-Ni in detecting SNAS were significantly higher than the forbidden food list diet, with an excellent patient adherence. Therefore, BraMa-Ni diet can be prescribed for the treatment of the syndrome other than for the diagnosis, the gold standard of which remains the oral
nickel
challenge.

Metal workers in urban Kano constitute a major workforce with a considerable population. The present work was aimed at obtaining baseline data on the extent of metal ion concentration in body fluids (urine and blood) of sampled population in the area. The investigation involves interaction with sampled population as well as blood and urine sample collection for heavy metals analysis. The health problems associated with the practice identified by respondents include: metal fume fever; eye and skin irritation;
dizziness
and respiratory problems; lack of or inadequate protective devices during activity were also reported. Laboratory investigation of urine samples by Atomic absorption spectrophotometry indicated higher concentrations for Manganese (Mn), Lead (Pb) and
Nickel
(Ni); in blood samples, there were higher concentrations of Manganese (Mn), Lead (Pb), Chromium (Cr) and
Nickel
(Ni). Metal workers of urban Kano are at risk because of the concentration of Mn and Pb in particular. There is the need to monitor occupational activities that are responsible for pollution and with serious health risk.
