The 159 patients with Parkinson's disease with onset after the age of 50 (mean: 62.3 years) were studied with reference to diagnosis and treatment. The results were as follows: 1. Other than the characteristic features, the symptoms at onset were depression (6% of the patients), lumbago (4%), hemiplegia-like (4%) and dizziness (3%), causing misdiagnoses in some of the patients. Among 159 patients studied, the severity most frequent was Yahr stage 3 (63%) at first examination, indicating the necessity of earlier diagnosis. 2. Magnetic resonance imaging (MRI) of the substantia nigra and striatum was investigated using a 1.5 Tesla field and T2-weighted images, which gave no specific results concerning diagnosis and severity. However, it was useful in differential diagnosis between this disease and parkinsonism caused by multiple system atrophy and cerebrovascular diseases. 3. With 123I-IMP SPECT, decrease in blood flow in the frontal and temporal lobes correlated with the severity (Yahr stage) of the disease. Regarding cognitive functions the scores of Hasegawa's Dementia Scale and Mini-Mental State showed a highly significant correlation with the amount of blood flow in frontal and parietal lobes, suggesting that dementia might be caused by dysfunction of these lobes. 4. In 98 patients treated with levodopa mixed with dopa-economizers for more than a year, the maximum improvement was small in severely disabled patients of Yahr stage 5 and 4 because none improved to stage 3A or below (3A is an arbitrary criterion meaning mild involvement in stage 3 with 3P meaning more severe cases).(ABSTRACT TRUNCATED AT 250 WORDS)

A right-handed, 77-year-old man, complaining of dizziness, was diagnosed as agenesis of the corpus callosum (ACC) by MRI, which demonstrated a complete defect of the corpus callosum (CC) with symmetrical enlargement of the posterior horns of the bilateral ventricles (colpocephy) and focal hypo-plasticity of the frontal lobes. Neurological examination revealed only mild impairment of mental function and poor transfer of tactile form-board learning from the right hand to the left hand. As to the cerebral circulation detected by 123I-IMP SPECT, isotope distribution corresponded to the MRI brain images, though the defects were clearly seen on the medial surfaces of the bilateral frontal lobes, so interhemispheric fissure seemed to be opened widely. IV-DSA revealed hypo-vascularity in the anterior territory of the anterior cerebral arteries. These findings suggested that this localized defect was due to hypo-vascularity and focal hypo-plasticity of the frontal lobes. On the other hand, cine-mode MRI findings that no signal-void phenomenon was found between the third ventricle and the longitudinal fissure indicated no shunt in cerebrospinal fluid.

To study the application of cerebral blood flow scintigraphy with 123I-IMP for the evaluation of central dizziness, the difference in 123I-IMP accumulation between the right and left cerebellar hemispheres was determined as an asymmetry index (A.I.) in 8 normal subjects and compared with A.I. from patients with central dizziness. According to estimated lesions, 123I-IMP findings were compared with CT findings. The normal subjects had A.I. of 5.8 +/- 2.1%, whereas 36 patients with central dizziness had A.I. of 12.7 +/- 7.5%. On the basis of an A.I. of 8% or more as an abnormal finding on IMP scintigraphy, 11 of the 14 patients with estimated lesions of cerebellar hemispheres and 16 of the 20 with those of the whole cerebellum including the vermis were evaluated to have had abnormalities. Seven of 11 with estimated lesions of the brainstem and 4 of 5 with vertebrobasilar arterial insufficiency were evaluated to have had abnormalities. The findings of 123I-IMP cerebral blood flow scintigraphy were in good agreement with clinical symptoms in patients with central dizziness with estimated lesions of the cerebellar hemispheres, suggesting the possibility that this method is more useful than CT.

Propentofylline and pentoxifylline were administered to patients age 65 years or older who were suffering from chronic cerebrocirculatory insufficiency. Changes in clinical symptoms and the level of cerebral blood flow before and after drug administration were compared. 123I-IMP single photon emission computed tomography was used to compare cerebral blood flow before treatment with that 3 months after initiation of therapy. Focal regions of interest were established in the cerebellum, the frontal, temporal, parietal, and occipital lobes, and the thalamus. Comparisons were made between patients age 75 years or older (very old age group) (n = 5) and those age 65 to 74 years (old age group) (n = 7). Complaints of dizziness and slight headache disappeared by the eighth week after the start of therapy in both age groups. Two of the five patients in the very old age group showed elimination of memory disturbance symptoms at the 12th week of treatment. Six of the seven patients in the old age group had no memory disturbance symptoms by the eighth week of treatment, which indicates a high therapeutic effect in this group. In the cerebral blood flow studies, no increases in blood flow after drug administration were observed in the very old age group. This finding is probably related to the poor level of recovery achieved in disturbances in memory in this group. In contrast, improvements in blood flow were observed for all regions of the brain monitored in the old age group, which is consistent with the amelioration of clinical symptoms observed in these patients.(ABSTRACT TRUNCATED AT 250 WORDS)
