Sudden death is a tragic complication of hypertrophic cardiomyopathy. We report the case of a young patient with hypertrophic cardiomyopathy in whom an episode of atrial fibrillation triggered ventricular fibrillation and cardiac arrest. A 21-year-old man with nonobstructive hypertrophic cardiomyopathy underwent cardioverter-defibrillator implantation for primary prevention of sudden death, after risk stratification with noninvasive strategies. After 6 weeks, during a moderate effort, the patient had a syncopal episode, preceded by palpitations and
dizziness
, and terminated by the cardioverter-defibrillator. Device interrogation revealed an episode of atrial fibrillation with high ventricular response, spontaneously followed by ventricular tachycardia/fibrillation. Atrial fibrillation is a potential trigger of life-threatening arrhythmias and sudden death in patients with hypertrophic cardiomyopathy. Clinical investigation of risk markers for sudden death should be encouraged to identify high-risk patients who may benefit from a prophylactic therapy with an implantable cardioverter-defibrillator.

, syncope and unstable angina. He underwent a combined surgical procedure of aorto-bicarotid bypass and off-pump myocardial revascularization. The operation was successful and he had no complications during 11 months of follow-up.

A 53 year-old woman had been followed up since 1996 after receiving bilateral adrenalectomy for the treatment of bilateral adrenal pheochromocytoma in von Hippel-Lindau (VHL) disease. In March 2003, she suffered from cough,
dizziness
and faint, and was referred to our hospital. The radiographic findings exhibited the presence of hypervascular tumors in the left hilum, left apex of the lung and the left neck. Tumor progression was suspected to occlude the left main pulmonary artery and hormonal assays showed elevated levels of the plasma norepinephrine, which may cause fatal complications. Percutaneous needle biopsy of the left cervical tumor revealed metastatic pheochromocytoma. After embolization of the main feeding artery, surgical resection of the tumor resulted in control of excess catecholamine release and symptoms.

Heart failure (HF) is increasing in prevalence. Patient education is essential and is included in both ambulatory and hospital performance measures used to ensure quality care. Nurses are often the primary providers of education to patients with HF. This study assessed nurses' knowledge of basic principles of HF self-management. The study surveyed 49 nurses who regularly provided care to patients with HF at a hospital in the southeastern United States. A 20-item, true/false survey was administered to participants. Mean HF self-management knowledge score was 15.97 (79.85% correct). Consistent with previous studies, nurses scored lowest on knowledge related to transient
dizziness
(16.3% answered correctly), daily weight monitoring (36.2% answered correctly), and asymptomatic hypotension (58.3% answered correctly). Findings confirm previous work suggesting that nurses may not be adequately prepared to educate patients with HF about self-management.

Accessory mitral valve (AMV) is a rare congenital abnormality that, rarely, causes left ventricular outflow tract (LVOT) obstruction in adults. We report the case of a 47-year-old man with deteriorating exertional

. Evaluations revealed that the left ventricular outflow tract obstruction was caused by the accessory mitral valve. The patient underwent a successful operation for removal of the accessory mitral valve.

Carotid endarterectomy is the standard treatment for carotid stenosis, but carotid artery stenting has emerged as a potential alternative. Elective carotid artery stenting was performed in 42 patients aged 42 to 79 years (mean, 67.05 +/- 8.67 years) after ultrasonography, computed tomography, magnetic resonance angiography and a neurological evaluation. There was bilateral carotid stenosis in 23 patients (55%), with > 90% stenosis in 18 vessels. All patients had significant associated coronary lesions. An emboli protection device and self-expanding stents were used. One year later, the patients were evaluated by Doppler sonography and selective angiography. Technical success was achieved in all procedures. During follow-up, 1 (2.4%) patient died from myocardial infarction, 1 underwent coronary artery bypass and 14 (40%) had minor complaints including occasional
dizziness
. No other neurological events were noted. Restenosis was found in one case, but selective angiography ruled out a significant lesion. One patient suffered embolization, but recovered completely within 24 hours. In 7 (17%) patients with type C arch interruption and a tortuous carotid course, stenting was successful and they had no embolization or restenosis. Carotid artery stenting is recommended in high-risk patients.

Cardiac hydatid disease is very rare, even in endemic regions. Clinical manifestations included chest pain, anaphylactic shock, constrictive pericarditis, congestive heart failure, and arterial embolism. Surgery is the exclusive therapy, where the cysts are excised during open-heart surgery. The surgical approach therefore must be performed carefully, given the potential complications that surgery may bring. Because of the risk of potentially lethal complications, early diagnosis and definitive treatment are important. A 32-year-old male patient was admitted with chest pain, weight loss, lethargy, and
dizziness
. On the transesophageal echocardiography study, a cystic mass (2.5 x 3 x 4.5 cm in dimension adjacent to the left ventricular posterior wall) that was divided into two by a septum was noted. Diagnosis of hydatidosis was confirmed with serologic tests (ELISA and indirect immunofluorescence). Echinococcosis, also known as hydatid disease, is common in several regions of the world, for example, the Mediterranean countries, the Middle East, South America, and East Africa. While performing pericystectomy in the anterior left ventricular wall, we noticed that there were three cysts, contrary to the preoperative diagnosis pointing a single one, and it was impossible to effectively complete the procedure without compromising anterosuperiorly displaced left anterior descending artery (LAD). We decided to go on bypass, arrest the heart, and complete the pericystectomy at the cost of injuring LAD and grafting the left internal mammary artery to LAD. Microscopic examination of the cyst showed a germinal layer and an avascular, eosinophilic, chitinous layer that confirmed the diagnosis of hydatid cyst. The patient was discharged on the fifth postoperative day on albendazole medication.

Primary osteosarcoma of the heart is an extremely rare entity. In this report, we describe a case of primary osteosarcoma of the heart that recurred and metastasized. The patient is a 50-year-old woman who presented with an abrupt onset of dyspnea,
dizziness
, and palpitations. Echocardiography results showed a left atrial tumor that was resected and had histological features of high-grade osteosarcoma. The patient was treated with adjuvant chemotherapy. The tumor recurred in the same location 4 years later and was resected. Three years later, the patient presented with a 4-cm polypoid mass in the stomach that was consistent with metastatic osteosarcoma. One year later, the patient presented with recurrent high-grade sarcoma in the soft tissue of the left chest wall. We herein present the spectrum of histological findings and molecular genotyping data.

Elderly patients frequently suffer from
dizziness
and syncope; however, an underlying disease may not always be identified. Three patients aged 69, 71 and 56, respectively, experienced spells of
dizziness
and syncope. Massage of the carotid sinus demonstrated the presence of a carotid sinus syndrome (CSS), an abnormal baroreflex response of the carotid sinus that leads to asystole and extreme hypotension. Conventional treatment is generally by insertion of a pacemaker. These patients, however, were referred to the vascular surgery department of our hospital for removal of adventitial layers of proximal portions of the internal carotid artery. Recovery was uneventful; all three are now free of symptoms. CSS should be considered in the differential diagnosis of
dizziness
and syncope. Surgical denervation of the carotid artery is a valid treatment option, especially in the vasodepressive or mixed type of CSS.

Clopidogrel is a thienopyridine antiplatelet agent that inhibits adenosine diphosphate-dependent platelet activation and aggregation and is currently one of the most widely prescribed antiplatelet drugs for the treatment of symptomatic coronary artery disease. Several large clinical trials have demonstrated that it has a potent protective effect against adverse vascular events, including coronary, cerebral, and peripheral arterial disease. These clinical trials have reported among other clopidogrel's adverse effects, the adverse effects related to the central nervous system including headache and
dizziness
occurring in 7.6% and 6.2% of patients, respectively. Aim of this study is to detect possible effect of clopidogrel on human psychomotor performance in healthy volunteers. Using a double-blind, placebo-controlled, balanced, between-subject design at laboratories of the pharmacology department in Al-Mustansiryiah University Medical College, Baghdad, Iraq, during the academic year 2007-2008. Fifty-four young healthy volunteers were enrolled, they received either single oral doses of clopidogrel 37.5 mg (Plavix; Sanofi-Synthelabo) or placebo. Each treatment was given to 27 subjects. Measurements of psychomotor performance including the choice reaction time and its 2 components, recognition reaction time and movement reaction time, and the critical flicker fusion threshold using the computerized Leed psychomotor tester were performed just before medication and 2 hours afterward. No statistically significant difference in the parameters measured was detected among the two treatment groups. In this study, we found that single oral doses of clopidogrel in our group of normal healthy volunteers did not affect their psychomotor performance.
